Overview
Our client
provides services in collaboration with a range of partners, including local
GPs, acute, mental health and community health service providers. They are a founding member of University
College Partners, working to adapt academic and laboratory research to enable
improved clinical outcomes for our patients. They also work closely with a
number of universities to provide training for doctors, nurses and other
healthcare professionals as part of both undergraduate and post-graduate
programmes.
They are a major local employer, with over 60% of our staff living locally in
Munster Region. The organisation continues to work in developing and enhancing
the skills of the staff it employs. This will be in line with the patient
safety strategy to deliver improved quality in clinical practice and also
through enhancing the clinical management and leadership skills within the
organisation.
Our total

As a result
of these increases, approximate average daily patient numbers using the
hospital’s services are now as follows:
· 500 patients in A&E
· 450 inpatients on our wards
· 50 patients undergoing major or minor surgery
· 800 outpatients attending clinics
· 200 women attending maternity clinics
· 15 babies born in our maternity unit
And, in
addition, each day we provide approximately:
· 540 X-rays and radiology tests
· 500 blood tests.
The key
objectives for the Trust will be maintaining the safe and high quality care
currently delivered. The organisation will continue to work in developing and
enhancing the skills of the staff it employs.
This will be
in line with the patient safety strategy to deliver improved quality in
clinical practice and also through enhancing the clinical management and
leadership skills within the organisation. The clinical strategy for the next
5-10 years is anchored on delivering high quality emergency clinical services
and networked specialist care to our local population in partnership with our
local healthcare providers.

Additional
Information
You will be
able to take advantage of the excellent Consultant contract for the Republic of
Ireland. The basic salary range for this post is euros 120,000 per annum to
euros 160,893 per annum plus a further euro 26,672 per annum available in
Commitment Awards.
Specific Duties
Direct
Clinical Care (DCCs): Shop floor presence – This job plan provides a physical
consultant presence out in the clinical arena of the Emergency Department. Each
consultant will ensure clinical leadership and support to the departmental and
other specialty staff as well as taking on their own patient workload. They
will maintain and improve quality of care delivered to patients and guarantee
the efficiency of the department.
There will
be a consultant present on the shop floor from 0800 hours until 2300 hours
Monday to Friday and from 0900 hours until 2300 hours Saturday and Sunday
Sessions are
allocated to Clinical administration and shop floor back up – each consultant
requires clinical time to undertake clinical administration and to ensure a
quality clinical service in times of need. Overall weekend frequency equals 1
in 7 therefore attracts 5% oncall allowance Number of weekend days worked in 52
weeks will be 15 (ie 7.5 weekends)
Core Shop
Floor DCC Roles:
(i) Medical
Controller
(ii)
Resuscitation Room supervision – to include oversight of the paediatric area
(iii)
Medical Lead – to ensure early senior medical input for ambulance arrivals and
to be a point of medical advice for those patients in the locally situated
walk-in assessment area
Each of the
three roles are allocated from Mon-Fri 8am-4pm. After that time 4pm-8pm roles
(i) and (ii) at any other time including weekends only the Medical Controller
role will be possible until further consultant sessions are available.
At certain
times there may be more than three consultants on the shop floor, in that
situation the expectation is for the EM consultant to see patients directly.
The
following is not an exhaustive or exclusive list of the duties of a consultant.
The appointee is expected to:
· Provide senior clinician services to
the Emergency Department as per job plan or as agreed within the Trust, and as
required when on call.
· Provide clinical leadership to the
department when undertaking clinical care, taking lead clinical responsibility
as rostered on a day to day basis. Facilitate safe and effective clinical care
· Lead on service design and
development to help with establishing pre-operative patient assessment
· Work with consultant colleagues and
nursing staff to ensure efficient use of Trust’s resources and contribute
actively to clinical governance in the department and to development of quality
enhancement projects in the unit.
· Show flexibility and undertake
different appropriate clinical tasks within allocated DCC sessions after
discussion with the clinical manager, as the need arises.
· Actively participate in the Unit’s
Teaching Programme and undertake supervision, teaching and training of medical
trainees and other multi-professional staff, in line with guidance from the
relevant Royal Colleges or specialty bodies.
· Actively participate in clinical
guidelines development, audit and CPD.

and 4th & 5th year medical students.

