Overview

 

Inpatient Services

Our client’s hospital ward is a purpose-built ward with facilities to care for acutely unwell Haematology and Oncology patients. It is run jointly by the 2 specialties due to the similar needs of the client group. It has a double-airlock security entrance and is equipped with a number of side rooms which have positive/negative airflow capabilities. It opened in 2009 and accommodates 18 inpatients. Medical cover is provided by the five Consultant Oncologists who operate a ‘consultant of the week’ rota. They are currently supported by one Specialist Registrar, one trust grade doctor, one SHO doctor, and 4 acute oncology Clinical Nurse Specialists.

The Consultant Oncologists provide an on call service to both hospitals with the group. Secondary site patients requiring inpatient review are discussed with the resident on call team at the main hospital and an appropriate management plan agreed or transfer arranged if appropriate. In line with current protocols day case patients requiring admission, either for complications of their disease or their treatment, continue to be admitted to both hospitals. These patients are currently admitted under the care of local physicians in secondary site and under the Oncologists in the main hospital site.

Acute Oncology

The Acute Oncology Service provides a management pathway for patients with acute complications of cancer or cancer therapy which will ensure that all patients presenting with these conditions will receive prompt attention and appropriate treatment 24 hours a day, 7 days per week. Prompt and appropriate treatment will ensure maximum safety for patients undergoing chemotherapy, preventing morbidity and mortality from treatment toxicity and acute cancer-related emergencies. Patients with CNS, melanoma, sarcoma, and germ cell cancers are referred to the Cancer Centre

The main site has a dedicated chemotherapy department with 2 large treatment rooms each with 8 chairs, a smaller treatment room with 3 chairs and two individual treatment rooms. In addition, patients can receive chemotherapy in Selby, Malton or Scarborough using the Mobile Chemotherapy Unit. The department also has a dedicated satellite aseptic pharmacy service and is located in close proximity to the outpatient clinic rooms.

Chemocare

Both hospital sites benefit from the ChemoCare prescribing system. This system enables chemotherapy to be planned, confirmed and authorised either on or off site. It enables all staff to see the regimen scheduled for an individual patient, the parameters in which that regimen can be delivered (bloods results etc.) and any notes pertinent to the treatment of the patient.

Specific Duties

·       Maintenance of the highest clinical standards in the management of patients.

·       To share with colleagues responsibility for the day-to-day management of oncology patients.

·       Teaching and training of junior staff, medical students, postgraduates and other healthcare personnel.

·       To actively participate in both departmental and Trust matters concerning Clinical Governance and audit.

·       To have responsibility for ensuring active participation in continuing medical education/professional development (CME/CPD)

Teaching Commitments

·       regular commitments to ward based teaching of undergraduates.

·       participation in the lecture programmes for 2nd and 4th & 5th year medical students.

·       teaching of other groups as required including other specialty juniors and specialist nurses.

 

Job Plan

 

This post will provide site specific medical oncology management for

 

·       Breast cancer and cancer of unknown primary.

 

Flexibility in the site specialist interest of candidates will be considered.

 

Part time or job share applicants are welcome.

 

The post holder will attend the New Consultants induction and support programme at the main hospital and have a named mentor at the hospital. Oncology consultants participate in monthly clinical supervision with the clinical psychology team.

 

The proposed job plan for this post is detailed below. This outlines activity in a standard week when not on-call. Currently the 1:5 on-call rota includes a 3hr ward round on Saturday and Sunday mornings, i.e. 0.5 PAs on average per week in addition to the job plan below. With expansion to 6 consultants the pattern of on-call provision will become 1:6 and job plans will be reviewed and agreed.

 

On appointment of the successful candidate, the Lead Clinician/Clinical Director will commence discussions to finalise the job plan which will become operational on commencement. Job plans will be reviewed at least annually, following an appraisal meeting. The job plan will be a prospective agreement that sets out a consultant’s typical working pattern, duties and responsibilities and objectives for the coming year. It will cover all aspects of a consultant’s professional practice including clinical work, teaching, research, education and managerial responsibilities and external commitments.

 

Full-time posts (10PAs) typically have ~8 programmed activities of DCC (direct clinical care) and 1.5-2.5 programmed activities of SPA (supporting professional activity) over 4-5 days. Additional programmed activities may be offered to the post holder, up to a maximum of 2 for full time posts, taking the maximum total to 12. Any offer of additional programmed activities will be based on the needs of the service and in line with the Trust objectives and will be reviewed on at least an annual basis.

On-call Arrangements

This post will involve on-call on a 1:6 basis. On-call consultants provide a daily weekend ward round at the main hospital and telephone advice to both hospitals.

 

Standard Duties and responsibilities

a) To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan  and in line with policies as specified by the Employer.

b) To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.

c) To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.

d) To co-operate with the expeditious implementation of the Disciplinary Procedure 

e) To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.

f) To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.

g) To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.

h) To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.

i) To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.

j) To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 

k) To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.

l) To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery

 

Service Provision and Cover during Absences

 

The consultant has a continuing responsibility for the care of patients in his/her charge and for the proper functioning of the service allowing for appropriate delegation for the training of his/her staff.

 

He/she must be available by telephone and able to attend the hospital in a timely fashion when on-call (40 minutes journey time).

 

Consultant staff must ensure that services are covered during his/her planned absences. Where fixed commitments need to be cancelled during planned absences appropriate notice (8 weeks) must be given. Absences must be co-ordinated with other consultant staff in the specialty to ensure senior cover is always available.


Essential Requirements

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of General Medicine: Medical Oncology

 

Office Accommodation

The post holder will undertake administrative work associated with their clinical and other professional duties.  Adequate time and facilities for clinical administration, including appropriate office space, secretarial support and access to a personal computer, software & internet access, will be available.


Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant  will offer mentoring support and lead the existing clinical team through; joint clinics, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.
