Overview
We are looking to appoint a
Consultant Physician with a specialist interest in Nephrology based at our
client’s University Hospital (on a full-time or part-time basis).
The successful candidate will
join a team of six Nephrologists and six (four full-time, two-part time) Acute
Physicians.

registration and license to practice is an essential for this role. If you do
not currently have IMC registration, but you are in the process of applying,

registration at the time of shortlisting will not be considered- as per
hospital requirements.
Additional
Information
You will be able to take
advantage of the excellent Consultant contract for the Republic of Ireland. The
basic salary range for this post is euros €217,235 to €261,051 Euro’s per annum
plus a further euro 26,672 per annum available in Commitment Awards.
Nephrology
The Department of Nephrology is based at the University Hospital and serves the
population of Leinster. The medical
staffing of the department currently consists of four Consultant Nephrologists,
two Acute Physicians with an interest in Nephrology, two Registrars (Nephrology
& G(I)M), and three Foundation doctors (one year two, and two year one); a
Core Medical Trainee.
The
Department currently cares for approximately 325 patients with 40 paediatrics
cases, with end stage renal failure, treated by haemodialysis, peritoneal
dialysis, kidney (and pancreas) transplantation, and conservative
(non-dialytic) care.
The
Department is supported by 3 Chronic Kidney Disease specialist nurses, who run
the anaemia programme and provide on-going care for 200+ low clearance
patients.
The main
Haemodialysis Unit is based at the hospital. This is a 14 stationed unit
operating three shifts a day on a Monday, Wednesday and Friday and two shifts
on a Tuesday, Thursday and Saturday.
There is a satellite unit based at a nearby Hospital with eleven
stations operating two shifts daily Monday to Saturday.
The Therapies Team has been recently expanded to allow more patients to
be supported to dialyse in their own home. This includes peritoneal as well as
haemodialysis and there are training rooms on both sites. There are currently approximately 35 patients
treated with peritoneal dialysis, split between APD and CAPD.
Acute transplantation
is undertaken at the other hospital facility in the region. Assessment and
work-up is undertaken locally and follow-up of post-transplant patients is
undertaken by the base unit at approximately day 10 post-transplant. There are approximately 160 renal transplant
patients currently under supervision in Leinster.
Out-patient
clinics are based at the University Hospital. Access for haemodialysis and
peritoneal dialysis is undertaken via Consultant Surgeons and Radiologists at
the University Hospital. Renal biopsies
(native and transplant) are undertaken by Consultant Radiologists, based at the
hospital. The interpretation of biopsies
is undertaken locally by two Pathologists with a special interest in renal
pathology. Other diagnostic services are
available from the Radiology, Medical Physics, and Blood Sciences Departments,
again based at the University Hospital and General Hospital.
The Renal ward is an eighteen
bedded renal / general medical ward. In addition, there is a haemodialysis
cubicle on the ward. The three
Consultant Nephrologists together with the above mentioned junior staff are
responsible for the inpatients both renal and general medical. The type of
general medical patients received from the admissions unit is dependent on bed
availability and underlying problem wherever possible. Patients are allocated
to individual consultants dependent upon their geographical position in the
ward unless patients are already under active outpatient follow up by an
individual consultant who will then continue their inpatient management. The
Nephrologists take part in the acute general medical rota, which is currently
running on an approximately 1 in 18 basis for the three Consultant
Nephrologists.
The medical
on call is based on the Acute Medical Unit on wat the University Hospital. The
duties are mostly covering evening shifts (5:30pm-10pm present in the hospital,
and then overnight from home) plus occasional weekend shifts for G(I)M, 1 in
every 6 weekends. The on call G(I)M physician supervises the on call team
undertaking unselected general medical admissions.
On call for
nephrology is on the basis of a Nephrologist of the week currently shared
between the four Consultant Nephrologists; two further Consultant Acute
Physicians contribute to the weekend rota.
The renal department regularly undertakes audit both locally and
nationally and the activities are supported by an IT/Audit officer.
Acute Medicine
The Acute Medical Unit is
situated on the hospital site has 30 beds. It comprises acute admission and
short stay for medical admissions only. Patients are admitted direct from the
community following assessment by general practice, or from the Emergency
Department. The typical take is of 30-40 patients over 24 hours. Patients
admitted overnight become the responsibility of the Acute Medicine Consultant
at 8am, and are reviewed as part of a post-take ward round by the Acute
Medicine Consultant.
The junior medical team comprises
3 Acute Medicine Registrars (one of whom is seconded to specialties as per
their training requirements), a rotating General Medicine Registrar, 7 SHO
(Senior House Officer) trainees undertaking a full-shift rota and 7 HO ( House
Officer) trainees undertaking a
full-shift rota
Ambulatory Emergency Care is
located on the ward with 2 assessment cubicles and an assessment bay of 5
spaces. It is open from 9am to 8pm on weekdays and is staffed by Nurse
Practitioners. This area is shared with the General Surgical team. Each day an
average of 10 medical patients attend Ambulatory Emergency Care each day.
Access is through General Practice or the Emergency Department.
The Acute Medicine Consultant
working on the ward for that day holds responsibility for the patients and is
expected to supervise the nurse practitioners and the allocated acute medical
registrar for that day.
Remote cardiac telemetry is
available on the ward and is monitored on CCU. Patients requiring a higher
degree of monitoring are transferred to CCU or HDU, as clinical need requires.
Stroke services run a regional
on-call with thrombolysis undertaken under the remote supervision of the on-call
stroke specialist by the medical registrar.
Cardiology has a dedicated on-call service providing 24 hour PCI cover
for STEMI and unstable NSTEMI, as well as cardiac pacing and advice.
The trust runs a GI Bleed rota
for interventional endoscopy 24 hours a day. This is rota is staffed by
gastroenterologists and general surgeons.

