Overview
This is a
replacement post to join the existing consultants in Cardiology based at our
clients hospital in the Munster Region (Republic of Ireland). The applicant
will be expected to participate in the primary angioplasty rota on a 1:6 basis.
The hospital
operates a busy Emergency Department alongside other acute services covering
most of the major specialties. The hospital provides primary angioplasty to the
majority of the region patients (~550,000) Hospital has close links with the
nearby cardiology department and also the other university hospital which
provides cardiothoracic surgery and tertiary cardiology facilities.
The hospital

Additional
Information
You will be
able to take advantage of the excellent Consultant contract for the Republic of
Ireland. The basic salary range for this post is euros 120,000 per annum to
euros 160,893 per annum plus a further euro 26,672 per annum available in
Commitment Awards.
Services
Provided In Munster, Cardiology Services are provided at the hospital for a
local catchment population of approximately 130,000. In addition, some
secondary cardiac services are provided for the population of NW, which serves a catchment population of
approximately 60,000.
The
inpatient facility comprises a 9 bed Coronary Care Unit with an integral
cardiac catheter department (with 2 Siemens catheter Labs) and a single pacing
laboratory. There are 12 day-case beds and 6 recovery beds attached to the
catheter lab, which is currently being converted into a radial lounge. In
addition, the cardiologists supervise the care of cardiac patients on medical
wards (approx. 42 beds).

Cardiac
Devices and Electrophysiology
The
department performs over 300 permanent pacing procedures annually, and
approximately 1/3rd of those are CRT/ICD procedures. The service is delivered
by 3 consultants at present.
The hospital currently refers EP cases to the nearby University Hospital.
Non Invasive Testing
The
department performs over 1000 24-hour tapes and 600 Novacor R investigations
per year. Ambulatory blood pressure monitoring is performed on over 300
patients annually. There is access to signal averaged ECGs, reveal devices and
tilt testing. In addition, offering an open access 24-hour ambulatory ECG
service for GPs.
Imaging
The
department has four echocardiography machines and performs over 4000
trans-thoracic, 100 trans-oesophageal and 150 stress echos yearly, with plans
in place for significant expansion. A community based open access diagnostic
echo service is also provided for regional GPs.
Hospital has
a cardiac CT and MRI service which is run jointly between radiology and
cardiology and currently undertakes around 700 CT scans and 500 MRI scans per
annum
Chest
Pain Service
The
department runs an average of 5 chest pain clinics a week and are now compliant
with legislation. There is one treadmill in the cardiology department, and a
second in the A&E chest pain assessment unit. The service is predominantly
nurse-led. The service is legislation compliant.
Cardiac
Rehabilitation Services
A highly successful
primary/secondary care rehab service has been running in for many years.
Heart
Failure Service
A hospital based, nurse led
service currently looks after approximately 230 patients, with a total of 670
patients on the books. There are 5 nurse specialists and a pharmacist.
DC
Cardioversion Service
The department has a weekly nurse
led DC cardioversion service.
Outpatient
clinics take place at the hospital, in the cardiology department, but also in
other locations as required (to be discussed in advance). The number of clinics
will vary depending on whether you are attending the wards or not but will
average 1.5 per week.
You will be
part of a team of nine consultant cardiologists attending the wards. The team
is divided into two groups of 4/5 who alternate a six-week block attending the
wards. Intervention is performed at the hospital. You will be part of a team of
five consultant cardiologists who provide a 1 in 6 24-7 primary PCI service.
Duties are cancelled to facilitate this.
Provision is
made to have the following morning off if you are busy overnight. After on-call
weekends, the Monday and Tuesday duties are cancelled.
Additional
interests outside intervention would be welcomed.
Work Load
Outpatient
clinic templates are based on HSE guidelines of 35 minutes for a new patient
and 20 minutes for a follow up consultation. The Ward work levels vary, but the
time allocated in the job plan does reflect the average time taken per month
covering the wards. Angiography lists have up to 5 patients per session and PCI
lists up to 4 patients, depending on complexity. Time is included in the job
plan for support of junior medical staff and specialist nurses and cardiac
physiologists.

