Overview
To provide a general ENT service
in the setting of the day surgery unit in our client HSE hospital, outpatients,
and to see referrals from other specialities in the Munster region
To liaise with the main emergency
unit and inpatient unit at the hospital site.
The post holder will collaborate,
support and supervise other practioners as agreed to be appropriate with the
lead clinician and may be required to deputise from time to time for absent
colleagues.
Exceptionally the post holder may
need to perform additional duties in emergencies, unforeseen circumstances or
for continuity of patient care.
The Unit:
The hospitals ENT department is a
spoke of the Regional (Munster) ENT service. The hub in University Hospital has
regional ENT services including head and neck surgery, academic unit, skull
base surgery etc. The ENT department has day case theatre sessions on
Monday/Tuesday/Wednesday, outpatients in on Tuesday/Wednesday/Thursday and on
Monday/Tuesday/Wednesday and Friday.
Scope of ENT service:
All aspects of ENT are seen in
the outpatients. There are nurse lead aural care, allergy clinics. There are
rapid access neck lump and head and neck cancer diagnosis clinics.
There are special paediatric
clinics and liaison with a consultant colleague (Consultant Paediatric
Audiological Physician)
The theatre cases include all
aspects of ENT with the exception of head and neck cancer resection,
tonsillectomy and some major cases or minor surgery on ill patients who are
sent to University Hospital.
Scope of ENT Surgery:
Otology such as stapes surgery,
tympanoplasty, meatoplasty, mastoidectomy, ossiculoplasty, pinnaplasty, grommet
insertion.
Rhinology such as FESS,
septorhinoplasty, septoplasty, turbinectomy, laser dacrocystorhinostomy,
microdebrider.
Endoscopy, pharyngoscopy,
oesophagoscopy, microlaryngoscopy. Facial plastic surgery for skin cancer with
flap repair.
The Post
Virtually all aspects of ENT are
carried out by the Consultants, except those requiring regional involvement eg
skull base surgery and specialised paediatrics.
Day surgery is performed, inpatient surgery and emergencies. The Munster
region head and neck cancer service works on a hub and spoke model with all
major head and neck cancer surgery taking place. The post holder will provide
assessment for head and neck cancer and ongoing follow-up after treatment in
addition to thyroid surgery and general ENT. They will attend the MDT meetings
for Head and Neck and Thyroid. There is a strong emphasis on day surgery and
accelerated recovery; and as well as the usual basket of procedures, it is
standard practice for patients undergoing mastoidectomy, myringoplasty,
stapedectomy, septorhinoplasty, endoscopy, endoscopic sinus surgery,
adenoidectomy etc. to be carried out as day cases.
The department currently consists
of 3 Consultants, a Specialty Doctor, and 6 SHO 1/2 doctors working jointly
with the hospital, and at any one time there are two SHO 1/2’s present at the hospital
between the hours of 9.00 am and 5.00 pm, attending clinics and theatres and
dealing with post-operative reviews and emergency work. The post-holder will be
expected to contribute to the training of the registrars.
The post holder will enjoy
secretarial support to maintain their clinical practice. Currently ENT
Consultants share an office with their named secretary. In addition head and
neck cancer patients are supported by a full-time head and neck clinical nurse specialist,
who assists with the day to day care of head and neck patients.
General
This is a new post. In
conjunction with Consultant and Senior colleagues, the post-holder will provide
a service in ENT with responsibility for head and neck, thyroid and general
ENT.
1. In conjunction with Consultant and Senior
colleagues, to play a full part in the out-of-hours On-Call service for the
department. This includes being on-call
for telephone advice and major incidents. Currently the on call frequency is 1
in 9 with Consultants.
2. To provide cover for Consultant and Senior
colleagues in respect of periods of leave.
3. In conjunction with Consultant and Senior
colleagues, to take part in medical audit and research as appropriate.
4. In conjunction with Consultant and Senior
colleagues, to ensure that the requirements of clinical governance are met.
5. To ensure that there are adequate arrangements for
hospital staff involved in the care of your patients to be able to contact you
when necessary
Junior Medical Staff
6. In conjunction with Consultant and Senior
colleagues, to play a full part in the professional supervision and management
of junior medical staff.
7. In conjunction with Consultant and Senior
colleagues, to take responsibility for and devote time to teaching, examination
and accreditation duties as required for junior medical staff.
Standard
Duties and responsibilities
a) To participate in development
of and undertake all duties and functions pertinent to the Consultant’s area of
competence, as set out within the Clinical Directorate Service Plan and in line with policies as specified by the
Employer.
b) To ensure that duties and
functions are undertaken in a manner that minimises delays for patients and
possible disruption of services.
c) To work within the framework
of the hospital / agency’s service plan and/or levels of service (volume, types
etc.) as determined by the Employer. Service planning for individual clinical
services will be progressed through the Clinical Directorate structure or other
arrangements as apply.
d) To co-operate with the
expeditious implementation of the Disciplinary Procedure
e) To formally review the
execution of the Clinical Directorate Service Plan with the Clinical Director /
Employer periodically. The Clinical Directorate Service Plan shall be reviewed
periodically at the request of the Consultant or Clinical Director / Employer.
The Consultant may initially seek internal review of the determinations of the
Clinical Director regarding the Service Plan.
f) To participate in the
development and operation of the Clinical Directorate structure and in such
management or representative structures as are in place or being developed. The
Consultant shall receive training and support to enable him/her to participate
fully in such structures.
g) To provide, as appropriate,
consultation in the Consultant’s area of designated expertise in respect of
patients of other Consultants at their request.
h) To ensure in consultation with
the Clinical Director that appropriate medical cover is available at all times
having due regard to the implementation of the European Working Time Directive
as it relates to doctors in training.
i) To supervise and be
responsible for diagnosis, treatment and care provided by non-Consultant
Hospital Doctors (NCHDs) treating patients under the Consultant’s care.
j) To participate as a right and
obligation in selection processes for non-Consultant Hospital Doctors and other
staff as appropriate. The Employer will provide training as required. The
Employer shall ensure that a Consultant representative of the relevant
specialty / sub-specialty is involved in the selection process.
k) To participate in clinical
audit and proactive risk management and facilitate production of all
data/information required for same in accordance with regulatory, statutory and
corporate policies and procedures.
l) To participate in and
facilitate production of all data/information required to validate delivery of
duties and functions and inform planning and management of service delivery
Teaching Commitments