teaching of other groups as required including other
specialty juniors and specialist nurses.
Job Plan
A formal job plan will be agreed between the appointee, the Operational Medical
Director and Lead Emergency Medicine Consultant, on behalf of the Trust’s
Medical Director, based on the provisional timetable. This will be signed by
the Chief Executive and will be effective from the commencement date of the
appointment.
It will be reviewed at 3 months & thereafter
annually or at any time, but no less than 3 months after a previous review, as
requested by the appointee, Operational Medical Director or Clinical Lead &
adjusted accordingly to the agreement of both parties.
For a whole-time 10 PA contract: the normal split is
· Direct Clinical Care:
8.5 PAs on average per week (Clinical activity, clinically related activity,
predictable & unpredictable emergency work )
·
Supporting Professional Activities:
1.5 PAs on average per week (CPD, audit,
teaching & research )
** ** An additional 1 SPA is available to all Consultant Staff subject to
review and agreement for where quality and service roles which must be approved
prospectively by the Clinical Director
Total Hours, Monitoring and Future Working:
Based on the above working pattern each new consultant
post will attract 11.5 PAs per week and a band A supplement (5%). This is made
up of 10 PAs DCC and 1.5 PAs SPA (with the opportunity to take up quality and
service role SPA and drop equivalent DCC once in post)
On Call and Internal Cover:
Each consultant will be expected to provide an on call
commitment to the department. Whilst on call the consultant must remain
contactable at all times and be prepared to give advice on clinical and
managerial issues as required by the hospital staff. The consultants must
always be prepared to return to the department whilst on call for pressing
clinical issues in accordance with escalation policy.
The request for a consultant to return to the
department should only be made for clinical reasons and is in line with the
College of Emergency Medicine recommendations. There will be continuous on call
cover from 2300 hours to 0800 hours Monday to Friday and from 2300 hours to
0900 hours Saturday and Sunday. Prospective cover is included in the rota for
weekend shifts, evening shifts, on-call shifts.
There are sufficient clinical PAs in the Team job plan
to guarantee the 3 core DCC shop floor shifts all year round. The department
must always be able to access the services of one of the emergency consultants.
Each successful applicant will share an office with a fellow consultant and be
supported by the secretarial pool based in the department.
Standard Duties and responsibilities
a) To participate in development
of and undertake all duties and functions pertinent to the Consultant’s area of
competence, as set out within the Clinical Directorate Service Plan and in line with policies as specified by the
Employer.
b) To ensure that duties and
functions are undertaken in a manner that minimises delays for patients and
possible disruption of services.
c) To work within the framework
of the hospital / agency’s service plan and/or levels of service (volume, types
etc.) as determined by the Employer. Service planning for individual clinical
services will be progressed through the Clinical Directorate structure or other
arrangements as apply.
d) To co-operate with the
expeditious implementation of the Disciplinary Procedure
e) To formally review the
execution of the Clinical Directorate Service Plan with the Clinical Director /
Employer periodically. The Clinical Directorate Service Plan shall be reviewed
periodically at the request of the Consultant or Clinical Director / Employer.
The Consultant may initially seek internal review of the determinations of the
Clinical Director regarding the Service Plan.
f) To participate in the
development and operation of the Clinical Directorate structure and in such
management or representative structures as are in place or being developed. The
Consultant shall receive training and support to enable him/her to participate
fully in such structures.
g) To provide, as appropriate,
consultation in the Consultant’s area of designated expertise in respect of
patients of other Consultants at their request.
h) To ensure in consultation with
the Clinical Director that appropriate medical cover is available at all times
having due regard to the implementation of the European Working Time Directive
as it relates to doctors in training.
i) To supervise and be
responsible for diagnosis, treatment and care provided by non-Consultant
Hospital Doctors (NCHDs) treating patients under the Consultant’s care.
j) To participate as a right and
obligation in selection processes for non-Consultant Hospital Doctors and other
staff as appropriate. The Employer will provide training as required. The
Employer shall ensure that a Consultant representative of the relevant
specialty / sub-specialty is involved in the selection process.
k) To participate in clinical
audit and proactive risk management and facilitate production of all
data/information required for same in accordance with regulatory, statutory and
corporate policies and procedures.
l) To participate in and facilitate production of all data/information
required to validate delivery of duties and functions and inform planning and
management of service delivery
Essential Requirements
This post is open to doctors who hold Registration
as a Specialist in the Specialist Division of the Registrar of Medical
Practitioners maintained by the Medical Council in Ireland in the specialty of Emergency
Medicine
Office Accommodation
Office accommodation with appropriate IT facilities
will be made available within the department together with secretarial support.
All consultants are required to check their emails on a regular basis. The ED
consultant body meets weekly.
There is a multidisclinary team meeting once per week.
There are two a monthly, multidisciplinary governance
meetings chaired by the governance lead with actions, and data distributed to
the wide ED team. There are weekly Departmental Meetings for teaching and
training. The Hospital Consultant body meets monthly as the Medical Staff
Committee to discuss issues affecting the Medical Staff as a whole. All
consultants are expected to participate in the management of their own
Department and the Hospital as a whole, and will be encouraged to develop their
management and leadership skills.
Service Provision and Cover
during Absences
The
consultant has a continuing responsibility for the care of patients in his/her
charge and for the proper functioning of the service allowing for appropriate
delegation for the training of his/her staff.
He/she
must be available by telephone and able to attend the hospital in a timely
fashion when on-call (40 minutes journey time).
Consultant
staff must ensure that services are covered during his/her planned absences.
Where fixed commitments need to be cancelled during planned absences
appropriate notice (8 weeks) must be given. Absences must be co-ordinated with
other consultant staff in the specialty to ensure senior cover is always
available.
Management Responsibility
All Consultants are required to attend the monthly
Directorate Meetings. Post holders will be expected to share in administrative
duties allocated by mutual agreement within the Directorate. As part of the
Consultant Team, the consultant will
offer mentoring support and lead the existing clinical team through; joint
clinics, one-to-one support, individual case management and complex cases,
identifying development needs and suggesting CPD (Continuous Professional
Development) opportunities and responding to clinical enquiries via e-mail and
telephone.
