Overview
A HSE Trust wishes to appoint a Consultant Physician in General and Respiratory
Medicine with a special interest in the care of patients with Lung Cancer. This
is a substantive replacement post. Applicants wishing to work less than full
time or to job share are encouraged to apply, we are happy to discuss
modifications to the suggested timetable. The post will involve responsibility
for general respiratory inpatients and running the two-week wait clinic for
patients with suspected lung cancer, as well as involvement in the respiratory
outpatient and bronchoscopy services, and in the General Medical on call rota.
Our team of respiratory physicians includes 10.5 whole time equivalent
Consultants plus an Associate
Specialist. The Respiratory Department is a large
teaching hospital department providing regional
specialist services in CF, TB, sleep and
non-invasive ventilation, as well as a full respiratory service to the local
population. There is an active research programme in CF, COPD, sleep, pulmonary
physiology and pulmonary hypertension. The respiratory department underwent
complete refurbishment in 2009, including a newly-built ward, additional office
space, a new exercise laboratory and a greatly improved clinic area.
The respiratory consultants participate in the general medical on-call rota,
which has recently been improved to provide enhanced weekend care. The Lung
Cancer team includes two Specialist Nurses. There is a regional Cardiothoracic
Surgical unit at the hospital and a regional oncology and radiotherapy service
in the neighbouring Dublin Oncology Centre. There is a weekly lung cancer
clinic and multi-disciplinary meeting with excellent radiology, pathology,
surgery, oncology, specialist nursing and palliative care support. There are
two bronchoscopy lists per week covered by the respiratory physicians by
rotation. We do not currently have an EBUS service, but they are keen to
develop this.
Clinical
Duties

Clinical:
The successful applicant will
have responsibility for the care of the patients entrusted to him or her.
All consultants are expected to
have a flexible attitude towards the work of the Directorate.
The successful candidates will be
expected to comply with all relevant Trust policies relating to administration
of patient care. This includes working to set timescales for the triage of
referrals, the completion of discharge summaries prior to patients being
discharged, following the specific process for the management of patients that
do not attend (DNA) their appointment or admission, and the completion of all
paperwork deemed necessary to support the management of their patients within
agreed timescales. Consultants are also expected to work with service managers
and clinical colleagues to manage their caseload in a flexible way, to ensure
clinical, national and local, priorities, such as maximum waiting times for
referral to treatment, are achieved.
This post will have 10 programmed activities (PA). .
The PAs will include:
· 8.5 PAs for direct clinical care; this includes one
PA for predictable on-call clinical work associated with general medical on
call work.
· 1.5 PAs for supporting professional activities
(including teaching, supervision, audit, research and non-clinical management).
An additional 1 SPA session is available by agreement with the Head of Division
for consultants undertaking additional approved SPA activity (e.g. additional
responsibilities in training, teaching, research, management or clinical
governance).
· If a less-than-full-time work pattern is agreed,
these commitments will be reduced pro-rata.
Duties of
the post:
The appointee will take the
primary responsibility for running the 2 week wait clinic, supported by the
other Respiratory Physicians and will Chair the Lung MDM. Other duties include:
· Day to day troubleshooting on lung cancer issues,
monitoring monthly performance data from lung cancer office
· Liaison and supervision of lung cancer nurses.
· Supervision of lung cancer clinic service and
pathway.
The appointee will share responsibility for the management of the general and respiratory medical inpatients on the respiratory wards in rotation with the other consultants supported by a team of junior medical staff. There will be time for other outpatient activities, bronchoscopy and subspecialist interests.
Job plan
A formal job plan will be agreed between the appointee, the Operational Medical
Director and Lead Respiratory Medicine Consultant, on behalf of the Trust’s
Medical Director, based on the provisional timetable. This will be signed by
the Chief Executive and will be effective from the commencement date of the
appointment.
It will be reviewed at 3 months
& thereafter annually or at any time, but no less than 3 months after a
previous review, as requested by the appointee, Operational Medical Director or
Clinical Lead & adjusted accordingly to the agreement of both parties.
Provisional assessment of Programmed Activities in Job Plan
For a whole-time contract:
· Direct Clinical Care 8.5 PAs on average per week
(includes clinical activity, clinically related activity, predictable and
unpredictable emergency work)
· Supporting Professional Activities 1.5 PAs on
average per week (includes CPD, audit, teaching and research) Department of
Health Guidance advises that no new post should be advertised for more than a
10 PA job plan.
There is the possibility that up
to 2 additional PAs may be available should the candidate wish to work up to 12
PAs. A view of the job plan will take place three months after the Consultant
has taken up the post. The work diary may demonstrate the need for an increased
number of programmed activities. Appropriate recognition/funding of this
activity will be backdated to the date of commencement of work
On-Call Commitment
The appointee will participate in
the General Medicine on-call rota. This is staffed by Consultant Physicians
from the Respiratory, Gastroenterology, Hepatology, Care of Elderly and
Diabetes and Endocrinology departments, supported by a full quota of medical
trainees.
· 7 consultants including the new appointee will
cover Mondays on call in rotation.
· All the Consultant Physicians rotate through
Friday, Saturday and Sunday on-call duties working approximately 1 in 7
weekends.
· When on call, the Consultant Physicians undertake
twice-daily post-take ward rounds reviewing all newly-admitted medical patients
who have not been seen by the Acute Physicians or Cardiologists, plus any other
patients requiring the attention of a consultant physician. The consultant on
call is expected to be free of other clinical commitments on the afternoon of
their take day, and to be actively involved in the management of the take
during this time.
There is no formal on-call arrangement for Respiratory Medicine or CF specialist advice, but all the respiratory physicians accept periodic out-of-hours requests for advice regarding specialist matters.
Teaching Commitments

specialty juniors and specialist nurses.
Requirements
This post is open to doctors who hold Registration
as a Specialist in the Specialist Division of the Registrar of Medical
Practitioners maintained by the Medical Council in Ireland in the specialty of
Clinical and Respiratory Medicine.

Management Responsibility
All Consultants are required to attend the monthly
Directorate Meetings. Post holders will be expected to share in administrative
duties allocated by mutual agreement within the Directorate. As part of the
Consultant Team, the consultant will
offer mentoring support and lead the existing clinical team through; joint
clinics, one-to-one support, individual case management and complex cases,
identifying development needs and suggesting CPD (Continuous Professional
Development) opportunities and responding to clinical enquiries via e-mail and
telephone.
