Overview

 

Applications are sought for a Consultant in Vascular Surgery at West Irelands University Teaching Hospitals. This is a replacement post to maintain the current team of 6 full time Consultants. The post will be based at a University Hospital.

 

The post offers an exciting opportunity to support the expanding and developing vascular services offered by a busy department with a strong record in teaching and training. This is an excellent opportunity to join a friendly and cohesive team of 6 consultant vascular surgeons and 6 consultant radiologists, serving a combined population of just over 540,000 working across two main sites.

 

The appointment is on a whole-time basis but applications are welcome from those who wish to job share.

Any person who is unable for personal reasons to work full-time will be eligible to be considered for the post. If such a person is appointed, modification of the job content will be discussed on a personal basis, in consultation with consultant colleagues. Suitably qualified job-share candidates will receive serious consideration.

 

Specific Duties

The successful candidate will be a full time appointment in Vascular Surgery and will work within the pre-existing Department team and the wider Multi-disciplinary team. Team working qualities will be a mandatory requirement of the appointees. It is expected that the post holder will work closely with radiology, integrated care colleagues and local GPs to maintain and develop local outpatient and vascular surgical services, maintaining patient care close to home as far as possible.

The appointee will be expected to provide the full range of vascular services and commitment to the 1 in 6 vascular emergency on-call rota is a core part of the role. They will work within a team to provide cross cover for leave.

The post will be based at a University Teaching Hospital and the appointee will be responsible for the care of patients referred by General Practitioners and in-patient referrals and will undertake clinics, ward rounds, in-patient care, day case operating lists on the secondary site and at clinic sites determined by the job plan.

The post holder’s duties will be split between a University Teaching Hospital, and General Teaching Hospital and a number of peripheral clinics located across Connacht, though primarily based at the University Teaching Hospital. It is a condition of the appointment that the post holder will be willing to work at any of the HSE’s sites.

Post Details

This is a post for a consultant vascular surgeon. A subspecialist interest such as endovascular, deep venous or diabetic limb reconstruction would be encouraged and supported. The post holder will form part of the arterial centre’s team of 6 Vascular Surgeons and 6 consultant interventional radiologists. New Consultant appointments will be provided support and mentorship by senior colleagues in accordance with the hospital’s mentorship programme.

·       Be expected to provide a patient centred service, respecting the privacy, dignity and choice of individual patients. They will be responsible with Vascular and Radiology colleagues for the provision of outpatient and inpatient services within the West Ireland Hospitals Group for both routine and emergency attenders.

·       Aim to deliver high quality care with the help of the existing surgical and interventional radiological team.

·       Assist consultant colleagues in the continued development of the Vascular Arterial Centre and contribute to service development.

·       Be involved in complex clinical, patient management problems and multi-disciplinary planning treatment.

·       Share in administrative duties allocated by mutual agreement within the Directorate.

·       Ensure the dictation of clinical letters is completed within 24 hours of a consultation.

·       Observe and comply with clinical and non-clinical policies and procedures (including local and national employment and personnel policies) agreed with employees and management. In particular, the post holder will be responsible for updating policies and guidelines for the vascular service along with colleagues, which should be reviewed annually.

·       Be closely involved in clinical audit, research and clinical governance.

·       Provide support to the complaints process (this may include attending complaint meetings with patients, families and their relatives).

·       Undertake all mandatory training, including annual safeguarding training (in-house)

Teaching Commitments

·       regular commitments to ward based teaching of undergraduates.

·       participation in the lecture programmes for 2nd and 4th & 5th year medical students.

·       teaching of other groups as required including other specialty juniors and specialist nurses.

 

Job Plan

A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead Vascular Surgery Consultant, on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.

It will be reviewed at 3 months & thereafter annually or at any time, but no less than 3 months after a previous review, as requested by the appointee, Operational Medical Director or Clinical Lead & adjusted accordingly to the agreement of both parties.

Standard Duties and responsibilities

a) To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan  and in line with policies as specified by the Employer.

b) To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.

c) To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.

d) To co-operate with the expeditious implementation of the Disciplinary Procedure 

e) To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.

f) To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.

g) To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.

h) To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.

i) To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.

j) To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 

k) To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.

l) To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery

 

Service Provision and Cover during Absences

 

The consultant has a continuing responsibility for the care of patients in his/her charge and for the proper functioning of the service allowing for appropriate delegation for the training of his/her staff.

 

He/she must be available by telephone and able to attend the hospital in a timely fashion when on-call (40 minutes journey time).

 

Consultant staff must ensure that services are covered during his/her planned absences. Where fixed commitments need to be cancelled during planned absences appropriate notice (8 weeks) must be given. Absences must be co-ordinated with other consultant staff in the specialty to ensure senior cover is always available.


Essential Requirements

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of General Surgery - Vascular Surgery.

Office Accommodation

The post holder will undertake administrative work associated with their clinical and other professional duties.  Adequate time and facilities for clinical administration, including appropriate office space, secretarial support and access to a personal computer, software & internet access, will be available.


Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant  will offer mentoring support and lead the existing clinical team through; joint clinics, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.
