Overview

 

We’re recruiting a permanent consultant paediatric haematologist to join a team in our clients university regional paediatric oncology/haematology unit at University Hospital in the Connacht Region. This is a replacement post, providing a tertiary service for both malignant and non-malignant haematology in the Connacht Region

ABOUT THE SERVICE

Tertiary services include the diagnosis and management of children with leukaemia, bone marrow failure syndromes and myelodysplasia. High dose chemotherapy with autologous stem cell rescue is performed and there is a paediatric apheresis service for stem cell collection.

The hospital is a designated comprehensive care centre for haemophilia, providing a regional service for both adults and children with congenital bleeding disorders and thrombotic problems. There is an expanding ethnic population affected by major haemoglobin disorders. The hospital provide all aspects of care as recommended in the national guidelines, including transcranial Doppler screening and erythrocytapheresis.

The haematology laboratory at the University Hospital is highly automated and provides a comprehensive range of routine and specialist services including malignant haematology, molecular pathology, thrombosis and haemostasis and HPLC for haemoglobin disorders. There will be an opportunity to participate in the development of the malignant haematological diagnostic service.

WHAT YOU’LL DO

You’ll join a team of four paediatric oncologists and a paediatric haematologist. Details of duties and responsibilities can be found in the attached documents, but amongst them, you’ll be expected to advise on haematological problems within the various sub-specialities. The hospital offers ample opportunities for undergraduate and postgraduate teaching and excellent facilities for research and clinical care.

WHAT WE’RE LOOKING FOR

You’ll be a well-motivated individual with a broad general interest in all aspects of paediatric haematology, both clinical and laboratory. You must have a CCT or be on the HSE’s Specialist Register, however consideration will also be given to applicants who are within six months of obtaining their CCT on the date of the AAC.

As well as the knowledge, clinical, and technical skills required to provide a consultant led service in paediatric haematology, you’ll be expected to have:

·       MRCPCH/FRCPath or equivalent

·       good leadership and interpersonal skills

·       completed specialist training in paediatric haematology

And experience in:

·       management of acute haematological emergencies

·       paediatric haematology including neonatal – two years

·       haematological laboratory investigations including morphology

·       clinical skills in bone marrow aspiration/biopsy and lumbar puncture

·       malignant and nonmalignant haematology, haemostasis and blood transfusion

Applicants will be expected to demonstrate their competence to communicate in spoken and written English.

Specific Duties

Malignant Haematology

The service is responsible for the diagnosis and management of children with acute leukaemia, bone marrow failure syndromes and myelodysplasia. 120 new cases of malignant diseases are seen each year of which 35 are acute leukaemia. There are 2 GA lists weekly for performing bone marrows and lumbar punctures. Patients requiring allogeneic stem cell transplant are referred to Bristol Children’s Hospital and care following discharge is provided at the University Hospital.

High dose chemotherapy with autologous stem cell rescue is carried out in the department and one of the paediatric haematologists is the HTA licence holder for stem cell procurement. On average 5 autologous transplants are performed annually.

Clinical Trials

Activity The department partakes in national and international clinical trials which include leukaemia and haemophilia. The post holder will be expected to be a PI for some of these trials.

Non-malignant Haematology services include:

Consultative haematology for Child Health, PICU, NICU and cardiology at UHS, for local GPs and the district hospitals

Haemostasis and Thrombosis

The hospital is a designated comprehensive care centre for haemophilia, providing a regional service for adults and children with congenital bleeding disorders and thrombosis. A total of 110 paediatric bleeding disorder patients are registered, 40 with haemophilia of which 15 are severe. There is an established multidisciplinary team of 2 adult haematologists, 2 paediatric haematologists, 2 nurse specialists and data manager. A monthly paediatric haemostasis clinic is held as well as a joint weekly MDT. Outreach haemophilia clinics are held at nearby facilities

Haemoglobinopathy services

This is a tertiary service for the region and a total of 45 paediatric cases with major haemoglobin disorders are reviewed. All aspects of care can be provided including transcranial doppler screening. The facility for automated erythrocytapheresis is located on the renal dialysis unit. There is a dedicated monthly haemoglobinopathy clinic. The clinical nurse specialist is attached to the paediatric and adult services. Ferriscans are carried out.

