Overview

 

Applications are invited for a Consultant post in Rheumatology/General Medicine at HSE University Hospital facilities. The post in based in a hospital in the Leinster region with 75/25 split between rheumatology and general medicine.

The Rheumatology SDU currently sits within the Division of Integrated Medicine which is led by a Divisional Chair. The successful applicant will join an enthusiastic team of 5 consultants, 7 Rheumatology Clinical Nurse Specialists, and 2 Rheumatology Specialty Trainees. There is excellent secretarial support within the department and dedicated occupational therapy, podiatry and clinical psychology service. The department is developing MSK ultrasound service to support Early Arthritis Clinics and plans to develop a CTD and vasculitis service. The department is involved in undergraduate and postgraduate teaching and research.

The new posts are created to provide cover for a new medical ward which is currently an outlier ward for medical patients run jointly by other medical teams. The plan is to share the ward cover between four of the consultants with 2 week on the ward and 6 weeks off the ward rota. Triage to the ward will prioritise patients with rheumatological conditions in addition to general medical patients.

Possession of the MRCP (or equivalent) and completion of Higher Specialist Training (or equivalent) in Rheumatology and General Internal Medicine are essential.

Additional Information

You will be able to take advantage of the excellent Consultant contract for the Republic of Ireland. The basic salary range for this post is euros €217,235 to €261,051 Euro’s per annum plus a further euro 26,672 per annum available in Commitment Awards.

Specific Duties

This is post is combining rheumatology (75%) and general medicine (25%). The post are created to provide cover for a new 22 bed medical ward which is currently an outlier ward for medical patients run jointly by other medical teams.

This post is a full time post (10PA) with responsibility for management of Rheumatology and General Medical inpatients and Rheumatology outpatient clinics.

·       Maintenance of the highest clinical standards in the management of General Medicine and Rheumatology

·       To share with colleagues responsibility for the day-to-day management of the Rheumatology Service across the Trust

·       To actively participate in both departmental and Trust matters concerning Clinical Governance and audit.

·       To provide support and training to the rheumatology and medical trainees and specialist nurses.

·       To have responsibility for ensuring active participation in continuing professional development (CPD).

Rheumatology

The Rheumatology clinics include general Rheumatology clinics, Early Arthritis Clinics (with access to MSK ultrasound), Rheumatology Hot Clinics and telephone clinics. The average clinic template includes 2 x 30 min new appointments, 1x 30 min flare appointment and 6x 15 min follow ups. All remaining rheumatology consultants will continue to see inpatient referrals unless admitted to the rheumatology medical ward, where the case load will be shared. The post holder will also participate in clinical pathway development or development of specialist services such as vasculitis or connective tissue disease. The post holder will be expected to provide support for the specialist nurses and junior trainees. There will be opportunities to participate in clinical research and to teach medical undergraduate students from a nearby University. The job plan for the post is based on 10 PAs divided into 8.5 DCC and 1.5 SPAs (1 PA for CPD, 0.25 PA for educational supervision and 0.25 for lead on service development or clinical governance).

General Medicine

The new rheumatology consultant will run a medical ward together with other consultant doctors joining the ward team at a 2:8 ratio (Monday – Friday). Triage to the ward will prioritise patients with rheumatological conditions in addition to general medical patients. The consultants will rotate onto the ward for two weeks every 6 weeks and when covering the wards reduce their outpatient commitment to 1.5 clinics a week. When off the wards they each do 4.5 clinics a week. These posts are required as part of the Division’s ambition to expand consultant provision within Integrated Medicine.

Teaching Commitments

·       regular commitments to ward based teaching of undergraduates.

·       participation in the lecture programmes for 2nd and 4th & 5th year medical students.

·       teaching of other groups as required including other specialty juniors and specialist nurses.

 

Job Plan

A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead Medicine: Rheumatology Surgery Consultant, on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.

It will be reviewed at 3 months & thereafter annually or at any time, but no less than 3 months after a previous review, as requested by the appointee, Operational Medical Director or Clinical Lead & adjusted accordingly to the agreement of both parties.

This job plan is provisional and subject to changes due to service reconfiguration due to COVID19, but maintaining the DCC/SPA split. Some of the general rheumatology clinics may be undertaken from AH as we are currently re-balancing the clinic provision across sites. This will be designed in a way which will minimise the need to travel across sites. Rheumatology clinic templates include 2 new (30min) slots, 1 urgent new/FU slots (30 min) followed by 6-7 FUs.

 

When on wards the team is responsible for a 22 bedded ward. Admin time includes 1.5 PA allocation for clinic generated admin: reviewing results, writing and phoning patients with results, and for reviewing new referrals and advice and guidance and supporting helpline and SPNs. The post holder will participate in the approx 2 in 16-18 weekend on call rota for acute medicine as POD3 who covers the speciality wards and 1 in 13.5 weekdays – unselected take. At the weekend, POD3 looks after the speciality wards and POD 2 looks after the ‘short stay ward’ and assists on AMU POD1 reviews all admissions after the post take ward round within 3 hours until 8pm in addition to AMU cover on the weekend. A day off, usually on Monday, is provided following a weekend on-call.

 

On weekdays there is only a POD1 for the acute medical take. The day begins with a post take ward round and admissions though the day are reviewed by the POD within 3 hours until 8pm. Other clinical commitments are cancelled on POD days. Acute physicians cover the acute admissions 2 days a week with speciality physicians only covering overnight on these days. Category A 3% & 1.25 PA for on-call plus day off post POD weekend. This may change as the hospital implements 7 day working.

 

Bank holiday ward cover will be arranged between the Rheumatology/GIM consultants. Days in lieu will be provided accordingly.

 

Standard Duties and responsibilities

a) To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan  and in line with policies as specified by the Employer.

b) To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.

c) To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.

d) To co-operate with the expeditious implementation of the Disciplinary Procedure 

e) To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.

f) To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.

g) To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.

h) To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.

i) To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.

j) To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 

k) To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.

l) To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery


Essential Requirements

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of General Medicine: Rheumatology

 

Office Accommodation

The post holder will undertake administrative work associated with their clinical and other professional duties.  Adequate time and facilities for clinical administration, including appropriate office space, secretarial support and access to a personal computer, software & internet access, will be available.


Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant  will offer mentoring support and lead the existing clinical team through; joint clinics, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.

 

Service Provision and Cover during Absences

 

The consultant has a continuing responsibility for the care of patients in his/her charge and for the proper functioning of the service allowing for appropriate delegation for the training of his/her staff.

 

He/she must be available by telephone and able to attend the hospital in a timely fashion when on-call (40 minutes journey time).

 

Consultant staff must ensure that services are covered during his/her planned absences. Where fixed commitments need to be cancelled during planned absences appropriate notice (8 weeks) must be given. Absences must be co-ordinated with other consultant staff in the specialty to ensure senior cover is always available.
