Overview
A whole-time post as maternity cover locum appointment for Consultant in Paediatric Oncology is available now at our clients Hospital/Children’s Hospital in Connacht.
The post is for 10.0 programmed activities (PAs).
Services:
The
Paediatric and Adolescent Haematology/Oncology unit provides a holistic service
for patients within the HSE Cancer Alliance. The unit provides all aspects of
solid tumour oncology, neurooncology, malignant haemato-oncology, benign
haematology and haemostasis/thrombosis care, apart from allogeneic
transplantation. The hospital has Joint Accreditation Committee-ISCT & EBMT
(JACIE) accreditation for the administration of high dose chemotherapy with
autologous stem cell rescue. Patients requiring allogeneic transplantation are
referred to Dublin services. The unit provides supra regional support for
haemophilia and Haemoglobinopathy services. The Children’s Hospital at the
hospital campus is the Principal Treatment Centre (PTC) providing tertiary
haematology/oncology services for designated Paediatric Oncology Shared Care
Units (POSCUs).The unit has excellent relationships with paediatricians in the
POSCUs. Outreach clinics for Paediatric Oncology and Haematology are held at
each of the POSCUs.. This dedicated 9-bedded ward provides care for patients
requiring chemotherapy or for the management of treatment- or disease-related
complications. Other wards within the hospital are used to accommodate outliers
when required. All surgical, laboratory and radiological services are
co-located in the hospital site. All Paediatric Haematology and Paediatric
Oncology services are based at the hospital site, with integrated oncology and
surgical services. There are approximately 115 new referrals per year of
children with malignant disease. The number of referrals has been increasing
year on year.
Haemato-oncology
and Solid tumour oncology service:
The service
has weekly malignant haematology multidisciplinary team (MDT), solid tumour
& lymphoma, Teenage and Young Adult (TYA), psycho–oncology, neuro-oncology
and sarcoma meetings. Patients are reviewed and management plans enacted at
these meetings. All MDTs are well supported by integrated surgical oncology,
pathology and diagnostic radiology teams. The hospital is the fourth largest
primary treatment centre for the treatment of leukaemia patients and all
chemotherapy is provided except bone marrow transplant, where the majority of patients
are referred to Dublin. The regional solid tumour oncology service manages all
solid tumours (including lymphoma) and CNS tumours for the Cancer Alliance
(CA). The Paediatric Neuro– oncology service is well supported with 4 fulltime
Paediatric Neurosurgeons. The neurosurgical department provides nationally
recognised expertise in tumour surgery, complex hydrocephalus surgery,
neuro-trauma, spasticity, complex spine and craniofacial subspecialties. The Sarcoma
Service (Sarc) is 1 of the 5 nationally commissioned services for the
management of bone sarcomas and limb girdle sarcomas. Patients between the ages
of 0 – 19 years are managed at designated Young People Primary Treatment
Centres (PTC)
Haemostasis
and thrombosis service:
The
Paediatric service is part of the National Haemophilia Comprehensive Care
Centre with the Paediatric Haemophilia Service. The team is consultant-led and
within working hours care is delivered by two Paediatric Haemophilia
consultants, 3 Haemophilia nurse specialists and a dedicated physiotherapist.
The service is actively involved in Haemophilia clinical trials and the use of
novel approaches in haemostasis. There is a regular consultation programme for
haemostasis/thrombosis advice to the Paediatric Cardiology, Paediatric
Gastroenterology and Paediatric and Neonatal Critical Care services. There is a
separate Haemophilia on-call rota currently staffed by Paediatric Haematology
Consultants. On-site out-of-hours assessments and management is provided by the
on-call adult Haematology Specialist Trainees, under the supervision of the
Paediatric Haematology Consultant on-call for paediatric haemophilia.
In-patient care is provided by Haematology Specialist Trainees/Nurse Specialist
under the direction of the Paediatric haematology consultants.
Haemoglobinopathy
service:
There is a specialist
centre for haemoglobiopathy patients in the Network and includes the management
of patients in Connacht. There are more than 150 patients cared for in the
network. Care of complex patients, including those undergoing surgery is
coordinated as well as admission to PICU for emergency red cell exchange.
Annual reviews, including transcranial Doppler are performed. The service is
lead by a Paediatric Haematology consultant. The patients are transitioned as
per “the ready steady go programme” into adult services in the region and local
adult haematology services.
Transfusion
service:
There is a
paediatric transfusion committee which supports and audits paediatric
transfusion practice and is lead by a paediatric haematology consultant and
transfusionist. Education is provided to all staff who prescribe or transfuse
blood products.
Specific Duties
Clinical:
The
successful candidate will predominantly be responsible for the management of
patients with haematological and/or haemato-oncological conditions aged 0 – 19
years, but the precise role is dependent on experience and training. The post
holder will participate in the 1 in 6 on-call system, providing cross cover for
in-patient activity, dealing with new referrals, initial diagnostic work-up,
planning and delivery of treatment and care. If the candidate is not an
accredited Paediatric Haematologist, provision of haematological care will be
limited to malignant haematology.
Paeditric
Oncologists are welcome to apply and the job plan can be negotitated to provide
for the needs of the service and the candidates interests. The post-holder will
work as an integral member of the team, attending and contributing to the
established multidisciplinary team (MDT) meetings.
The
post-holder will share responsibilities to review patients on the day unit,
supporting the fellows and staff grades, and independently lead outpatient
clinics. The postholder will provide advice and support to the other paediatric
specialists within the Children’s Services and paediatricians.
Haemophilia
support and advice is provided by separate on-call rota. These duties are
subject to review from time to time in the light of the changing requirements
of the service. If alterations to the described duties are required these will
be mutually agreed between the appointee, his/her Consultant colleagues, and
the Trust. The on-call weekend ward round is delivered by consultants, who will
review all haematology and oncology inpatients and provide telephone advice for
patients in POSCUs. Cross-cover of consultant colleagues is expected for
periods of annual/study leave and sickness.
Laboratory
skills:
Laboratory based duties will depend on the candidate’s experience. Candidates
with laboratory haematology expertise will provide interpretation and report
blood films, bone marrow aspirates and CSF cytology.
Outpatient Care:
On-treatment outpatient care is delivered on the Day Care Centre, a separate
area for the administration of chemotherapy, blood products and assessment of
patients. The successful candidate will be expected to see patients and support
the Speciality / Registrar doctors in Day Care. Follow-up clinics for patients
not on active treatment occur in the dedicated Children’s Outpatients.
Children’s
Outpatients is also the site for haematology follow up and benign haematology
clinics, e.g. haemoglobinopathy. A large and active Paediatric Haemophilia
service is based in the outpatients clinic at the hospital.
Teaching Commitments
·
regular commitments to ward based teaching of
undergraduates.

