Overview
The purpose
of this post is to enhance the provision of gastroenterology services and
meeting the demands of endoscopy service. There is opportunity to IBD service
further. The role encompasses ward work as well as both gastroenterology clinic
and endoscopy sessions and the candidate will be expected to embrace the other
aspects of hospital gastroenterology including attendance and involvement with
MDT, and will be required to partake in the out of hours GI bleeding rota and
future expansion of 7 day service.
We welcome
candidates with CCT or equivalent in gastroenterology. Any special interest and
IBD in particular will be considered and role development will be supported.

that full IMC registration and license to practice is an essential for this
role. If you do not currently have IMC registration, but you are in the process

per hospital requirements.
Additional
Information
You will be
able to take advantage of the excellent Consultant contract for the Republic of
Ireland. The basic salary range for this post is euros €217,235 to €261,051
Euro’s per annum plus a further euro 26,672 per annum available in Commitment
Awards.
The main
duties are to provide elective and emergency care to general gastroenterology
and medical patients presenting to the hospital.
b. To
provide out-of-hour GI bleeding rota cover currently 1 in 5 rota (extra supplement
is payable)
c.
Responsibilities which relate to a special interest will be supported. There
will be opportunities to develop special interests including IBD.
d.
Professional supervision, training and management of junior medical staff.
e.
Responsible for carrying out teaching, as requested by the College Tutor. This
will involve contributing to and participating in postgraduate and continuing
medical education activities, both locally and nationally. There is opportunity
to get involved in teaching of medical school students.
f.
Requirement to participate in the Directorate’s medical audit activities and
take a share in running the education activities including teaching and audit.
g.
Involvement in research is encouraged.
h. The
successful candidate will be expected to work with local managers and
professional colleagues in the efficient running of services and will share
with consultant colleagues in the contribution to management roles and
responsibilities. Subject to the provision of the Terms and Conditions of the
Service, the appointee will be expected to observe the hospital’s agreed
policies and procedures, drawn up in consultation on clinical matters, and to
follow the local and national employment and personnel policies and procedures.
The successful candidate will be expected to make sure that there are adequate
arrangements for hospital staff involved in the care of their patients to be
able to contact them when necessary.
i. The post
holder may be occasionally required to carry out domiciliary consultations or
services by mutual agreement.
j. All medical
staff employed by the hospital are expected to comply with regional and
appropriate hospital health and safety policies.
k. The
successful candidate agrees to have responsibility for the training and supervision
of junior medical staff whom work with them and will devote time to this
activity on a regular basis. If appropriate, the successful candidate will be
named in the contracts of doctors in training grades as the person responsible
for overseeing their training and as the initial source of advice to such
doctors on their careers.
l. The post
holder is expected to undergo an annual job planning and appraisal as a
contractual obligation.

Job Plan
A formal job plan will be agreed between the appointee, the Operational Medical
Director and Lead Medicine Consultant, on behalf of the Trust’s Medical
Director, based on the provisional timetable. This will be signed by the Chief
Executive and will be effective from the commencement date of the appointment.
It will be reviewed at 3 months & thereafter
annually or at any time, but no less than 3 months after a previous review, as
requested by the appointee, Operational Medical Director or Clinical Lead &
adjusted accordingly to the agreement of both parties.
The job plan on appointment will consist of a split of
7.5:2.5 DCC/SPA. The SPA component is made up of core activities (appraisal,
revalidation, CME, routine departmental meetings etc.) and non-core activities.
Non-core activities are discussed and agreed within the hospital (via an agreed
tariff and, on occasion, through ad personal agreement). Non-core SPA may
reflect activities such as educational supervision, research, leadership and
quality improvement. Following appointment, non-core SPA may be used to
participate in our consultant development programme, or in acquiring or
consolidating specific skills and expertise. Non-core SPA is only available if
specific activities and objectives are mutually agreed in job planning.

