Overview

 

This post forms part of a sustained development in Breast imaging to meet rising demand and replace sessions following the departure of a colleague form our client radiology team.

 

 The successful candidate will join a cohesive team of four Consultant Radiologists and a Consultant Breast Clinician in a dedicated breast unit for symptomatic and screening services. The successful applicant will also work in conjunction with other Consultant Radiologists delivering diagnostic and interventional Radiology services. The split between General and Breast work will be flexible to address any potential candidates experience and career aspirations. The successful candidate will be a Fellow of, and accredited by, the Royal College of Radiologists.

 

The hospital has an outpatient unit dedicated to the diagnosis and treatment of breast disease and is located at the main hospital site. 500 new breast cancer cases are diagnosed each year through the symptomatic and screening services. There is also a dedicated family history/high risk screening service. They are fully digitalised with an integrated PACS and NBSS; three static units (Siemens Mammomat Inspiration, two with stereotactic equipment), and two vans with Siemens units to deliver the screening rounds. There are three Toshiba Aplio ultrasound machines. Breast MRI is undertaken on a 1.5T Philips Achieva scanner with a dedicated seven-channel breast coil. The biopsy equipment includes a vacuum assisted biopsy device (Encor). They provide MRI guided biopsies as well. There is a full range of nuclear medicine investigations and we use isotopes for localisation biopsies and sentinel lymph node biopsy.

Additional Information


You will be able to take advantage of the excellent Consultant contract for the Republic of Ireland. The basic salary range for this post is euros €217,235 to €261,051 Euro’s per annum plus a further euro 26,672 per annum available in Commitment Awards.

 

Unit Workload

They are currently in the ninth round of the screening programme and during this time will be inviting 75,000 women for screening.

 

There are approximately 4,000 new referrals to the symptomatic service each year, which are seen in one-stop-clinics together with the surgeons.

 

Skill Mix

 

The Breast Unit includes a Consultant Radiographer, who performs the full range of breast investigations as well as an advanced practitioner performing stereotactic biopsies and supporting family history service.

Specific Duties

Provision with Consultant Colleagues of Breast Radiology services to the Hospitals’ Group, with responsibility for the prevention, diagnosis and treatment of illness.

·       Provision of General Radiology services to the hospital.

·        Support and ensure the efficient functioning of the department and breast imaging service

·       Cover for colleagues' annual leave and other authorised absences

·       Professional supervision and management of junior medical staff

·       Contribution to undergraduate, postgraduate and continuing medical education activity

·       Participation in medical audit, the Trust’s Clinical Governance processes and in CPD

·       Managerial responsibilities where appropriate

·       To maintain a safe and healthy environment for patients, visitors and staff

·       To ensure that all duties are carried out to the highest possible standard, and in accordance with current quality initiatives within the area of work

Job Plan

A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead Radiology Consultant, on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.

The exact timetable will be flexible and arranged by discussion with the Specialty Director, to whom the post-holder will report, to meet the needs of the department and the individual. The job plan will follow the basis of the new consultant contract and comprise a minimum of 10 PAs for a full time post. The example below assumes a contribution to the general departmental workload. As detailed above the split between Breast and general Radiology will be subject to negotiation depending upon a individuals expertise and career aspirations.

 

A new Consultant will be offered 2.5 PA’s of SPA for the first year in post and then 1.5 PA’s thereafter. Established Consultants are offered 1.5PA’s of SPA from commencement. 1 SPA may be worked flexibly in a time and place to cover core CPD, informal teaching, audit, departmental meetings and non-clinical management activity. Further SPA allocations are calculated for formal teaching as well as for any additional responsibilities the consultant may have. After accounting for fixed clinical commitments such as MDT meetings, the remaining time is allocated to direct clinical care. Work flexibly to cover some colleagues’ leave and teaching commitments and this work is incorporated into the calculations. There are a number of Specialist Registrars from the Radiology Academy who rotate through the department.

 

Review of the job plan will be carried out annually in conjunction with the Specialty and Divisional Directors and is a requirement of the appraisal process.

 

The performance of audit and CPD and attendance at multi-disciplinary team meetings are contractual requirements.

 

A Consultant appraisal system is established and the post-holder will be required to undertake appraisal by a Trust approved Consultant appraiser colleague annually. The Trust also implements an annual mandatory training programme for all staff that covers Health and Safety; Infection Control; Basic Life Support and similar topics. It is the individual staff member’s contractual responsibility to ensure this training is kept up to date.

Standard Duties and responsibilities

a) To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan  and in line with policies as specified by the Employer.

b) To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.

c) To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.

d) To co-operate with the expeditious implementation of the Disciplinary Procedure 

e) To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.

f) To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.

g) To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.

h) To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.

i) To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.

j) To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 

k) To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.

l) To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery


Essential Requirements

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of Radiology: Breast

Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant  will offer mentoring support and lead the existing clinical team through; joint clinics, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.

 

Service Provision and Cover during Absences

 

The consultant has a continuing responsibility for the care of patients in his/her charge and for the proper functioning of the service allowing for appropriate delegation for the training of his/her staff.

 

He/she must be available by telephone and able to attend the hospital in a timely fashion when on-call (40 minutes journey time).

 

Consultant staff must ensure that services are covered during his/her planned absences. Where fixed commitments need to be cancelled during planned absences appropriate notice (8 weeks) must be given. Absences must be co-ordinated with other consultant staff in the specialty to ensure senior cover is always available.
