Overview
This is a full time 10PAs post,
newly created because of increasing number of referrals to dermatology service
in our clients hospital based in the Leinster Region. However part-time
applicants will also be considered. The job plan will be determined by clinical
need and space availability and some aspects of the job plan are negotiable.
Specialist interests fitting in with the departmental need is most welcome;
experience in paediatric dermatology is desirable.
· Knowledge and experience of out-patient dermatology
and management of acute dermatoses
· Knowledge and experience of paediatric dermatology
(desirable but not essential)
· Knowledge and experience of out-patient dermatology
and management of tumours including melanoma, squamous cell carcinoma and basal
cell carcinoma
· Knowledge and experience of in-patient and acute
dermatology

registration and license to practice is an essential for this role. If you do

hospital requirements.
Additional Information
You will be able to take
advantage of the excellent Consultant contract for the Republic of Ireland. The
basic salary range for this post is euros €217,235 to €261,051 Euro’s per annum
plus a further euro 26,672 per annum available in Commitment Awards.
Specific Duties
The appointee will be expected to
help provide a Clinical Dermatology service including the key tasks above.
The hospital delivers services to
patients seven days a week and aims to provide equality of treatment and
outcome regardless of the day of the week.
To meet these aims and changing service requirements, the hospital may
need you either to work temporarily outside of your core hours or to
permanently change your core working days and hours. The hospital will give you reasonable notice,
where possible, of temporary changes to your days or hours of work and will
consult with you and/or your staff representatives about substantial permanent
changes, but ultimately, these are changes which may be required of you
The Dermatology Department is
based at the hospital but provides peripheral clinics to the surrounding
counties. The hospital anticipates that the new post-holder would attend other
site clinics once to twice weekly.
The appointee will work closely
with the Dermatology Consultants already in post as well as the Clinical Nurse
Specialists.
The clinics at hospital are held
in the general outpatients department and are well supported by nursing and
reception staff. The current consultants have mixed new and follow up patients
with 15 minute slots for both. New to
follow up ratios and consultant to consultant referrals are monitored as part
of the agreement with the Primary Care hospitals.
The specialist nurses share care for patients
with chronic disease on second-line treatment. They provide pre-treatment
counselling and see new patients with acne.
Rapid access clinics are for new
cases only and have 10 minute slots. A skin cancer specialist nurse provides
support and counselling for tumour patients.
Surgical lists are for
dermatological local anaesthetic surgery only. The list size depends on the
case mix. The complexity of cases will be decided by the post-holder. Surgical
treatment of benign lesions is restricted, as per guidelines issued by the
local hospitals. The hospital currently has plastic surgery service provided
from another nearby hospital with clinics and local anaesthetic lists taking
place at the hospital site. There are strong links between dermatology and
plastic surgery and they hope to strengthen these in future.
Paediatric dermatology is run in
close cooperation with the paediatricians in the outpatient clinic in the
Children’s unit. Children are admitted under the paediatricians and emergency
referrals for children as inpatients are as per shared rota. They have support
from paediatricians who help with eczema follow up and skin prick
testing/allergen advice. The hospital have close links with the paediatric
dermatology unit/plastic surgery in the nearby hospital and tertiary referrals
are sent to the children’s hospital in the region.
Teaching Commitments

regular commitments to ward based teaching of
undergraduates.

participation in the lecture programmes for 2nd
and 4th & 5th year medical students.

teaching of other groups as required including other
specialty juniors and specialist nurses.
Job Plan
A formal job plan will be agreed between the appointee, the Operational Medical
Director and Lead Medicine: Dermatology Consultant, on behalf of the Trust’s
Medical Director, based on the provisional timetable. This will be signed by
the Chief Executive and will be effective from the commencement date of the
appointment.
It will be reviewed at 3 months & thereafter
annually or at any time, but no less than 3 months after a previous review, as
requested by the appointee, Operational Medical Director or Clinical Lead &
adjusted accordingly to the agreement of both parties.
· Total of 3-4 clinics per week with a mix of
specialist clinics, general clinics, peripheral and rapid access clinics. Up to two clinics a week will be a peripheral
clinic,
·
1 surgical list
per week.
·
Attendance at MDT
x 3 a month.
·
Attendance at
dermatology unit meetings x 1 a month.
·
Availability for
ward referrals once to twice weekly. The hospital does not have dermatology
beds. If dermatology patients are admitted it is under the general physicians
and the dermatologist consults as needed.
·
Patient related
administration.
·
All posts at the
hospital have 1.5 SPA’s in line with other hospitals locally. If additional
duties such as Educational Supervisor are undertaken extra payment can be
negotiated.

