Overview

 

You will join colleagues to deliver a modern Diabetes and Endocrinology service. We are particularly looking for an individual who has a particular interest in general diabetes, endocrinology and diabetes in pregnancy – other areas of interest will also be considered.

Office accommodation, secretarial support and full IT facilities including access to intranet and internet will be provided.

The Diabetes and Endocrinology department at the hospital provides all the speciality outpatient services for Connacht and surrounding area. Part of the role will be the supervision of a general medical ward and inpatient referrals. They are in particular looking for candidates to take forward hospital based diabetes services (inpatients, antenatal, insulin pumps, complicated renal and foot disease and young persons’ diabetes) and also support community delivered diabetic care.

Supervision of junior staff will be part of the role as well as mentoring medical students from nearby Medical Schools. Many of the current physicians’ have honorary roles within the Post-graduate Medical School at the University and the successful candidate would be encouraged to do the same.

As well as the medical school links the department has a strong research record with two of the current consultants previously holding clinical fellowships with portfolio registered research projects and co-supervision of both clinical and non-clinical higher degree students. Opportunities for a successful candidate to develop specific research interests will also be supported.

This post comes during a period of hospital wide service reviews, including a comprehensive reconfiguration of services for Mental Health in West (Connacht). The potential for hospital wide developments and a closer liaison with primary care and social services is substantial and exciting. A major expansion in undergraduate medical student teaching is planned.

Specific Duties

To provide, with consultant colleagues (as appropriate) a service in Diabetes and Endocrinology and General Medicine and supporting the development of sub-speciality interest to the hospitals and community so designated with responsibility for the prevention, diagnosis and treatment of illness.

You would work in both the acute, secondary care setting and in the community supporting the innovative and exciting work occurring outside the acute hospital. You would also take part in the acute medical in-patient service for all adult age groups and be part of the General Medical On Call rota , currently 1 in 10, (0900 – 2000 Monday to Friday covered by Acute Physician).

All clinical staff are accountable and responsible for their own clinical competence and should limit their actions to those for which they are deemed competent. Clinical staff are also required to comply with the requirements of their professional organisation regarding supervision. As a senior employee of the hospital the post holder will work in close cooperation with, and support other clinical, medical professional and managerial colleagues in providing high quality healthcare to the hospital’s patients.

Integral to the responsibilities of the post are the following requirements:-

·       To ensure the provision and delivery of a first class clinical service To provide effective leadership to all staff engaged in the specialty

·       To sustain and develop teaching and research wherever appropriate

·       To undertake all work in accordance with the hospital’s procedures and operating policies

·       To conduct clinical practice in accordance with contractual requirements and within the parameters of the hospital’s service plans

·       To maintain the confidence of business plans and development strategies formulated for the specialty or the hospitals Core clinical expertise in Diabetes, Endocrinology and General Medicine Experience and knowledge of running a sub specialty service

·       Specific skill in managing insulin pumps and antenatal diabetes is desired Leadership and team building skills as well as working as part of a multidisciplinary team Supporting and training multidisciplinary teams

·       Delivering care in the community alongside primary health care and social care teams

·       Providing persuasive strategic advice to LHB’s and Social Services Departments *if applicable*

·       Addressing the Intermediate Care Agenda Participating in CPD including Audit

·       You are expected to maintain your professional development for the IMC

·       A teaching and training role for medical undergraduates and postgraduates

·       Educational Supervisor role to junior and middle grade medical staff

·       To undertake regular multidisciplinary clinical audit and provide evidence based medicine

Clinical

·       To share the responsibility for the acute care of General Medical as well as Diabetes and Endocrine inpatients at the hospital – approx 25 patients

·       To undertake outpatient clinics supporting approximately 650 new referrals per year, with waiting lists below 36 weeks (currently), and all the necessary real and virtual follow-up outpatient activity

·       To support the development and delivery of a sub specialty service in hospital and in the community

·       To conduct clinical practice in accordance with contractual requirements within the parameters of the Division’s and hospital’s agreed objectives.

·       To develop and deliver a comprehensive service in Diabetes and Endocrinology

·       regular commitments to ward based teaching of undergraduates.

·       participation in the lecture programmes for 2nd and 4th & 5th year medical students.

·       teaching of other groups as required including other specialty juniors and specialist nurses.

 

Job Plan

A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead Medicine – Diabetes Consultant, on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.

It will be reviewed at 3 months & thereafter annually or at any time, but no less than 3 months after a previous review, as requested by the appointee, Operational Medical Director or Clinical Lead & adjusted accordingly to the agreement of both parties.

Standard Duties and responsibilities

a) To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan  and in line with policies as specified by the Employer.

b) To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.

c) To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.

d) To co-operate with the expeditious implementation of the Disciplinary Procedure 

e) To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.

f) To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.

g) To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.

h) To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.

i) To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.

j) To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 

k) To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.

l) To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery

 

Service Provision and Cover during Absences

 

The consultant has a continuing responsibility for the care of patients in his/her charge and for the proper functioning of the service allowing for appropriate delegation for the training of his/her staff.

 

He/she must be available by telephone and able to attend the hospital in a timely fashion when on-call (40 minutes journey time).

 

Consultant staff must ensure that services are covered during his/her planned absences. Where fixed commitments need to be cancelled during planned absences appropriate notice (8 weeks) must be given. Absences must be co-ordinated with other consultant staff in the specialty to ensure senior cover is always available.


Essential Requirements

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of General Medicine - Diabetes & Endocrinology

Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant  will offer mentoring support and lead the existing clinical team through; joint clinics, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.
