Epilepsy
service
There is an
active epilepsy unit with two consultant neurologists and a consultant
neurophysiologist supported by routine and activation EEG, two video/telemetry
beds, epilepsy nursing team . There is a weekly meeting at the University
Hospital for pre-surgical assessment for intractable epilepsy.
Movement
Disorders
Movement
Disorders services are delivered by 2 Consultant Neurologists and 3 Consultant
Geriatricians, supported by 3 Parkinson’s Disease Specialist Nurses. Dedicated
Movement Disorders and Parkinson’s clinics are provided on all hospital sites,
as well as an Apomorphine service. Strong links exist with the regional
Neurosciences Centre at the University Hospital, where care for complex
disorders is shared, as well as training, and neuroradiology review if
required.
Multiple
Sclerosis service
The Multiple
Sclerosis Service is delivered by 2 Consultant Neurologists, 3 MS Specialist
Nurses and a Specialist Physiotherapist. The department delivers a full
Disease-Modifying Therapy Service and has liaises closely with the
Neurorehabilitation Unit at the University Hospital.
Motor
Neurone Disease Service
The Motor
Neurone Disease service is delivered by a Consultant Neurologist and a
Specialist Nurse and a Care Manager employed to support people living with MND.
The team work with other community health professionals including Occupational
Therapists, Dieticians, Speech & Language Therapists, Physiotherapists and
communication aids specialists.
Acute
Neurology Ward
The
University Hospital hosts the acute neurology ward with 16 neurological beds
and 2 telemetry beds which is supported by a neurophysiology department. In
addition there is also a 19 bedded Neurorehabilitation Unit. The Neurology and
Neurorehabilitation wards are supported by a consultant attending rota and
junior doctor team.
Stroke
Services
The hospital
has actively developed award-winning stroke services in recent years and during
the next year have plans for significant improvements to the delivery of
hyper-acute, acute and rehabilitation services for patients. The hospital
operates as an independent operational group within the wider Munster Region
Network aligned with and leading the regional stroke strategy. The hospital
currently provides combined acute and rehabilitation stroke units as well as
neurovascular clinics on all main sites. The hospital has extended the
thrombolysis service on all sites in the last 5 years to be a 24 hour, 7 day
service. This is achieved through a rota of Stroke Physicians and neurologists
with an interest in neurovascular disease using the Mytec horizontal tele-medicine
system, to allow remote monitoring of assessment, CT scan viewing and decision
making (one of the first examples of horizontal stroke telemedicine in the
country). Thrombolysis experience exceeds 250 cases / year, treating 18% of
patients (ICD 163). CT perfusion with CT angiography is available for all
patients at presentation 24/7; the images being available via PACS.
Neurovascular
clinics are established on all sites to enable referrals to be seen in the
shortest time possible using same day MRI/CEMRA as the imaging modality of
choice. The service runs seven days per week. Most high risk patients are seen
<24hrs with all others being seen <7 days.
A vascular
surgery programme of Rapid Access Carotid Endarterectomy has evolved, aiming to
intervene upon all symptomatic (TIA / minor stroke) extra cranial carotid
vessels with in 48hrs of diagnosis. This service is a national leader with
published data unparalleled; median delay to intervention is 2 days. The stroke
service is well supported by the radiology department with rapid access to
CT/A, MRI/A and duplex scanning. There is a weekly neuro-radiology meeting on
all main sites and an interventional neuroradiology meeting once monthly.

that full IMC registration and license to practice is an essential for this
role. If you do not currently have IMC registration, but you are in the process

without IMC registration at the time of shortlisting will not be considered- as
per hospital requirements.
Additional
Information
You will be
able to take advantage of the excellent Consultant contract for the Republic of
Ireland. The basic salary range for this post is euros 120,000 per annum to
euros 160,893 per annum plus a further euro 26,672 per annum available in
Commitment Awards.
Clinical
The post
holder’s duties will be primarily at the hospital. Time for specialist interest
clinic/ research and to carry out teaching duties as required r could be
arranged according to the interest of the appointee.
The
postholder will be responsible for providing a comprehensive general outpatient
neurology service to the catchment area of the hospital, alongside the existing
Neurology consultant and Neurology specialist nurse. The postholder will also
be providing specialist inpatient review where required, if inpatient beds
required the postholder will have access to inpatient beds at the hospital. The
postholder will be encouraged to practice a special interest clinics that is
relevant to the population of the Munster Region health community.

