Overview
The post
holder will be contracted to provide a Haematology service, along with their
five consultant colleagues to our clients University Hospital. Candidates must
hold full registration with the Irish Medical Council, hold membership with the
Royal College of Physicians or show evidence of equivalent qualification and/or
Fellowship of the Royal College of Pathologists or show evidence of equivalent
qualification and be on the Specialist Register of the Irish Medical Council.
The main
purpose of this post is to maintain and develop the haematology service
provided for the population who present to University Hospital. This includes
both laboratory and clinical responsibilities relating to the diagnosis, also
the treatment and aftercare of patients with haematological problems presenting
as either emergency or elective referrals.
Working
with the existing haematologists, the post holder will be responsible for
providing these inpatient and outpatient services for which the hospital will
provide all necessary secretarial support and a new microscope.
The
successful candidate will provide with the other five consultants comprehensive
cover for the haematology service, including an on call service (1 in 5).
Prospective on call cover will be required for planned and unplanned absence of
consultant colleagues.
The
Pathology Department has CPA and MHRA Accreditation.
Specific Duties
Teaching:
· To assist in the professional supervision and management of trainee
medical staff.
· To organise and participate in teaching, examination and accreditation
duties for undergraduate, post graduate and continuing medical education
activity.
· To teach Medical School students and trainee SRP’s on rotation form
Dublin deanery.
· To participate in the education and teaching of nursing staff.
· To undertake appraisal as required.

Record Keeping:
The Department
The
Clinical Haematology Service provides facilities for the care of patients with
haematological disorders, including remission induction treatment for adult
patients with acute leukaemia and intensive treatment for patients with
high-grade lymphomas. Patients requiring autologous or allogeneic transplants
are referred to a regional centre. Hickman lines are inserted under ultrasound
guidance by a team of two interventionist consultant radiologists.
The
hospital maintains close links with the Haematology Department at the
University site in Dublin. Dublin undertakes specialist diagnostic work
(including immunophenotyping and cytogenetics) as well as providing expert
haematopathological review of all cases of lymphoma. The University Hospital is
a member of the Dublin MDTs for lymphoma, myeloma and leukaemia, with a weekly
Lymphoma Videoconference and an additional weekly Videoconference (alternately
Myeloma and Leukaemia). There are annual managerial and clinical meetings for
each of the three haematology malignancies. The University Hospital has a good
record of participating in clinical trials and has a Research Officer to
facilitate trial approvals (through the ethical committee), patient entry and
record keeping. In the haematology laboratory the main analysers are two Sysmex
XE 2100. The Haematology laboratory service is fully accredited and has full
quality assurance.
Haemophilia/Bleeding
Disorders:
The
University hospital patients have their routine follow up appointments at the
Haemophilia Centre. Dublin provides excellent support for acute patients
presenting at the University Hospital. There are currently discussion to
repatriate some of this work to the University Hospital.
The
Haematology coagulation laboratory has two Instrumentation Laboratories ACL Top
-500 and a Sysmex PFA100 platelet function analyser.
Blood
Transfusion:
The
Blood Transfusion Laboratory receives approximately 26,000 requests and
approximately issues 11,000 blood products per annum. An Immucor Galileo and
NEO machine will perform blood group and antibody screens. A Clinisys
laboratory information system, linked to Blood Track, monitors the movement of
all blood products. Stocks are maintained by daily routine deliveries from
Dublin Transfusion Centre, with “ad hoc” deliveries when necessary. Stocks of
factor concentrates, including Prothrombin Complex Concentrate and recombinant
factor VIIa are held in the department.
Haemoglobinopathy:
A
consultant haematologist has clinical responsibility for haemoglobinopathy and
is developing the service as haemoglobinopathy lead. Laboratory provision
includes the Biorad variant II HPLC analyser.
Links:
There
is no provision for clinical paediatric haematology in the department at the
University Hospital. Paediatric leukaemia patients are referred on to Dublin
for management. There is shared care between paediatricians in the University
Hospital and paediatric haematologists in Dublin for this patient group.
Clinical
Facilities:
Clinical
Haematology is fortunate in having good facilities for in-patients,
out-patients and day-case treatment within very close proximity of each other.
In-patient
haematology beds (as many as necessary) are provided on Ward which opened in
April 2009. There are 12 single rooms (one with laminar flow) and 2 four bedded
bays. The ward is currently shared with the diabetic/endocrinology team.
Out-patient
clinics are run in a purpose-built facility adjacent to the Pathology
Laboratory with its own waiting room, reception, phlebotomy room and two
clinical consultation rooms.
A purpose-built Unit for haematology and oncology
day treatment was opened in 2004. It has a large area for outpatient
treatments, with twelve treatment chairs and three beds. There is a specific
room for procedures, and five consultation rooms. There is a sizeable reception
area, a patient information room, and a small room for meetings. This will be
replace in November 2019 by a new purpose built Cancer Centre.

