As a key member of the stroke team you will work together with other colleagues and the Lead Clinician for stroke, to implement and provide a comprehensive stroke service and achieve the national and regional quality standards for stroke.

Additional Information

You will be able to take advantage of the excellent Consultant contract for the Republic of Ireland. The basic salary range for this post is euros €217,235 to €261,051 Euro’s per annum plus a further euro 26,672 per annum available in Commitment Awards.

The Stroke Service

Stroke medicine has been undergoing a dramatic reorganisation in recent years in Ireland, as a result of the combination of the increased research evidence base and the impact of the benchmarking National Stroke Audit, leading to the National Stroke Strategy.  As a result, of this, the acute stroke and stroke rehabilitation service in the university hospital is developing in line with the National Stroke Strategy requirements and the soon to be released National Stroke Plan. The Hospital Stroke Service is part of the Regional & National Stroke Network.  The successful candidate will be expected to participate in this network as well as work alongside our Clinical Lead for stroke in continuing to develop our services.

The service consists of daily contact with stroke patients on the Stroke Unit shared with a colleague with an interest in Stroke. This unit operates as a combined Unit and includes a HASU, Acute Stroke and Stroke Rehabilitation and aims to accept patients within 4 hours of presenting to our Emergency Department

Thrombolysis of suitable patients with Ischaemic Stroke is currently provided twenty four hours, seven days a week. Other duties will include twice weekly ward rounds on the Stroke Unit.

The hospital are also currently working with their Network colleagues in order to develop a pathway for Intra Arterial Thrombectomy (IAT)

The service is supported by a Stroke Specialist Team composed of a Nurse Consultant in Stroke (Clinical Lead), Stroke Trained Critical Care Outreach Team who are trained in stroke, Occupational Therapists, Physiotherapists, Speech and Language Therapists, Nurses trained in the management of stroke patients, Dieticians and Care Managers (Social Workers) with specialist skills within the Unit and as an outreach service to the rest of the hospital.  There are 1.5 Consultant Physicians, 1 Registrar, 2 HO Doctor, 1 SHO and 1 CMT (Core Medical Trainee) Doctor. The new post is additional. There are two Stroke Nurse Specialists and 0.2 Clinical Psychologist.

A daily one stop Transient Ischaemic Clinic (TIA clinic) is run Mondays to Sundays supported by three Consultants (inc a Consultant Nurse) There is a plan to include a regular Registrar, Core Medical Trainee and GP Vocational Trainee in the TIA clinic roster, with one of the two Stroke Physicians overseeing their work as a training opportunity. Ten dedicated CT and carotid duplex slots have been agreed with the Imaging (Radiology Department) per week.  The weekend TIA referrals are managed by the on call Specialist Medical Registrar.

Monthly Imaging meetings occur in the Stroke and Rehabilitation Therapy Gym on Fridays between 8.30 a.m. and 9.30 a.m. with a Consultant Radiologist in attendance. Thrombolysed patients and images of interest which pose interpretation difficulties are discussed. Every two months an interdisciplinary Team Continuing Professional Development meeting takes place. Weekly MDT meetings take place regularly to discuss all the stroke patients, set goals and plan discharges.

The Stroke Unit is currently participating in large International Multicentre Trials. The Unit is supported by a dedicated 0.5 Stroke Research Nurse. The appointee is encouraged to participate in research.

Department of Medicine

The department of medicine is based within the Medicine Clinical Business Unit of the hospital.

There are currently 4 medical ward areas

Ward 1 (28 beds)
Ward 2 (up to 28 beds of which 18 are Respiratory)
Ward 3 -Stroke Unit (24 bedded combined stroke unit inc 4 HASU beds)
Ward 4 - General and Neuro Rehabilitation Unit (30 beds)

Coronary Care Unit (CCU) (6 beds) and Cardiac Step/Acute (12 beds).  The Coronary Care Unit provides the cardiac monitoring facilities including the insertion of temporary and permanent pacemakers.

In addition to this there is a 24 bedded Medical Assessment and Admissions Unit (MAAU). Most emergency admissions will be admitted to the MAAU first and then triaged to the other ward areas. Consultant ward rounds, led by the Acute Medicine Physicians, take place in the MAAU in the morning and in the early evening.

In addition to the above, the Trust has a 6 bed Intensive Care Unit caring for both level 2 and level 3 patients.

The post holder will also be part of the Consultant on-call rota.

General

·       The provision of a comprehensive Acute Stroke and Stroke Rehabilitation Service, with Consultant colleagues.

·       Clinical responsibility for a number of general medical inpatients.