Job Plan
A formal job plan will be agreed between the appointee, the Operational Medical
Director and Lead Medicine Consultant, on behalf of the Trust’s Medical
Director, based on the provisional timetable. This will be signed by the Chief
Executive and will be effective from the commencement date of the appointment.
It will be reviewed at 3 months & thereafter
annually or at any time, but no less than 3 months after a previous review, as
requested by the appointee, Operational Medical Director or Clinical Lead &
adjusted accordingly to the agreement of both parties.
Non-Emergency Work in Premium Time:
The Trust will require you, in line with service needs to undertake non
emergency work during premium time.
On Call Arrangements:
The on-call rota is 1:8 and is non-residential. It requires the consultant to
be contactable at all times and to return to hospital as clinically indicated.
The primary duties are, to support the on-call Registrar in the out of hours
running of the Acute Neurology Ward and the provision of advice and support for
patients with neurological problems throughout the hospital. Although most of
this will be based in the hospital visits to the other acute hospitals may
sometimes be clinically indicated. The final work programme will be agreed with
the appointee and the Divisional Medical Director.
The duties and responsibilities in this job

continue to meet the needs of the service, future service developments and the
post holder
a) To participate in development
of and undertake all duties and functions pertinent to the Consultant’s area of
competence, as set out within the Clinical Directorate Service Plan and in line with policies as specified by the
Employer.
b) To ensure that duties and
functions are undertaken in a manner that minimises delays for patients and
possible disruption of services.
c) To work within the framework
of the hospital / agency’s service plan and/or levels of service (volume, types
etc.) as determined by the Employer. Service planning for individual clinical
services will be progressed through the Clinical Directorate structure or other
arrangements as apply.
d) To co-operate with the
expeditious implementation of the Disciplinary Procedure
e) To formally review the
execution of the Clinical Directorate Service Plan with the Clinical Director /
Employer periodically. The Clinical Directorate Service Plan shall be reviewed
periodically at the request of the Consultant or Clinical Director / Employer.
The Consultant may initially seek internal review of the determinations of the
Clinical Director regarding the Service Plan.
f) To participate in the
development and operation of the Clinical Directorate structure and in such
management or representative structures as are in place or being developed. The
Consultant shall receive training and support to enable him/her to participate
fully in such structures.
g) To provide, as appropriate,
consultation in the Consultant’s area of designated expertise in respect of
patients of other Consultants at their request.
h) To ensure in consultation with
the Clinical Director that appropriate medical cover is available at all times
having due regard to the implementation of the European Working Time Directive
as it relates to doctors in training.
i) To supervise and be
responsible for diagnosis, treatment and care provided by non-Consultant
Hospital Doctors (NCHDs) treating patients under the Consultant’s care.
j) To participate as a right and
obligation in selection processes for non-Consultant Hospital Doctors and other
staff as appropriate. The Employer will provide training as required. The
Employer shall ensure that a Consultant representative of the relevant
specialty / sub-specialty is involved in the selection process.
k) To participate in clinical
audit and proactive risk management and facilitate production of all
data/information required for same in accordance with regulatory, statutory and
corporate policies and procedures.
Essential Requirements
This post is open to doctors who hold Registration
as a Specialist in the Specialist Division of the Registrar of Medical
Practitioners maintained by the Medical Council in Ireland in the specialty of General
Medicine – Neurology
Office Accommodation
The post holder will undertake administrative work
associated with their clinical and other professional duties. Adequate time and
facilities for clinical administration, including appropriate office space,
secretarial support and access to a personal computer, software & internet
access, will be available.
Service Provision and Cover during
Absences
The
consultant has a continuing responsibility for the care of patients in his/her
charge and for the proper functioning of the service allowing for appropriate
delegation for the training of his/her staff.
He/she
must be available by telephone and able to attend the hospital in a timely
fashion when on-call (40 minutes journey time).
Consultant
staff must ensure that services are covered during his/her planned absences.
Where fixed commitments need to be cancelled during planned absences
appropriate notice (8 weeks) must be given. Absences must be co-ordinated with
other consultant staff in the specialty to ensure senior cover is always
available.
Management Responsibility
All Consultants are required to attend the monthly
Directorate Meetings. Post holders will be expected to share in administrative
duties allocated by mutual agreement within the Directorate. As part of the
Consultant Team, the consultant will
offer mentoring support and lead the existing clinical team through; joint
clinics, one-to-one support, individual case management and complex cases,
identifying development needs and suggesting CPD (Continuous Professional
Development) opportunities and responding to clinical enquiries via e-mail and
telephone.
