Overview
A full-time Consultant Child and
Adolescent Psychiatrist is required to work within and support the development
of the Complex Mental Health Pathway of our client is the Connacht Region. This
Pathway will support young people presenting with moderate to severe
depression, anxiety, Obsessive Compulsive Disorder, Trauma and other mental
health conditions. Whilst a number of pathways exist or are in development
there will be flexibility and fluidity between the pathways to wrap around the
young person and family to provide the type and level of care required
reflecting the fact that many young people present with a number of
comorbidities.
It is envisaged the 10 sessions
would be worked over five days. There is the potential to develop additional
areas of interest, for example, in other pathways, teaching or research. The
post holder will work alongside other consultants in the pathway and the
clinical service lead to support development ultimately of an Integrated
Pathway involving partner agencies in Health, Education, Social Care and the
Third Sector. This work will align closely with the Commissioner led 5 year
Transformational Plan and their work streams looking at Early Intervention in
mental health with a focus on early years, an integrated neurodevelopmental
pathway, the development of a Tier 3 Service, and a reduction of usage of
Hospital Services. The hospital is a dynamic compassion-focussed Service with a

Applications
are invited for a full time Consultant in Child and Adolescent Psychiatry post
working in the Complex Mental Health Pathway. The post is 10 sessions and it is
envisaged the sessions would be worked over five days but flexibility is
possible including evening clinics if wished. Part time applications are
welcome. The successful candidate will be committed to the highest standard of
clinical practice and be willing to develop strong links with colleagues in
Children’s Health Services, GP’s, partners in Education, Social Services, the
wider Trust, Commissioners and the 3rd Sector.
The post
holder will be based at a very pleasant base near the Town Centre with good
parking. The post holder is integral to the multi-disciplinary team and will
provide medical and clinical leadership working closely with other Consultants
and the Pathway Clinical Lead/Manager to provide strategic development to the
Team. This Pathway will support young people presenting with moderate to severe
depression, anxiety, Obsessive Compulsive Disorder, Trauma and other mental
health conditions. Whilst a number of pathways exist or are in development
there will be flexibility and fluidity between the pathways to wrap around the
young person and family to provide the type and level of care required
reflecting the fact that many young people present with a number of
comorbidities. The final Consultant sessional commitment to the pathway is
being finalised but currently we are working to approximately 4.0 WTE. The Post
Holder will be primarily be responsible for delivering a quality clinical
service through direct work with patients, Consultation and liaison to other
professionals in the Team, MAT, Social Care, Education, General Practice,
Public Health Nursing and Community Paediatrics. This supports other
professionals to manage young people with Emotional and Mental health problems
and is an important strategy to avoid unnecessary referral to Tier 3 CAMHS.
All new
referrals are discussed in the Single point of Access though some young people
may enter the Service through rapid access via the RISE Team. As one of the
original pilots for CY-IAPT outcome measures, incorporating real time feedback,
are now standard for the Service. The use of goal based outcomes and SPOA has
led to a reduction of referrals to CAMHS, more targeted treatment with shorter
length of stay in service hence we are currently the only Service in the region
where referral rates are equivalent to discharge rates.
Additional
Information
You will be
able to take advantage of the excellent Consultant contract for the Republic of
Ireland. The basic salary range for this post is euros 120,000 per annum to
euros 160,893 per annum plus a further euro 26,672 per annum available in
Commitment Awards.

Job Plan
A formal job plan will be agreed between the appointee, the Operational Medical
Director and Lead Psychiatry: Child & Adolescent Consultant, on behalf of
the Trust’s Medical Director, based on the provisional timetable. This will be
signed by the Chief Executive and will be effective from the commencement date
of the appointment.
It will be reviewed at 3 months & thereafter
annually or at any time, but no less than 3 months after a previous review, as
requested by the appointee, Operational Medical Director or Clinical Lead &
adjusted accordingly to the agreement of both parties.