·       Provision of outpatient clinics including new and follow up patients.

·       To participate in the general medical on call rota (currently 1:11).  The weekday on calls require the Consultant to be available between 2000-0800 for advice and to come in if exceptional circumstances require it.  The evening and morning weekday WRs are undertaken by the Acute Physicians.  Weekends requires the on call Consultant to perform a morning and evening Ward Round.

·       Professional and clinical supervision and management of junior clinical staff that are attached to your team.

·       Substituting in an emergency for absent or ill colleagues.

 

Clinical governance and audit

 

In line with requirements of clinical governance, the appointee will be expected to participate in clinical audit, including where appropriate, specialty regional and national audit programmes; this includes the implementation of an on-going clinical audit programme within the department.

 

Teaching and training

 

The post holder will be expected to play his/her part in postgraduate activities, teaching and clinical audit.  This will include contribution to education and training programmes for hospital medical staff, GPs and other groups of hospital staff.

 

Responsibility for carrying out teaching, examination and accreditation duties as required and for contributing to undergraduate, postgraduate and continuing medical and nursing education, where appropriate.

 

Research and development

 

There are opportunities for research relevant to the service to be pursued within the agreed job-plan. 

 

Management

 

You will be required to co-operate with local clinical and operational managers as well as contribute to the efficient running of services.  You will be expected to share with Consultant colleagues in the medical contribution to management within your supporting professional activities. Attendance and contribution to Departmental and Directorate Meetings is encouraged.

 

Professional

 

The appointee will be expected to participate in clinical audit and to be registered for continuing professional development (CPD) with the Royal College of Physicians; the post holder will be expected to undertake activities necessary to fulfill CME requirements for annual certification and Irish Medical Council

 

Job Plan

A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead Medicine Stroke Consultant, on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.

It will be reviewed at 3 months & thereafter annually or at any time, but no less than 3 months after a previous review, as requested by the appointee, Operational Medical Director or Clinical Lead & adjusted accordingly to the agreement of both parties.

 

Programmed Activities in Job Plan

·     Direct Clinical Care (includes clinical activity, clinical related activity, unpredictable emergency work) - 7.5 PA’s on average per week.

·     Supporting Professional Activities (includes CPD, audit, teaching and research) – 1 SPA is given for all Consultants to do their CPD, Audit and related work whilst 1.5 SPA will be allocated based on discussion with the CD and subsequent approval that will be based on need, demand and relevance.

The provisional timetable represents a job plan consisting of 10-programmed activities (PAs) per week. The actual number of PAs and personal Job Plan objectives will be agreed between the Consultant and Clinical Director. 

The Job Plan will be reviewed at 3 months and conducted annually thereafter by the Clinical Director.

1:11 General Medical on call = 1 PA

1:11 On Call

The on call commitments is 1 in 11 and the appropriate supplement is Category A low frequency with 3% payable. There is an on-call allocation of 1.0 PA which is incorporated into the 10 PA job plan. The on call is covered by the MAU Consultants 8am-5pm Monday to Friday, so the Consultant will only take over the on call from 5pm to 8am during a weekday on call. The Consultant will work a 1 in 11 weekend and come in for a post take ward round on both Saturday and Sunday morning when on call.

The Medical Admissions Unit receives approximately 20 – 30 Medical admissions throughout the 24 hour day.  The on call intensity is classed as low frequency with a 3% intensity supplement payment.

Standard Duties and responsibilities

a) To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan  and in line with policies as specified by the Employer.

b) To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.

c) To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.

d) To co-operate with the expeditious implementation of the Disciplinary Procedure 

e) To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.

f) To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.

g) To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.

h) To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.

i) To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.

j) To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 

k) To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.

l) To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery


Essential Requirements

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of General Medicine: Stroke

Office Accommodation

The post holder will undertake administrative work associated with their clinical and other professional duties.  Adequate time and facilities for clinical administration, including appropriate office space, secretarial support and access to a personal computer, software & internet access, will be available.

 

Service Provision and Cover during Absences

 

The consultant has a continuing responsibility for the care of patients in his/her charge and for the proper functioning of the service allowing for appropriate delegation for the training of his/her staff.

 

He/she must be available by telephone and able to attend the hospital in a timely fashion when on-call (40 minutes journey time).

 

Consultant staff must ensure that services are covered during his/her planned absences. Where fixed commitments need to be cancelled during planned absences appropriate notice (8 weeks) must be given. Absences must be co-ordinated with other consultant staff in the specialty to ensure senior cover is always available.


Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant  will offer mentoring support and lead the existing clinical team through; joint clinics, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.