On-call with Saturday 3hrs in-reach (1:5 rota 8%
supplement): 1.25 PA
Supporting Professional Activity: up to 2.5
The Direct clinical care (DCC) includes outpatient
clinics, endoscopy sessions, ward and board rounds, MDT meetings, paper work
related to patient associated clinical work. The participation in endoscopy
rota (1:5) for GI bleed and 3hour in-reach on Saturday mornings earns 1.25 PA
and 5% intensity payments. There is no commitment to acute medical take.
Normal clinic has 4 new and 7 follow up patients; Most
endoscopy sessions have 11 units equivalent procedures.
The SPA allowance is currently being negotiated in the
trust and 1.0 SPA will be allocated to all for the core activities, and further
1.5 SPA can be allocated according to the responsibility taken e.g. 0.25 SPA
for educational supervisor role per trainee (to a max of 0.5 SPA), 0.25 Spa for
being an appraiser, Clinical audit lead 0.5 SPA etc. For non-core SPA output of
the roles will reviewed regularly.
In future if 7-day service is commenced then there
will time off in-lieu of the weekend duties in the job plan.
Standard Duties and responsibilities
a) To participate in development
of and undertake all duties and functions pertinent to the Consultant’s area of
competence, as set out within the Clinical Directorate Service Plan and in line with policies as specified by the
Employer.
b) To ensure that duties and
functions are undertaken in a manner that minimises delays for patients and
possible disruption of services.
c) To work within the framework
of the hospital / agency’s service plan and/or levels of service (volume, types
etc.) as determined by the Employer. Service planning for individual clinical
services will be progressed through the Clinical Directorate structure or other
arrangements as apply.
d) To co-operate with the
expeditious implementation of the Disciplinary Procedure
e) To formally review the
execution of the Clinical Directorate Service Plan with the Clinical Director /
Employer periodically. The Clinical Directorate Service Plan shall be reviewed
periodically at the request of the Consultant or Clinical Director / Employer.
The Consultant may initially seek internal review of the determinations of the
Clinical Director regarding the Service Plan.
f) To participate in the
development and operation of the Clinical Directorate structure and in such
management or representative structures as are in place or being developed. The
Consultant shall receive training and support to enable him/her to participate
fully in such structures.
g) To provide, as appropriate,
consultation in the Consultant’s area of designated expertise in respect of
patients of other Consultants at their request.
h) To ensure in consultation with
the Clinical Director that appropriate medical cover is available at all times
having due regard to the implementation of the European Working Time Directive
as it relates to doctors in training.
i) To supervise and be
responsible for diagnosis, treatment and care provided by non-Consultant
Hospital Doctors (NCHDs) treating patients under the Consultant’s care.
j) To participate as a right and
obligation in selection processes for non-Consultant Hospital Doctors and other
staff as appropriate. The Employer will provide training as required. The
Employer shall ensure that a Consultant representative of the relevant
specialty / sub-specialty is involved in the selection process.
k) To participate in clinical
audit and proactive risk management and facilitate production of all
data/information required for same in accordance with regulatory, statutory and
corporate policies and procedures.
Essential Requirements
This post is open to doctors who hold Registration
as a Specialist in the Specialist Division of the Registrar of Medical
Practitioners maintained by the Medical Council in Ireland in the specialty of
General Medicine: Gastroenterology
Office Accommodation
The post holder will undertake administrative work
associated with their clinical and other professional duties. Adequate time and
facilities for clinical administration, including appropriate office space,
secretarial support and access to a personal computer, software & internet
access, will be available.
Service Provision and Cover
during Absences
The
consultant has a continuing responsibility for the care of patients in his/her
charge and for the proper functioning of the service allowing for appropriate
delegation for the training of his/her staff.
He/she
must be available by telephone and able to attend the hospital in a timely
fashion when on-call (40 minutes journey time).
Consultant
staff must ensure that services are covered during his/her planned absences.
Where fixed commitments need to be cancelled during planned absences
appropriate notice (8 weeks) must be given. Absences must be co-ordinated with
other consultant staff in the specialty to ensure senior cover is always
available.
Management Responsibility
All Consultants are required to attend the monthly
Directorate Meetings. Post holders will be expected to share in administrative
duties allocated by mutual agreement within the Directorate. As part of the
Consultant Team, the consultant will
offer mentoring support and lead the existing clinical team through; joint
clinics, one-to-one support, individual case management and complex cases,
identifying development needs and suggesting CPD (Continuous Professional
Development) opportunities and responding to clinical enquiries via e-mail and
telephone.
