Overview

 

To provide a laboratory and clinical service in haematology at both the University Hospital and General Hospitals of our clients remit.  To lead the Lymphoma service.

Please note that full IMC registration and license to practice is an essential for this role. If you do not currently have IMC registration, but you are in the process of applying, please use the supporting information section of the application form to provide an update about the status of your IMC application.

Candidates without IMC registration at the time of shortlisting will not be considered- as per hospital requirements.

Additional Information

You will be able to take advantage of the excellent Consultant contract for the Republic of Ireland. The basic salary range for this post is euros 120,000 per annum to euros 160,893 per annum plus a further euro 26,672 per annum available in Commitment Awards.

The Haematology Department

The haematology department delivers a comprehensive haematology service to a population of about 1 million in Munster and nearby facilities. Inpatient services are centralised in the Cancer Centre at the University hospital where there is a 38 bedded ward shared with oncology. There are usually 20-25 haematology inpatients. As well as the inpatient ward, the Cancer Centre also contains outpatient rooms, the radiotherapy department and a large chemotherapy suite and day unit. Outpatient clinics and outpatient chemotherapy are also provided at the General Hospitals, a district general hospital about 20 miles away from the University hospital. This post would involve clinical duties at the University Hospital and General Hospitals only. All 3 sites have modern, high quality facilities.

All aspects of haemato-oncology are treated within the haematology department, with the exception of patients requiring allogeneic stem cell transplantation who are referred to the University Hospital, or Dublin facilities.  There is an active, fully JACIE accredited, autologous stem cell harvesting and transplantation service, and 29 transplants were performed in the year ending April 2016.

Patients with non-malignant conditions who require specialist inpatient haematology care are also admitted to the haematology ward at the hospital.

Adjacent to the inpatient ward is a 5 bedded Emergency Assessment Unit (EAU), which allows 24/7 direct access to haematology and oncology patients who require assessment and possible admission without the need for patients to go via the Emergency Department or Acute Medical unit. The 24 hour patient helpline is operated by the nurses on EAU. Approximately 1500 patients (haematology and oncology) attend EAU annually, of whom about 55% require admission to the inpatient ward.

An anaemia service has recently been established with a view to reducing blood usage and improving the coordination of care of patients with chronic anaemia.

There is an active clinical trials team and a commitment to continuing to broaden our research and trails portfolio.

Paediatric patients with haematological conditions are managed by 2 consultant paediatricians, with children with haematological malignancies treated in a shared care arrangement with a Children’s Hospital nearby. The adult haematologists are not involved in the management of these children. The Consultant Lead runs a joint clinic monthly with one of the paediatricians for children with bleeding disorders.

The haematology department at General Hospitals provides an outpatient and day case (including chemotherapy) service to patients from sites within area as well as some patients from other hospitals. An expanded and refurbished chemotherapy unit, that opened at General Hospital in Summer 2016, enhanced the environment for patients undergoing treatment there. There are no dedicated haematology beds at General Hospital, and patients who require inpatient treatment because of their haematological disease are transferred to the University Hospital. The haematologists at the General Hospitals work closely with the physicians and the Acute Oncology Team to provide a high quality liaison haematology service. One consultant is currently based full time at General Hospital. This post, in addition to another post make up a further consultant at General and several other members of the consultant team spend either half a day a week or a full day at General, to constitute a third consultant presence there.

Any applicant who is unable for personal reasons, to work full-time will be eligible to be considered for the post. If such a person is appointed, modification of the job content will be discussed on a personal basis with the Trust in consultation with consultant colleagues.

Specific Duties

This is a replacement to work between the University Hospital and General Hospital. There are two positions on offer. You will join a cohesive team of 11 other consultant colleagues to deliver a haemato-oncology and general haematology service at General Hospital and University Hospital Facility.

Outpatient clinics within the department are presently being restructured to increase the degree of subspecialisation, and you will provide 4 outpatient clinics weekly, one of the roles includes a Lymphoma and CLL clinic at the University Hospital. You will also be required to do further subspecialty clinics and general haematology clinics.

One of the roles will be expected to provide leadership in the further development of the Lymphoma service across all three sites. Approximately 60 new patients with Lymphoma are referred to the department (all sites) annually.

Outpatient clinics are organised on a team basis, with 2-4 consultants working within each clinic, depending on subspecialty interest. Outpatient clinics usually include 4-6 new patients or 13-15 follow-up patients per consultant.

You will be expected to participate in the ward attending rota, which is likely to be approximately 2 weeks in every 12. For each two week period 2 consultants attend the ward and are responsible for day to day management of all inpatients, and for consultative haematology/haematology liaison with the Acute Oncology team for the University Hospital.

On call duties are likely to be ca 1:10. You will be expected to cover all aspects of the service when on call. There will normally be a middle grade doctor first on call at University Hospital. A weekend ward round on Saturday and Sunday at University Hospital when on call is expected. You will not be required to attend General Hospital when on call, but will provide a telephone advice service for both. On call is currently paid at category A.

Laboratory Haematology

You will also be expected to contribute to laboratory haematology by reporting blood films and marrow aspirates. All marrows from the General Hospital are reported at the Haematology Oncology diagnostic service at the University Hospital. Marrows from the hospital (ca 800 aspirates and 600 trephines annually) are reported locally, and this will continue for the foreseeable future. There will be the opportunity for an interested and suitably experienced appointee to participate in trephine reporting, however this will not be compulsory.

The haematology laboratory is one of the largest in the region and is divided into the following sections.

1. Combined Blood Sciences Specimen Reception
2. Automated Section
3. Specialist Coagulation
4. Films and differential cytomorphology.
5. Special investigations
6. Blood transfusion

Teaching Commitments

·       regular commitments to ward based teaching of undergraduates.

·       participation in the lecture programmes for 2nd and 4th & 5th year medical students.

·       teaching of other groups as required including other specialty juniors and specialist nurses.

 

Job Plan

A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead Medicine - Haematology Consultant, on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.

It will be reviewed at 3 months & thereafter annually or at any time, but no less than 3 months after a previous review, as requested by the appointee, Operational Medical Director or Clinical Lead & adjusted accordingly to the agreement of both parties.

** Attendance at the ward discussion on Thursday morning is required on ward attending weeks and the week preceding attending. You are also required to attend this meeting on 2 of the remaining 7 weeks in every 10 (to ensure adequate consultant representation at this meeting)

 

Standard Duties and responsibilities

a) To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan  and in line with policies as specified by the Employer.

b) To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.

c) To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.

d) To co-operate with the expeditious implementation of the Disciplinary Procedure 

e) To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.

f) To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.

g) To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.

h) To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.

i) To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.

j) To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 

k) To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.

l) To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery


Essential Requirements

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of Medicine - Haematology

Office Accommodation

The post holder will undertake administrative work associated with their clinical and other professional duties.  Adequate time and facilities for clinical administration, including appropriate office space, secretarial support and access to a personal computer, software & internet access, will be available.

 

Service Provision and Cover during Absences

 

The consultant has a continuing responsibility for the care of patients in his/her charge and for the proper functioning of the service allowing for appropriate delegation for the training of his/her staff.

 

He/she must be available by telephone and able to attend the hospital in a timely fashion when on-call (40 minutes journey time).

 

Consultant staff must ensure that services are covered during his/her planned absences. Where fixed commitments need to be cancelled during planned absences appropriate notice (8 weeks) must be given. Absences must be co-ordinated with other consultant staff in the specialty to ensure senior cover is always available.


Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant  will offer mentoring support and lead the existing clinical team through; joint clinics, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.
