Overview

 

Applications are invited for a dynamic and enthusiastic Consultant Paediatric Haematologist. This is a new post developed to support the growing paediatric haematology services provided from a University Hospital in Leinster- Republic of Ireland. The hospital are looking for an enthusiastic colleague to further drive developments in service. The service includes haemostasis, haemoglobinopathies and general non-malignant haematology (including bone marrow failure syndromes and immunohaematology).

There is a provision of non-malignant haematology clinics including subspecialty clinics in haemoglobinopathy and haemostasis along with a number of joint clinics with other specialities. The haematologists work closely with PICU, NICU, general paediatrics, fetal medicine, paediatric oncology and infectious diseases. The service provides an extensive consult service and tertiary advice service to surrounding hospitals in the region. The aim is to develop further services within the local community and develop services with our regional DGH (District General Hospital) partners.

Please note that full IMC registration and license to practice is an essential for this role. Additional Information

You will be able to take advantage of the excellent Consultant contract for the Republic of Ireland. The basic salary range for this post is euros €217,235 to €261,051 Euro’s per annum plus a further euro 26,672 per annum available in Commitment Awards.

Specific Duties

Clinical responsibilities of this post:

This is a substantive 10PA full-time post based at a University Hospital –Republic of Ireland.

Applicants must hold Full and Specialist registration (and with a licence to practise) with the Irish Medical Council (IMC), or be eligible for registration within 6 months of interview.

Applicants that are UK trained must ALSO be a holder of a Certificate of Completion of Training (CCT), or be within 6 months of award of CCT by date of interview. Applicants that are non-UK trained will be required to show evidence of equivalence to the UK CCT

The Haematology Service

The hospital is a large teaching trust in Leinster . They provide community services to residents of the Leinster, local hospital services to 562,000 people in Dublin, Meath, Louth, Monaghan, Drogheda and specialist services to 1.4 million people in SW Leinster and beyond. There is a diverse local population of mixed socioeconomic and ethnic groups.

The paediatric haematology service encompasses clinical and laboratory aspects of the service with a focus on non-malignant and diagnostic haematology. As well as serving the local population, it provides a tertiary referral service for the region. Aspects of the service include Haemoglobinopathies, other congenital anaemias and bone marrow failure disorders, a haemophilia service with European Comprehensive Care Centre status, thrombosis service and immunohaematology. There is an extensive consultative practice including support to neonatal and fetal medicine, oncology, infectious diseases and the Major Trauma Centre. There is a comprehensive diagnostic service with excellent support from four paediatric radiologists with state of the art facilities for MRI, CT and Ultrasound. There are dedicated theatre lists for bone marrow biopsies and other procedures.

The post holder will be expected to lead on paediatric haemoglobinopathies and developing the local and regional consultative service.

The paediatric haematology service benefits from close working relationships with the adult haematology service and the expertise of both services complement each other. The adult service encompasses all aspects of clinical and laboratory haematology. In addition to haemoglobinopathy, immunohaematology and haemophilia services, the department has a large haemato-oncology service and a bone marrow transplant unit with a national and international reputation for the treatment of aplastic anaemia and other bone marrow failure disorders. Both the paediatric and adult haematologists provide laboratory diagnostic services as part of National Network of Pathology.

Outpatient Clinics

There are weekly clinics for paediatric haemostasis, paediatric haemoglobinopathies and general paediatric haematology. The haemoglobinopathy clinics include joint clinics with the paediatric respiratory and endocrine consultants. These clinics are supported by the two paediatric haematology nurse specialists, physiotherapist (for haemophilia) and community paediatric haemoglobinopathy nurse specialist. Referrals are from primary care, secondary care and tertiary referrals from the Leinster Region.

Ward Duties

The postholder will share ward attending for children admitted to hospital with haematological conditions with the other consultant. Patients are admitted via acute paediatrics and the haematologists provide shared care with the acute paediatric consultants. The named consultant for admission is the acute paediatrician of the week.

They will be available to consult on children admitted under the general paediatric team or any of the other specialties. The consultant will have time dedicated to attend the wards on a daily basis. They will also provide advice on paediatric laboratory results and morphological interpretation of blood films, bone marrows, cytospins and other haematological diagnostic investigations. The paediatric haematology consultants are responsible for signing out the laboratory reports for children.

They will also be available for advice out of hours. Currently this is on a 1 in 2 basis for telephone advice only but is subject to review. There is always a haematology Registrar 1st on call covering both adult and paediatric haematology. Out of hours morphological interpretation is covered by the adult haematology service. The on call will usually be covered in fortnightly blocks running concurrently with attending weeks.

Out of hours responsibilities include:

·       Giving advice on management of inpatients (seen daily by paediatric team; all admissions reviewed by the on call paediatric consultant)

·       Giving advice regarding paediatric haematology queries both teams at the University Hospital and network DGHs

Teaching Commitments

·       regular commitments to ward based teaching of undergraduates.

·       participation in the lecture programmes for 2nd and 4th & 5th year medical students.

·       teaching of other groups as required including other specialty juniors and specialist nurses.

 

Job Plan

A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead Paediatric Consultant, on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.

It will be reviewed at 3 months & thereafter annually or at any time, but no less than 3 months after a previous review, as requested by the appointee, Operational Medical Director or Clinical Lead & adjusted accordingly to the agreement of both parties.

The job plan covers 5 days per week at 10 PA. The post holder will have a day off (Monday or Friday) on non-attending weeks

Standard Duties and responsibilities

a) To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan  and in line with policies as specified by the Employer.

b) To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.

c) To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.

d) To co-operate with the expeditious implementation of the Disciplinary Procedure 

e) To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.

f) To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.

g) To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.

h) To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.

i) To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.

j) To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 

k) To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.

l) To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery


Essential Requirements

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of Paediatric – Haematology

Office Accommodation

The post holder will undertake administrative work associated with their clinical and other professional duties.  Adequate time and facilities for clinical administration, including appropriate office space, secretarial support and access to a personal computer, software & internet access, will be available.


Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant  will offer mentoring support and lead the existing clinical team through; joint clinics, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.

 

Service Provision and Cover during Absences

 

The consultant has a continuing responsibility for the care of patients in his/her charge and for the proper functioning of the service allowing for appropriate delegation for the training of his/her staff.

 

He/she must be available by telephone and able to attend the hospital in a timely fashion when on-call (40 minutes journey time).

 

Consultant staff must ensure that services are covered during his/her planned absences. Where fixed commitments need to be cancelled during planned absences appropriate notice (8 weeks) must be given. Absences must be co-ordinated with other consultant staff in the specialty to ensure senior cover is always available.