Job Plan
A formal job plan will be agreed between the appointee, the Operational Medical
Director and Lead Medicine - Haematology Consultant, on behalf of the Trust’s
Medical Director, based on the provisional timetable. This will be signed by
the Chief Executive and will be effective from the commencement date of the
appointment.
It will be reviewed at 3 months & thereafter
annually or at any time, but no less than 3 months after a previous review, as
requested by the appointee, Operational Medical Director or Clinical Lead &
adjusted accordingly to the agreement of both parties.

The SPA component is made up of core activities (appraisal,
revalidation, CME, routine departmental meetings etc.) and non-core activities.
Non-core activities are discussed and agreed within the hospital (via an agreed
tariff and, on occasion, through ad personal agreement). Non-core SPA may
reflect activities such as educational supervision, research, leadership and
quality improvement. Following appointment, non-core SPA may be used to
participate in our consultant development programme, or in acquiring or
consolidating specific skills and expertise. Non-core SPA is only available if
specific activities and objectives are mutually agreed in job planning.
Through its inclusive and comprehensive job planning process, the trust
commits to providing equitable working conditions to medical colleagues across
specialties and their sub-specialties. Delivery of job plans, whilst owned by
consultants and their Clinical Service Unit Leads, is overseen by the
Divisional Director and subject to peer challenge at divisional level, thus
enabling a further layer objectivity to our commitment in this regard
Standard Duties and responsibilities
a) To
participate in development of and undertake all duties and functions pertinent
to the Consultant’s area of competence, as set out within the Clinical
Directorate Service Plan and in line
with policies as specified by the Employer.
b) To
ensure that duties and functions are undertaken in a manner that minimises
delays for patients and possible disruption of services.
c) To
work within the framework of the hospital / agency’s service plan and/or levels
of service (volume, types etc.) as determined by the Employer. Service planning
for individual clinical services will be progressed through the Clinical
Directorate structure or other arrangements as apply.
d) To
co-operate with the expeditious implementation of the Disciplinary
Procedure
e) To
formally review the execution of the Clinical Directorate Service Plan with the
Clinical Director / Employer periodically. The Clinical Directorate Service
Plan shall be reviewed periodically at the request of the Consultant or
Clinical Director / Employer. The Consultant may initially seek internal review
of the determinations of the Clinical Director regarding the Service Plan.
f) To
participate in the development and operation of the Clinical Directorate
structure and in such management or representative structures as are in place
or being developed. The Consultant shall receive training and support to enable
him/her to participate fully in such structures.
g) To
provide, as appropriate, consultation in the Consultant’s area of designated
expertise in respect of patients of other Consultants at their request.
h) To
ensure in consultation with the Clinical Director that appropriate medical
cover is available at all times having due regard to the implementation of the
European Working Time Directive as it relates to doctors in training.
i) To
supervise and be responsible for diagnosis, treatment and care provided by
non-Consultant Hospital Doctors (NCHDs) treating patients under the
Consultant’s care.
j) To
participate as a right and obligation in selection processes for non-Consultant
Hospital Doctors and other staff as appropriate. The Employer will provide
training as required. The Employer shall ensure that a Consultant
representative of the relevant specialty / sub-specialty is involved in the
selection process.
k) To
participate in clinical audit and proactive risk management and facilitate
production of all data/information required for same in accordance with
regulatory, statutory and corporate policies and procedures.
l) To participate in and facilitate production of all data/information
required to validate delivery of duties and functions and inform planning and
management of service delivery
Service
Provision and Cover during Absences
The consultant has a continuing responsibility for the
care of patients in his/her charge and for the proper functioning of the
service allowing for appropriate delegation for the training of his/her staff.
He/she must be available by telephone and able to
attend the hospital in a timely fashion when on-call (40 minutes journey time).
Consultant staff must ensure that services are covered
during his/her planned absences. Where fixed commitments need to be cancelled
during planned absences appropriate notice (8 weeks) must be given. Absences
must be co-ordinated with other consultant staff in the specialty to ensure
senior cover is always available.
Essential Requirements
This post is open to doctors who hold Registration
as a Specialist in the Specialist Division of the Registrar of Medical
Practitioners maintained by the Medical Council in Ireland in the specialty of
Medicine - Haematology
Office Accommodation
The
post holder will undertake administrative work associated with their clinical
and other professional duties. Adequate time and facilities for clinical
administration, including appropriate office space, secretarial support and
access to a personal computer, software & internet access, will be
available.
Management Responsibility
All Consultants are required to attend the monthly
Directorate Meetings. Post holders will be expected to share in administrative
duties allocated by mutual agreement within the Directorate. As part of the
Consultant Team, the consultant will
offer mentoring support and lead the existing clinical team through; joint
clinics, one-to-one support, individual case management and complex cases,
identifying development needs and suggesting CPD (Continuous Professional
Development) opportunities and responding to clinical enquiries via e-mail and
telephone.
