Overview
The local
neonatal unit is within the Maternity Unit of our client’s hospital facility.
Neonatal intensive Care is available for 4 Neonates, alongside 5
high-dependency cots. There are state of the art monitors, and facilities for
HFOV, cooling therapy and cerebral ultrasound. The delivery rate is
approximately 4,000 deliveries per year. The unit is fully staffed by trained
neonatal nurses with a high recruitment and retention record. The unit is
supported by a pharmacist, medical technicians, dietician, physiotherapist and
speech & language therapist. Separate middle grade and SHO cover are
provided at all times.
The post
holder will be expected to cover Neonatal services on an attending rota provide
twice daily ward rounds. Infants requiring specialist surgical and cardiac care
are transferred out to other providers. There is an excellent NTS service to
support such transfers. Close links are maintained with the Obstetric
Department. There is a Foetal Assessment Unit on-site, and a Consultant
Obstetrician with a special expertise in high risk pregnancy. Close links are
maintained through the National Neonatal Network, with the hospital as the
neonatal intensive care and surgical unit. Babies expected to be born at less
than 27 weeks gestation, or to have complex medical or surgical needs, are
electively transferred in utero to a nearby hospital, in line with network
agreements.
The unit
currently has 7 intensive care cots, 12 high dependency and 16 special care
cots. There are approximately 600 admissions and > 12,000 cot days per
annum. 40% of babies are in-utero or ex-utero transfers; and over a quarter of
all admissions have a surgical condition. 8% of babies are <27 weeks
gestation. The unit cares for all conditions requiring intensive care support,
except complex cardiac and ECMO cases. Available facilities include
conventional and high frequency ventilation, inhaled nitric oxide,
echocardiography, complex imaging and neuro-imaging, CFM, EEG, and two cooling
cots. Other specialist paediatric services include infectious diseases,
ophthalmology, radiology, neurology, genetics, endocrinology, community
paediatrics, ENT, plastic surgery, orthopaedics, neurosurgery, haematology,
oncology, gastroenterology and respiratory medicine, as well as a visiting
cardiologist. The service works closely with the fetal and maternal medicine
department who provide a supra-regional service in fetal medicine. Allied
services include physiotherapy, dietetics, audiology and speech therapy and
there is an excellent Child Development Centre located within the hospital.

Additional
Information
You will be
able to take advantage of the excellent Consultant contract for the Republic of
Ireland. The basic salary range for this post is euros €217,235 to €261,051
Euro’s per annum plus a further euro 26,672 per annum available in Commitment
Awards.
Specific Duties
This is a
post arisen as a result of a vacancy within the Department. We are seeking a
post holder with expertise in Neonatology that could augment the current
provision of services within the Department. The post holder would be expected
to play a significant role in the management and development of the unit,
acting as liaison with the Neonatal Network. They would be expected in due
course to assume the lead role for neonatal services.
The
successful candidate is expected to provide a strong visible presence across
the organisation, be a role model of good leadership and effective
communication at individual, team, directorate and whole organisation level.
They are expected to help shape the development of the Department and to share
the long term vision for a safe and sustainable service, in line with the
current developments. The post is built around a 10 programmed activity job
plan (see below).
The
attending Consultant will cover the neonatal unit from 8 am to 5 pm on a 1:4
rota. The other weeks include outpatient clinics and admin sessions. Annual
leave will be covered internally. On call commitment is 1 in 12 with internal
cover. When on call, the post holder is expected to cross cover General
Paediatrics, Neonatology and Paediatric emergencies within ED. When on call,
the post holder is expected to be on site until 22:00 hrs to ensure compliance
with Quality standards of on- site consultant presence during hours of peak
activity.
The neonatal
service intends to work more closely with the lead units in the neonatal
network to improve training, consistency in care and governance across the
network. As a first step in this initiative, this post holder will be expected
to work some attending weeks on the tertiary neonatal unit at the hospital. The
post holder will be supported to do attending weeks on the high dependency unit
of the neonatal intensive care unit, and, depending on the expertise of the
appointee, may have opportunities to work on the intensive care area as well.
During these periods, Consultant Neonatologists from hospital will cover the
neonatal unit at the nearby hospital.
As a senior
employee of the Trust you will work in close co-operation with, and support
other clinical, medical professional and managerial colleagues in providing
high quality healthcare to the Trust’s patients. Integral to these
responsibilities is the following:-
§ The provision of a first class clinical service
§ Effective leadership to all staff engaged in the specialty
§ Sustaining and developing teaching and research in
conjunction with the hospitals procedures
§ Undertaking all work in accordance with the Trust’s
procedures and operating policies
§ Conducting clinical practice in accordance with contractual
requirements and within the parameters of the Trust’s service plans
§ Maintaining the confidence of business plans and development
strategies formulated for the specialty, the Care Group or the Trust
Clinical:
· Where specified, the Consultant is expected to provide
continuity of care for all the patients coming through the neonatal service
over their attending weeks. They provide support and are available for all
neonatal emergencies during the specified time period.
· The post holder is expected to take on and lead in a special
interest to augment those already provided within the Department
· They would be required to cover the General Paediatrics and
Paediatric Emergency Departments as well when on call and over weekends.
· The Consultant would do at least 2 Paediatric Outpatient
clinic weekly when not attending to include general paediatric problems. The
post holder is expected to support junior doctors and nursing colleagues in the
performance of their duties
· The postholder is expected to provide support to the
maternity service in planning the management of preterm deliveries and women
with complicated pregnancies.
· While on service at the hospital the post holder will be
expected to cover the high dependency area as attending consultant. The HDU
consultant works closely with the other 2 attending consultants to plan patient
care across the unit, while providing direct care to the patients on the 12 bed
HDU. They will attend complex deliveries with the junior medical team, and
provide counselling to mothers on the delivery suite. The HDU caters to a large
number of complex surgical and medical patients. Many patients are shared care
with paediatric surgical and medical specialists, with the lead being provided
by the neonatal consultants.
· The delivery suite at the hospital provides high risk
obstetric care supporting many regional and supra-regional obstetric and fetal
medicine services. The neonatal consultant will attend deliveries where complex
neonatal care and resuscitation is expected to be required.
Teaching Commitments