regular commitments to ward based teaching of
undergraduates.

participation in the lecture programmes for 2nd
and 4th & 5th year medical students.

teaching of other groups as required including other
specialty juniors and specialist nurses.
Job Plan
A formal job plan will be agreed between the appointee, the Operational Medical
Director and Lead ENT Surgery Consultant, on
behalf of the Trust’s Medical Director, based on the provisional timetable.
This will be signed by the Chief Executive and will be effective from the
commencement date of the appointment.
It will be reviewed at 3 months & thereafter
annually or at any time, but no less than 3 months after a previous review, as
requested by the appointee, Operational Medical Director or Clinical Lead &
adjusted accordingly to the agreement of both parties.
This is an

This post is open to doctors who hold Registration
as a Specialist in the Specialist Division of the Registrar of Medical
Practitioners maintained by the Medical Council in Ireland in the specialty of
Clinical and Surgery-
ENT.

Management Responsibility
All Consultants are required to attend the monthly
Directorate Meetings. Post holders will be expected to share in administrative
duties allocated by mutual agreement within the Directorate. As part of the
Consultant Team, the consultant will
offer mentoring support and lead the existing clinical team through; joint
clinics, one-to-one support, individual case management and complex cases,
identifying development needs and suggesting CPD (Continuous Professional
Development) opportunities and responding to clinical enquiries via e-mail and
telephone.