Cross Cover and On-Call Arrangements
The post holders will be one of 10 Consultant
Child and Adolescent Psychiatrists (7 of whom are part-time) employed by the
hospital. The post holder will be required to participate in reciprocal pro
rata medical cover (arrangements to be agreed with consultant colleagues). The
post holder will be part of a second on-call rota (first on-call offered by
Core Trainees and CAMHS) covering evenings and weekends (1 in 9) and a daytime
rota offering advice to CAMHS clinicians and the team (1 in 9). The Consultants
work together very amicably to ensure leave can be accommodated and that the
Service needs are met. During the daytime on call the post holder offers
medical and clinical advice to any of the CAMHS teams if the Team Consultant or
junior doctor is not available to clinicians.
On rare occasions this may involve Mental Health
Act or other emergency assessments. The Team now has a 0.5 WTE consultant
therefore it is anticipated that the day time rota commitment will reduce. At
night the Consultant is second on call and offers advice to the junior doctor
on call for Adults and Children and the Staff who operate 7 days a week between
8am to 11pm. The majority of requests for advice occur during daytime hours at
the weekends as the majority of young people under 16 years who self-harm are
still admitted overnight to the Children’s Hospital. Occasional assessments to
support RISE and the junior Doctors may occur in the Emergency Department. The
junior will contact the psychiatrist pre and post-assessment to discuss, the Team
will contact if they need advice rather than every assessment. Rarely,
emergency assessments are required such as Mental Health Act Assessments.
a) To participate in development
of and undertake all duties and functions pertinent to the Consultant’s area of
competence, as set out within the Clinical Directorate Service Plan and in line with policies as specified by the
Employer.
b) To ensure that duties and
functions are undertaken in a manner that minimises delays for patients and
possible disruption of services.
c) To work within the framework
of the hospital / agency’s service plan and/or levels of service (volume, types
etc.) as determined by the Employer. Service planning for individual clinical
services will be progressed through the Clinical Directorate structure or other
arrangements as apply.
d) To co-operate with the
expeditious implementation of the Disciplinary Procedure
e) To formally review the
execution of the Clinical Directorate Service Plan with the Clinical Director /
Employer periodically. The Clinical Directorate Service Plan shall be reviewed
periodically at the request of the Consultant or Clinical Director / Employer.
The Consultant may initially seek internal review of the determinations of the
Clinical Director regarding the Service Plan.
f) To participate in the
development and operation of the Clinical Directorate structure and in such
management or representative structures as are in place or being developed. The
Consultant shall receive training and support to enable him/her to participate
fully in such structures.
g) To provide, as appropriate,
consultation in the Consultant’s area of designated expertise in respect of
patients of other Consultants at their request.
h) To ensure in consultation with
the Clinical Director that appropriate medical cover is available at all times
having due regard to the implementation of the European Working Time Directive
as it relates to doctors in training.
i) To supervise and be
responsible for diagnosis, treatment and care provided by non-Consultant
Hospital Doctors (NCHDs) treating patients under the Consultant’s care.
j) To participate as a right and
obligation in selection processes for non-Consultant Hospital Doctors and other
staff as appropriate. The Employer will provide training as required. The
Employer shall ensure that a Consultant representative of the relevant
specialty / sub-specialty is involved in the selection process.
k) To participate in clinical
audit and proactive risk management and facilitate production of all
data/information required for same in accordance with regulatory, statutory and
corporate policies and procedures.
Service
Provision and Cover during Absences
The consultant has a continuing responsibility for the
care of patients in his/her charge and for the proper functioning of the
service allowing for appropriate delegation for the training of his/her staff.
He/she must be available by telephone and able to
attend the hospital in a timely fashion when on-call (40 minutes journey time).
Consultant staff must ensure that services are covered
during his/her planned absences. Where fixed commitments need to be cancelled
during planned absences appropriate notice (8 weeks) must be given. Absences
must be co-ordinated with other consultant staff in the specialty to ensure
senior cover is always available.
Essential Requirements
This post is open to doctors who hold Registration
as a Specialist in the Specialist Division of the Registrar of Medical
Practitioners maintained by the Medical Council in Ireland in the specialty of
Psychiatry: Child & Adolescent
Office Accommodation
The
post holder will undertake administrative work associated with their clinical
and other professional duties. Adequate time and facilities for clinical
administration, including appropriate office space, secretarial support and
access to a personal computer, software & internet access, will be
available.
Management Responsibility
All Consultants are required to attend the monthly
Directorate Meetings. Post holders will be expected to share in administrative
duties allocated by mutual agreement within the Directorate. As part of the
Consultant Team, the consultant will
offer mentoring support and lead the existing clinical team through; joint
clinics, one-to-one support, individual case management and complex cases,
identifying development needs and suggesting CPD (Continuous Professional
Development) opportunities and responding to clinical enquiries via e-mail and
telephone.
