Overview

 

We are looking to recruit to a substantive 10 session post which has been created in Old Age Psychiatry in a predominantly rural service undergoing significant modernisation for our HSE client. You will join colleagues in a very dynamic and forward looking Old Age Psychiatry Service and there will be opportunities to develop special areas of interest within the MH & LD Directorate. Office accommodation, secretarial support and full IT facilities including access to intranet and internet will be provided.

The consultant will be working with a full time specialty doctor, and a multidisciplinary Community Health team to Provide Old Age Psychiatric services in the Munster area.

The design of the post is consistent with the New Ways of working documentation.

All Adult Mental Health Services across the Health Board use the electronic FACE record

All clinical staff are accountable and responsible for their own clinical competence and should limit their actions to those for which they are deemed competent. Clinical staff are also required to comply with the requirements of their professional organisation regarding supervision.

As a senior employee of the Health Board the post holder will work in close co-operation with, and support other clinical, medical professional and managerial colleagues in providing high quality healthcare to the HSE’s patients.

Integral to the responsibilities of the post are the following requirements:-

·       To ensure the provision and delivery of a first class clinical service

·       To provide effective leadership to all staff engaged in the specialty

·       To sustain and develop teaching and research wherever appropriate

·       To undertake all work in accordance with the Health Board’s procedures and operating policies

·       To conduct clinical practice in accordance with contractual requirements and within the parameters of the HSE’s service plans

·       To maintain the confidence of business plans and development strategies formulated for the specialty or the HSE

·       Participating in CPD including Audit

·       A Teaching and Training Role for medical undergraduates and postgraduates

·       Educational Supervisor role to Junior and Middle Grade Medical Staff

·       To undertake regular multidisciplinary clinical audit and provide evidence based medicine

Specific Responsibilities of the Post

The post holder will be based at our clients HSE hospital. They will provide clinical leadership to the mental health patient unit; the liaison service for the Munster region; and hold a community caseload covering the locality. The GPs from this area made 394 of the total referrals (449) for the population of older adults in area over the last year and the post holder can expect approximately 6 new referrals per month. It is expected that he /she will work closely with the full time specialty doctor to provide the community service and to facilitate a robust care pathway ensuring safety during the transfer of care to and from the community.

The speciality doctor contributes to the liaison service and they will be supervised by the post holder (yourself)

The post holder will be responsible for the assessment and management of older adult inpatients admitted by the Mental Health Services. They will be the Responsible Clinician for those patients detained under the Mental Health Act. They will provide Consultant advice to the hospital if the team is looking after an Older Adult.

From time to time a younger adult may be more suitably nursed on the hospital in patient ward and the post holder will be the consultant in charge of their care and will liaise with the appropriate community consultant regarding discharge arrangements and follow up. A reciprocal arrangement is in place for older adults who are not suitable for the hospital ward and if warranted, transferred to the Psychiatric Intensive Care Unit

In the calendar year 2014 there 25 direct admissions and 13 transfers into the 7 beds of the hospital ward, resulting in occupancy rate of 94.6%.

Average length of stay was 99.3 days and, for all patients on the Ward during 2014, 28.6% were formally detained at some point.

The Senior Nurse and Team Leader will be responsible for ensuring that HSE policies and procedures for safe functioning of the team is followed, including referral processes, assessment and adherence to the key performance indicators for the service and the Mental Health measure. The post holder will be expected to work in close co-operation with the team management.

Clinical

·       To provide clinical leadership and full participation in a multidisciplinary team to the ward at the hospital

·       To support the Older Adult liaison service

·       To provide clinical leadership and full participation in a multidisciplinary Older Adult Community Mental Health Team.

·       To contribute senior clinical input to the modernisation, development and ongoing operational management of specialist services for older adults and those with early onset dementia.

·       To conduct clinical practice in accordance with contractual requirements within the parameters of the Division’s and Health Board’s agreed objectives.

·       To work within the framework of the Mental Health Measure. 2010

Teaching Commitments

·       regular commitments to ward based teaching of undergraduates.

·       participation in the lecture programmes for 2nd and 4th & 5th year medical students.

·       teaching of other groups as required including other specialty juniors and specialist nurses.

 

Standard Duties and responsibilities

a) To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan  and in line with policies as specified by the Employer.

b) To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.

c) To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.

d) To co-operate with the expeditious implementation of the Disciplinary Procedure 

e) To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.

f) To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.

g) To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.

h) To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.

i) To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.

j) To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 

k) To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.

l) To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery

 

Job Plan

A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead Old Age
Psychiatry Consultant, on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.

It will be reviewed at 3 months & thereafter annually or at any time, but no less than 3 months after a previous review, as requested by the appointee, Operational Medical Director or Clinical Lead & adjusted accordingly to the agreement of both parties.

Actual DCC/SPA split will be discussed and agreed with the successful applicant in the light of their

individual agreed SPA needs.


Also the successful candidate will be allocated 0.5 SPAs per week in their first 12 months to participate

Requirements

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of Clinical and Psychiatry- Old Age.

Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant  will offer mentoring support and lead the existing clinical team through; joint clinics, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.
