The Department

These new posts will join other consultant Radiologists, delivering all aspects of diagnostic imaging and interventional radiology. The Radiology department at the hospital, is a large teaching department responsible for specialist registrar training. The main x-ray service is accommodated within two large adjacent departments, and a third smaller one in A&E. The delivery of radiology services is currently organised to optimise specialist services, in addition to provision of primary and secondary care imaging. There are four multi-slice CT scanners (GE HD 750 128 slice, GE XTE 64 slice, GE Optima 64 slice and GE Lightspeed 16 slice) providing 600 CTs per week. All scanners are capable of performing CT angiography, CT Colonography and CT Urography. There are Specialist workstations for advanced applications such as Gemstone spectral imaging, coronary artery angiography, CT colonography, and perfusion imaging. The Lightspeed replacement has been agreed and it is planned that this will be co-located with a further scanner specifically to serve ED when the emergency department reconfiguration is complete. CT services are operated on an extended working day and at weekends. There are four MRI scanners (Siemens 1.5T Avanto, Philips 1.5T Ingenia, Philips 1.5T Achieva mobile and Esaote 0.3T extremities) providing in excess of 330 examinations per week. The equipment base provides for Perfusion Weighted and Diffusion Weighted Imaging.

The MRI service operates an extended working day – from 8am to 7pm with a radiographic shift-system. In addition to 13 x-ray rooms, and three fluoroscopy suites, the department includes two Phillips interventional suites; one of which includes a 3D rotational facility and bi-planar imaging. Plans to develop an interventional theatre are at an advanced stage.

The ultrasound department is based within x-ray and provides a full diagnostic service. There are six nonobstetric ultrasound examination rooms, all equipped with modern high specification machines (mostly

Toshiba Aplio XG), and in addition there are 3 new Toshiba Viamo systems for portable/interventional work. There is also a Hitachi EUS system.

 

The hospital has a full Insignia PACS service connected to all Imaging modalities, installed in 2013. There is Voice Recognition integrated to the Radiology Information System and PACS.


Clinical Duties (guideline only)

·       The post is to assist the Breast Unit team in delivery of a high-quality screening and symptomatic radiology service, and to share responsibility for provision of a comprehensive diagnostic radiology service. Breast radiologists keen to do symptomatic work without a screening commitment would also be considered. All radiology consultants are actively involved in teaching for the Medical School and Radiology Training Program.

·       The successful applicant will share an office and secretarial support with consultant colleagues. A networked PC will be provided with this post and there will be office access to a PACS reporting workstation. Development of competence with voice recognition software will be required.

·       The post holder will be required to comply with all Trust Policies notably in relation to job planning, appraisal and leave taking in line with Departmental and Trust objectives. The programs shown are for illustrative purposes and could be subject to modification, in agreement with the post holder and the Clinical Director.

·       The post holder will be expected to share the on –call commitment equally with the existing consultants, although with the advent of 7 day working the on call responsibility will change.

·       There are separate on-call rotas for Interventional Radiology and Neuroradiology

·       The Directorate is planning to deliver extended and 7 day working (other than on-call) and the job plan and timetable will reflect this ambition in common with existing consultants

·       The Directorate would support any research interests of the Consultant in conjunction with the Trust lead for R & D. All consultants are expected to actively participate in the department’s audit and clinical governance program.

·       The successful candidate will be expected to share in the support of MDT meetings with consultant colleagues.

Standard Duties and responsibilities

a) To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan  and in line with policies as specified by the Employer.

b) To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.

c) To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.

d) To co-operate with the expeditious implementation of the Disciplinary Procedure 

e) To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.

f) To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.

g) To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.

h) To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.

i) To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.

j) To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 

k) To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.

l) To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery

Teaching Commitments

·       regular commitments to ward based teaching of undergraduates.

·       participation in the lecture programmes for 2nd and 4th & 5th year medical students.

·       teaching of other groups as required including other specialty juniors and specialist nurses.

Job plan

The job plan is presented in annualised format and is indicative. The successful appointee will negotiate with the clinical director and consultant colleagues how the job plan is timetabled. Some of the sessions will be delivered in a flexible way dependant on the needs of the service and to reflect the move to 7 day working. Job planning for all consultants (existing and new appointees) will reflect the ambition to improve access to imaging over the 7 day week.

A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead Respiratory Consultant, on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.

It will be reviewed at 3 months & thereafter annually or at any time, but no less than 3 months after a previous review, as requested by the appointee, Operational Medical Director or Clinical Lead & adjusted accordingly to the agreement of both parties.

Requirements

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of Clinical and Emergency Medicine.

Must have:

·       Experience and expertise in body CT

·       Experience in body MRI

·       Experience in non-obstetric ultrasound and ultrasound guided intervention

·       Ability to offer expert clinical opinion on full range of both emergency and routine cross sectional imaging studies

·       Ability to supervise radiology SpRs across a broad range of cross sectional imaging.

·       Willing to participate in radiology on-call rota

·       RCR approved specialist training in breast radiology as registrar or subsequently (see current guidelines)

·       Able to independently run symptomatic breast clinics including US biopsy, Axilla biopsy, MMO and US performance

·       Able to perform independently stereotactic biopsy

·       Able to independently report Breast MRI

Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant  will offer mentoring support and lead the existing clinical team through; joint clinics, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.