participation in the lecture programmes for 2nd
and 4th & 5th year medical students.
·
teaching of other groups as required including other
specialty juniors and specialist nurses.
Job Plan
A formal job plan will be agreed between the appointee, the Operational Medical
Director and Lead Paediatrician: Oncology Consultant, on behalf of the Trust’s
Medical Director, based on the provisional timetable. This will be signed by
the Chief Executive and will be effective from the commencement date of the
appointment.
It will be reviewed at 3 months & thereafter
annually or at any time, but no less than 3 months after a previous review, as
requested by the appointee, Operational Medical Director or Clinical Lead &
adjusted accordingly to the agreement of both parties.
Summary of main duties:

Standard Duties and responsibilities
a) To participate in development
of and undertake all duties and functions pertinent to the Consultant’s area of
competence, as set out within the Clinical Directorate Service Plan and in line with policies as specified by the
Employer.
b) To ensure that duties and
functions are undertaken in a manner that minimises delays for patients and
possible disruption of services.
c) To work within the framework
of the hospital / agency’s service plan and/or levels of service (volume, types
etc.) as determined by the Employer. Service planning for individual clinical
services will be progressed through the Clinical Directorate structure or other
arrangements as apply.
d) To co-operate with the
expeditious implementation of the Disciplinary Procedure
e) To formally review the
execution of the Clinical Directorate Service Plan with the Clinical Director /
Employer periodically. The Clinical Directorate Service Plan shall be reviewed
periodically at the request of the Consultant or Clinical Director / Employer.
The Consultant may initially seek internal review of the determinations of the
Clinical Director regarding the Service Plan.
f) To participate in the
development and operation of the Clinical Directorate structure and in such
management or representative structures as are in place or being developed. The
Consultant shall receive training and support to enable him/her to participate
fully in such structures.
g) To provide, as appropriate,
consultation in the Consultant’s area of designated expertise in respect of
patients of other Consultants at their request.
h) To ensure in consultation with
the Clinical Director that appropriate medical cover is available at all times
having due regard to the implementation of the European Working Time Directive
as it relates to doctors in training.
i) To supervise and be
responsible for diagnosis, treatment and care provided by non-Consultant
Hospital Doctors (NCHDs) treating patients under the Consultant’s care.
j) To participate as a right and
obligation in selection processes for non-Consultant Hospital Doctors and other
staff as appropriate. The Employer will provide training as required. The
Employer shall ensure that a Consultant representative of the relevant
specialty / sub-specialty is involved in the selection process.
k) To participate in clinical
audit and proactive risk management and facilitate production of all
data/information required for same in accordance with regulatory, statutory and
corporate policies and procedures.
l) To participate in and facilitate production of all data/information
required to validate delivery of duties and functions and inform planning and
management of service delivery
Service
Provision and Cover during Absences
The consultant has a continuing responsibility for the
care of patients in his/her charge and for the proper functioning of the
service allowing for appropriate delegation for the training of his/her staff.
He/she must be available by telephone and able to
attend the hospital in a timely fashion when on-call (40 minutes journey time).
Consultant staff must ensure that services are covered
during his/her planned absences. Where fixed commitments need to be cancelled
during planned absences appropriate notice (8 weeks) must be given. Absences
must be co-ordinated with other consultant staff in the specialty to ensure
senior cover is always available.
Essential Requirements
This post is open to doctors who hold Registration
as a Specialist in the Specialist Division of the Registrar of Medical
Practitioners maintained by the Medical Council in Ireland in the specialty of
Paediatrics: Paediatric Oncology

Management Responsibility
All Consultants are required to attend the monthly
Directorate Meetings. Post holders will be expected to share in administrative
duties allocated by mutual agreement within the Directorate. As part of the
Consultant Team, the consultant will
offer mentoring support and lead the existing clinical team through; joint
clinics, one-to-one support, individual case management and complex cases,
identifying development needs and suggesting CPD (Continuous Professional
Development) opportunities and responding to clinical enquiries via e-mail and
telephone.