This gives 3 to 4 unprogrammed
half days a week. The MDT and the dermatology unit meetings do not occupy a
full session.
The clinic profiles would be
expected to be 5 new and 7 follow ups with 15 minute appointments for new
patients and 10 minute slots for follow-ups. The rapid access clinic profiles
are 10 minute appointments with 12 patients. This profile gives sufficient time
to both see patients and complete clinic correspondence.
Ward referrals: demand is low.
Usually see between 0 and 3 a day.
The details of the
timetable will be discussed with the post holder.
Standard Duties and responsibilities
a) To participate in development
of and undertake all duties and functions pertinent to the Consultant’s area of
competence, as set out within the Clinical Directorate Service Plan and in line with policies as specified by the
Employer.
b) To ensure that duties and
functions are undertaken in a manner that minimises delays for patients and
possible disruption of services.
c) To work within the framework
of the hospital / agency’s service plan and/or levels of service (volume, types
etc.) as determined by the Employer. Service planning for individual clinical
services will be progressed through the Clinical Directorate structure or other
arrangements as apply.
d) To co-operate with the
expeditious implementation of the Disciplinary Procedure
e) To formally review the
execution of the Clinical Directorate Service Plan with the Clinical Director /
Employer periodically. The Clinical Directorate Service Plan shall be reviewed
periodically at the request of the Consultant or Clinical Director / Employer.
The Consultant may initially seek internal review of the determinations of the
Clinical Director regarding the Service Plan.
f) To participate in the
development and operation of the Clinical Directorate structure and in such
management or representative structures as are in place or being developed. The
Consultant shall receive training and support to enable him/her to participate
fully in such structures.
g) To provide, as appropriate,
consultation in the Consultant’s area of designated expertise in respect of
patients of other Consultants at their request.
h) To ensure in consultation with
the Clinical Director that appropriate medical cover is available at all times
having due regard to the implementation of the European Working Time Directive
as it relates to doctors in training.
i) To supervise and be
responsible for diagnosis, treatment and care provided by non-Consultant
Hospital Doctors (NCHDs) treating patients under the Consultant’s care.
j) To participate as a right and
obligation in selection processes for non-Consultant Hospital Doctors and other
staff as appropriate. The Employer will provide training as required. The
Employer shall ensure that a Consultant representative of the relevant
specialty / sub-specialty is involved in the selection process.
k) To participate in clinical
audit and proactive risk management and facilitate production of all
data/information required for same in accordance with regulatory, statutory and
corporate policies and procedures.
l) To participate in and facilitate production of all data/information
required to validate delivery of duties and functions and inform planning and
management of service delivery
Essential Requirements
This post is open to doctors who hold Registration
as a Specialist in the Specialist Division of the Registrar of Medical
Practitioners maintained by the Medical Council in Ireland in the specialty of
General Medicine: Dermatology
Office Accommodation
The post holder will undertake administrative work
associated with their clinical and other professional duties. Adequate time and
facilities for clinical administration, including appropriate office space,
secretarial support and access to a personal computer, software & internet
access, will be available.
Management Responsibility
All Consultants are required to attend the monthly
Directorate Meetings. Post holders will be expected to share in administrative
duties allocated by mutual agreement within the Directorate. As part of the
Consultant Team, the consultant will
offer mentoring support and lead the existing clinical team through; joint
clinics, one-to-one support, individual case management and complex cases,
identifying development needs and suggesting CPD (Continuous Professional
Development) opportunities and responding to clinical enquiries via e-mail and
telephone.
Service Provision and Cover
during Absences
The
consultant has a continuing responsibility for the care of patients in his/her
charge and for the proper functioning of the service allowing for appropriate delegation
for the training of his/her staff.
He/she
must be available by telephone and able to attend the hospital in a timely
fashion when on-call (40 minutes journey time).
Consultant
staff must ensure that services are covered during his/her planned absences.
Where fixed commitments need to be cancelled during planned absences
appropriate notice (8 weeks) must be given. Absences must be co-ordinated with
other consultant staff in the specialty to ensure senior cover is always
available.