Job Plan
A formal job plan will be agreed between the appointee, the Operational Medical
Director and Lead Medicine: Nephrology Paediatric Consultant, on behalf of the
Trust’s Medical Director, based on the provisional timetable. This will be
signed by the Chief Executive and will be effective from the commencement date
of the appointment.
It will be reviewed at 3 months & thereafter
annually or at any time, but no less than 3 months after a previous review, as
requested by the appointee, Operational Medical Director or Clinical Lead &
adjusted accordingly to the agreement of both parties.

On-call commitment:

(weekend)
Acute Medicine on Call: 6 weekends per year
General (Internal) Medicine on call: 1 in eighteen

Standard Duties and responsibilities
a) To participate in development
of and undertake all duties and functions pertinent to the Consultant’s area of
competence, as set out within the Clinical Directorate Service Plan and in line with policies as specified by the
Employer.
b) To ensure that duties and
functions are undertaken in a manner that minimises delays for patients and
possible disruption of services.
c) To work within the framework
of the hospital / agency’s service plan and/or levels of service (volume, types
etc.) as determined by the Employer. Service planning for individual clinical
services will be progressed through the Clinical Directorate structure or other
arrangements as apply.
d) To co-operate with the
expeditious implementation of the Disciplinary Procedure
e) To formally review the
execution of the Clinical Directorate Service Plan with the Clinical Director /
Employer periodically. The Clinical Directorate Service Plan shall be reviewed
periodically at the request of the Consultant or Clinical Director / Employer.
The Consultant may initially seek internal review of the determinations of the
Clinical Director regarding the Service Plan.
f) To participate in the
development and operation of the Clinical Directorate structure and in such
management or representative structures as are in place or being developed. The
Consultant shall receive training and support to enable him/her to participate
fully in such structures.
g) To provide, as appropriate,
consultation in the Consultant’s area of designated expertise in respect of
patients of other Consultants at their request.
h) To ensure in consultation with
the Clinical Director that appropriate medical cover is available at all times
having due regard to the implementation of the European Working Time Directive
as it relates to doctors in training.
i) To supervise and be
responsible for diagnosis, treatment and care provided by non-Consultant
Hospital Doctors (NCHDs) treating patients under the Consultant’s care.
j) To participate as a right and
obligation in selection processes for non-Consultant Hospital Doctors and other
staff as appropriate. The Employer will provide training as required. The
Employer shall ensure that a Consultant representative of the relevant
specialty / sub-specialty is involved in the selection process.
k) To participate in clinical
audit and proactive risk management and facilitate production of all
data/information required for same in accordance with regulatory, statutory and
corporate policies and procedures.
l) To participate in and facilitate production of all data/information
required to validate delivery of duties and functions and inform planning and
management of service delivery
Essential Requirements
This post is open to doctors who hold Registration
as a Specialist in the Specialist Division of the Registrar of Medical
Practitioners maintained by the Medical Council in Ireland in the specialty of
General Medicine: Nephrology Paediatric
Office Accommodation
The post holder will undertake administrative work
associated with their clinical and other professional duties. Adequate time and
facilities for clinical administration, including appropriate office space,
secretarial support and access to a personal computer, software & internet
access, will be available.
Management Responsibility
All Consultants are required to attend the monthly
Directorate Meetings. Post holders will be expected to share in administrative
duties allocated by mutual agreement within the Directorate. As part of the
Consultant Team, the consultant will
offer mentoring support and lead the existing clinical team through; joint
clinics, one-to-one support, individual case management and complex cases,
identifying development needs and suggesting CPD (Continuous Professional
Development) opportunities and responding to clinical enquiries via e-mail and
telephone.
Service Provision and Cover
during Absences
The
consultant has a continuing responsibility for the care of patients in his/her
charge and for the proper functioning of the service allowing for appropriate
delegation for the training of his/her staff.
He/she
must be available by telephone and able to attend the hospital in a timely
fashion when on-call (40 minutes journey time).
Consultant
staff must ensure that services are covered during his/her planned absences.
Where fixed commitments need to be cancelled during planned absences
appropriate notice (8 weeks) must be given. Absences must be co-ordinated with
other consultant staff in the specialty to ensure senior cover is always
available.