Outreach Clinics

In addition to the outpatient clinics held at the University Hospital, the post holder will do 2 general haematology/leukaemia clinics per annum and share the paediatric haemophilia clinics at the district hospitals.

Laboratory Haematology and Blood Transfusion

The laboratory services fall under Laboratory Medicine which is led by a triumvirate comprising the Clinical Lead for Pathology, Lead Clinician for laboratory Haematology and Blood Transfusion and an Operations Manager.

The laboratories provide services 24 hours a day, 7 days a week supporting the activity of the University Hospital and Community Mental Health Facilities, supporting approximately 1100 beds including a large contingent of specialist ITU and HDU beds in Paediatrics, neonatal care, general medicine and surgery, bone marrow transplantation, neurosurgery andcardiothoracic surgery. In 2014 this equated to over 9 million haematology tests.

The laboratories are well equipped with a wide range of automated systems and are fully accredited. They are EQA registered with NEQAS schemes for Haematology, Coagulation and Blood transfusion covering a range of over 25 tests. The main haematology analysers are 4 Sysmex XN FBC analysers, and in coagulation 2 ACL TOP700 CTS analysers. Erythrocyte sedimentation rate is measured by the Starrsed ESR . Malaria screening is by manual microscopy and the CareStart antigen test.

Teaching Commitments

·       regular commitments to ward based teaching of undergraduates.

·       participation in the lecture programmes for 2nd and 4th & 5th year medical students.

·       teaching of other groups as required including other specialty juniors and specialist nurses.

 

Job Plan

A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead Paediatric Consultant / Pathologist, on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.

It will be reviewed at 3 months & thereafter annually or at any time, but no less than 3 months after a previous review, as requested by the appointee, Operational Medical Director or Clinical Lead & adjusted accordingly to the agreement of both parties.

The 2 SPA within this job plan should be used for personal development, teaching and training of other health professionals, service review and development of clinical services and the regional network. This may include the maintenance of clinical competencies in general and acute paediatrics. There are many opportunities for teaching at undergraduate level, and postgraduate teaching in primary care and as part of the regional network. There are opportunities to develop a research portfolio and to contribute by recruitment to commercial and networked research studies.

 

On call

 

The post holder will contribute to the non-residential Paediatric Oncology / Haematology on call rota (1in 7 weekends, 1 in 7 weekdays) .

 

Standard Duties and responsibilities

a) To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan  and in line with policies as specified by the Employer.

b) To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.

c) To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.

d) To co-operate with the expeditious implementation of the Disciplinary Procedure 

e) To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.

f) To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.

g) To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.

h) To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.

i) To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.

j) To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 

k) To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.

l) To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery

 

Service Provision and Cover during Absences

 

The consultant has a continuing responsibility for the care of patients in his/her charge and for the proper functioning of the service allowing for appropriate delegation for the training of his/her staff.

 

He/she must be available by telephone and able to attend the hospital in a timely fashion when on-call (40 minutes journey time).

 

Consultant staff must ensure that services are covered during his/her planned absences. Where fixed commitments need to be cancelled during planned absences appropriate notice (8 weeks) must be given. Absences must be co-ordinated with other consultant staff in the specialty to ensure senior cover is always available.

 

Essential Requirements

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of Pathology

Office Accommodation

The post holder will undertake administrative work associated with their clinical and other professional duties.  Adequate time and facilities for clinical administration, including appropriate office space, secretarial support and access to a personal computer, software & internet access, will be available.


Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant  will offer mentoring support and lead the existing clinical team through; joint clinics, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.