and 4th & 5th year medical students.

teaching of other groups as required including other
specialty juniors and specialist nurses.
Job Plan
A formal job plan will be agreed between the appointee, the Operational Medical
Director and Lead Paediatrics & Neonates Consultant, on behalf of the
Trust’s Medical Director, based on the provisional timetable. This will be
signed by the Chief Executive and will be effective from the commencement date
of the appointment.
It will be reviewed at 3 months & thereafter
annually or at any time, but no less than 3 months after a previous review, as
requested by the appointee, Operational Medical Director or Clinical Lead &
adjusted accordingly to the agreement of both parties.
On appointment of the post holder, the Clinical
Director or CBU lead will commence discussions to finalise the job plan which
will become operational on appointment. The job plan is an indicative plan of
the duties the post holder is likely to be asked to undertake. Additional
programmed activities may be offered to the post holder. Any offer of
additional programmed activities will be based on the needs of the service and
in line with the Trust objectives and will be reviewed on an annual basis.
Standard Duties and responsibilities
a) To participate in development
of and undertake all duties and functions pertinent to the Consultant’s area of
competence, as set out within the Clinical Directorate Service Plan and in line with policies as specified by the
Employer.
b) To ensure that duties and
functions are undertaken in a manner that minimises delays for patients and
possible disruption of services.
c) To work within the framework
of the hospital / agency’s service plan and/or levels of service (volume, types
etc.) as determined by the Employer. Service planning for individual clinical
services will be progressed through the Clinical Directorate structure or other
arrangements as apply.
d) To co-operate with the
expeditious implementation of the Disciplinary Procedure
e) To formally review the
execution of the Clinical Directorate Service Plan with the Clinical Director /
Employer periodically. The Clinical Directorate Service Plan shall be reviewed
periodically at the request of the Consultant or Clinical Director / Employer.
The Consultant may initially seek internal review of the determinations of the
Clinical Director regarding the Service Plan.
f) To participate in the
development and operation of the Clinical Directorate structure and in such
management or representative structures as are in place or being developed. The
Consultant shall receive training and support to enable him/her to participate
fully in such structures.
g) To provide, as appropriate,
consultation in the Consultant’s area of designated expertise in respect of
patients of other Consultants at their request.
h) To ensure in consultation with
the Clinical Director that appropriate medical cover is available at all times
having due regard to the implementation of the European Working Time Directive
as it relates to doctors in training.
i) To supervise and be
responsible for diagnosis, treatment and care provided by non-Consultant
Hospital Doctors (NCHDs) treating patients under the Consultant’s care.
j) To participate as a right and
obligation in selection processes for non-Consultant Hospital Doctors and other
staff as appropriate. The Employer will provide training as required. The
Employer shall ensure that a Consultant representative of the relevant
specialty / sub-specialty is involved in the selection process.
k) To participate in clinical
audit and proactive risk management and facilitate production of all
data/information required for same in accordance with regulatory, statutory and
corporate policies and procedures.
l) To participate in and facilitate production of all data/information
required to validate delivery of duties and functions and inform planning and
management of service delivery
Essential Requirements
This post is open to doctors who hold Registration
as a Specialist in the Specialist Division of the Registrar of Medical
Practitioners maintained by the Medical Council in Ireland in the specialty of
General Paediatrics & Neonates

Management Responsibility
All Consultants are required to attend the monthly
Directorate Meetings. Post holders will be expected to share in administrative
duties allocated by mutual agreement within the Directorate. As part of the
Consultant Team, the consultant will
offer mentoring support and lead the existing clinical team through; joint
clinics, one-to-one support, individual case management and complex cases,
identifying development needs and suggesting CPD (Continuous Professional
Development) opportunities and responding to clinical enquiries via e-mail and
telephone.
Service Provision and Cover
during Absences
The
consultant has a continuing responsibility for the care of patients in his/her
charge and for the proper functioning of the service allowing for appropriate
delegation for the training of his/her staff.
He/she
must be available by telephone and able to attend the hospital in a timely
fashion when on-call (40 minutes journey time).
Consultant
staff must ensure that services are covered during his/her planned absences.
Where fixed commitments need to be cancelled during planned absences
appropriate notice (8 weeks) must be given. Absences must be co-ordinated with
other consultant staff in the specialty to ensure senior cover is always
available.