Job Plan
A formal job plan will be agreed between the appointee, the Operational Medical
Director and Lead Medicine Consultant, on behalf of the Trust’s Medical
Director, based on the provisional timetable. This will be signed by the Chief
Executive and will be effective from the commencement date of the appointment.
It will be reviewed at 3 months & thereafter
annually or at any time, but no less than 3 months after a previous review, as
requested by the appointee, Operational Medical Director or Clinical Lead &
adjusted accordingly to the agreement of both parties.

a) To participate in development
of and undertake all duties and functions pertinent to the Consultant’s area of
competence, as set out within the Clinical Directorate Service Plan and in line with policies as specified by the
Employer.
b) To ensure that duties and
functions are undertaken in a manner that minimises delays for patients and
possible disruption of services.
c) To work within the framework
of the hospital / agency’s service plan and/or levels of service (volume, types
etc.) as determined by the Employer. Service planning for individual clinical
services will be progressed through the Clinical Directorate structure or other
arrangements as apply.
d) To co-operate with the
expeditious implementation of the Disciplinary Procedure
e) To formally review the
execution of the Clinical Directorate Service Plan with the Clinical Director /
Employer periodically. The Clinical Directorate Service Plan shall be reviewed
periodically at the request of the Consultant or Clinical Director / Employer.
The Consultant may initially seek internal review of the determinations of the
Clinical Director regarding the Service Plan.
f) To participate in the
development and operation of the Clinical Directorate structure and in such
management or representative structures as are in place or being developed. The
Consultant shall receive training and support to enable him/her to participate
fully in such structures.
g) To provide, as appropriate,
consultation in the Consultant’s area of designated expertise in respect of
patients of other Consultants at their request.
h) To ensure in consultation with
the Clinical Director that appropriate medical cover is available at all times
having due regard to the implementation of the European Working Time Directive
as it relates to doctors in training.
i) To supervise and be
responsible for diagnosis, treatment and care provided by non-Consultant
Hospital Doctors (NCHDs) treating patients under the Consultant’s care.
j) To participate as a right and
obligation in selection processes for non-Consultant Hospital Doctors and other
staff as appropriate. The Employer will provide training as required. The
Employer shall ensure that a Consultant representative of the relevant
specialty / sub-specialty is involved in the selection process.
k) To participate in clinical
audit and proactive risk management and facilitate production of all
data/information required for same in accordance with regulatory, statutory and
corporate policies and procedures.
Essential Requirements
This post is open to doctors who hold Registration
as a Specialist in the Specialist Division of the Registrar of Medical
Practitioners maintained by the Medical Council in Ireland in the specialty of
General Medicine: Cardiology
Office Accommodation
The
post holder will undertake administrative work associated with their clinical
and other professional duties. Adequate time and facilities for clinical
administration, including appropriate office space, secretarial support and
access to a personal computer, software & internet access, will be
available.
Service Provision and Cover during
Absences
The
consultant has a continuing responsibility for the care of patients in his/her
charge and for the proper functioning of the service allowing for appropriate
delegation for the training of his/her staff.
He/she
must be available by telephone and able to attend the hospital in a timely
fashion when on-call (40 minutes journey time).
Consultant
staff must ensure that services are covered during his/her planned absences.
Where fixed commitments need to be cancelled during planned absences
appropriate notice (8 weeks) must be given. Absences must be co-ordinated with
other consultant staff in the specialty to ensure senior cover is always
available.
Management Responsibility
All Consultants are required to attend the monthly
Directorate Meetings. Post holders will be expected to share in administrative
duties allocated by mutual agreement within the Directorate. As part of the
Consultant Team, the consultant will
offer mentoring support and lead the existing clinical team through; joint
clinics, one-to-one support, individual case management and complex cases,
identifying development needs and suggesting CPD (Continuous Professional
Development) opportunities and responding to clinical enquiries via e-mail and
telephone.
