Overview

 

We are looking for an enthusiastic individual with a specialist interest in Pain Medicine and a range of anaesthetic specialities, who will provide a high quality anaesthetic service and pain management service whilst contributing fully to the activities of the anaesthetic department and hospital, to complement the present department.

Please note that full IMC registration and license to practice is an essential for this role. If you do not currently have IMC registration, but you are in the process of applying, please use the supporting information section of the application form to provide an update about the status of your IMC application.

Candidates without IMC registration at the time of shortlisting will not be considered- as per hospital requirements.

Additional Information

You will be able to take advantage of the excellent Consultant contract for the Republic of Ireland. The basic salary range for this post is euros €217,235 to €261,051 Euro’s per annum plus a further euro 26,672 per annum available in Commitment Awards.

The Anaesthetic Department  

The Anaesthetic Department is part of the Elective Care division Anaesthesia Services span the 8 main theatres, plus satellite theatres in dedicated units including Urology, Ophthalmology, Day Surgery, Oral Surgery, ECT and Maternity.

The Anaesthetic Department has accommodation adjacent to the Theatres at the hospital. The Department has an administration team within the Anaesthetic Office and on the Intensive Care Unit they have a Departmental Secretary and an ICNARC Administrator.

The department now includes the secretarial office, shared consultant offices and a trainee room, which holds the departmental library and is used for the trainees’ tutorial sessions. There are also coffee/tea making facilities and toilet facilities within the department.

As well as this the department has its own Seminar Room which is situated in the theatre suite, in close proximity to the Anaesthetic Department. This is an excellent sized room for audit meetings, consultant meetings and training sessions for use by all staff within the Directorate. Facilities in this area include the seminar room, toilet facilities and a beverage room, television and video, overhead projector, slide projector and flipchart.

The Chronic Pain Management Service

The multidisciplinary pain management team consists of nursing, clinical psychology, physiotherapy and consultants in pain medicine. All team members have dedicated sessions for the chronic pain service. The team works very closely together and carries out multidisciplinary new patient assessments. A broad range of therapy options are offered including a full pain management programme which is run multiple times each year. Dedicated office space is provided within the main theatre suite.

THE WORK OF THE DEPARTMENT


 
MATERNITY UNIT

The Maternity Unit has been significantly redeveloped as part of the `making it better` reconfiguration of Obstetric Services, which places the hospital as a major women`s healthcare provider, workload is likely to increase to > 6500 deliveries per annum. The Unit has its own newly refurbished two theatre suite, which is equipped with modern, full monitoring equipment and recovery facilities. A 24 hour epidural analgesia service is provided

c)       OPHTHALMOLOGY

There is a purpose built Ophthalmology Unit and 2 dedicated theatres which have excellent monitoring, staffing and recovery facilities.

d)       UROLOGY

The Urology Department has its own theatre and day case ward.

e)       INTENSIVE CARE UNIT

The Intensive Care Unit, is currently an 8 bedded level 3 unit. Twelve consultant anaesthetists have a sessional commitment to critical care.

f)        HIGH DEPENDENCY UNIT

Ten bedded level 2 facility, which receives patients from all acute specialties.

g)       DAY CARE SURGICAL UNIT

The Surgical Day Care Unit has facilities for 15 trolleys and has two dedicated theatres and a minor procedure room.

h)       ECT

Two sessions per week of ECTs are carried out.

i)         ORAL SURGERY SESSIONS

Regular oral surgery anaesthetic sessions are held in the Main Operating Theatres.

j)         PAEDIATRICS

An expanding service as part of MIB, with routine / urgent anaesthesia for children as young as 2 years old, provided on dedicated paediatric lists including Orthopaedics, ENT, Ophthalmology, Dental, Urology and General Surgery. There is also involvement with stabilisation of critically ill children prior to transfer out to tertiary paediatric centres.

Specific Duties

1)

a) Work as part of the multidisciplinary chronic pain management team. The applicant will be expected to work closely with all members of the multidisciplinary team. The applicant will also be expected to work with specialist physiotherapists in joint outpatient clinics.

b) Provision of range of anaesthetic services and contribute to a proper functioning anaesthetic department.

c) Out of hours cover on call dependant on the rota requirements for which an appropriate supplement will be paid. Also participate in the weekend trauma theatre rota.

d) Responsibility for the supervision and education of junior medical and other clinical staff where appropriate.

e) An expectation that the post holder will work with managers and colleagues towards achieving the goals and service delivery targets of the department and Trust while ensuring the quality of delivery of service.

f) A requirement to respond positively to appropriate requests for assistance wherever possible in the event of a major incident or other event where anaesthetic or professional expertise is felt to be advantageous.

g) A requirement to comply with HSE Policies.

2) Clinical

a) A job plan for the post (draft attached) will be agreed with the candidate after appointment.

b) The sessions for operating work will be arranged to suit the departmental needs, including pre/post/operative care, emergencies, administration and teaching. Some flexibility and changes are inevitable when colleagues are away. Candidates will be encouraged to pursue specialist interests where possible.

3) Continuing Education and Professional Development

a) This is a crucial aspect of the appointment as part of provision of high quality healthcare and responsibility for clinical governance. It is an essential part of the individual’s development portfolio and department service development. Appropriate time and funding will be allocated according to local policy

Teaching Commitments

·       regular commitments to ward based teaching of undergraduates.

·       participation in the lecture programmes for 2nd and 4th & 5th year medical students.

·       teaching of other groups as required including other specialty juniors and specialist nurses.

 

Job Plan

A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead Anaesthesia: Pain Management Consultant, on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.

It will be reviewed at 3 months & thereafter annually or at any time, but no less than 3 months after a previous review, as requested by the appointee, Operational Medical Director or Clinical Lead & adjusted accordingly to the agreement of both parties.

Two chronic pain management clinics 1.25 PA each (to include all DCC time related to the clinics) 2x1.25 = 2.5PA


One interventional pain theatre session 1.25 PA (to include all DCC time related to the session) 1x1.25 = 1.25PA

 

Additional chronic pain management direct clinical care (GP correspondence, triaging referrals etc.) 0.25 PA

 

Anaesthetic theatre sessions 1.25 PA each 2x1.25 = 2.5 PA


On call 1 in 17 = 1 PA


Trauma weekends 1 in 17 = 0.4 PA


SPA 1.5 - 2.5 PA depending on activity

Total DCC 7.9 PA

Job plan to be agreed following appointment

 

Standard Duties and responsibilities

a) To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan  and in line with policies as specified by the Employer.

b) To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.

c) To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.

d) To co-operate with the expeditious implementation of the Disciplinary Procedure 

e) To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.

f) To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.

g) To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.

h) To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.

i) To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.

j) To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 

k) To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.

l) To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery


Essential Requirements

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of Anaesthesia: Pain Management

Office Accommodation

The post holder will undertake administrative work associated with their clinical and other professional duties.  Adequate time and facilities for clinical administration, including appropriate office space, secretarial support and access to a personal computer, software & internet access, will be available.


Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant  will offer mentoring support and lead the existing clinical team through; joint clinics, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.

 

Service Provision and Cover during Absences

 

The consultant has a continuing responsibility for the care of patients in his/her charge and for the proper functioning of the service allowing for appropriate delegation for the training of his/her staff.

 

He/she must be available by telephone and able to attend the hospital in a timely fashion when on-call (40 minutes journey time).

 

Consultant staff must ensure that services are covered during his/her planned absences. Where fixed commitments need to be cancelled during planned absences appropriate notice (8 weeks) must be given. Absences must be co-ordinated with other consultant staff in the specialty to ensure senior cover is always available.
