Overview
The Health Board employs around 11,000 staff and has a budget of around 1.1
billion euro’s. It is responsible for
the operation of three district general hospitals. There are twenty two other
acute and community hospitals, and a network of over ninety health centres,
clinics, community health team bases and mental health units. It also coordinates the work of GP practices
and the HSE services provided in the munster region by dentists, opticians and
pharmacies.
The Health Board is currently undergoing a
programme of change. Mental Health
Services are provided by a Mental Health and Learning Disabilities Division.
Adult psychiatry
services are based around multidisciplinary Community Mental Health Teams which
deliver secondary mental health services to defined catchment populations. The
teams are organised in line with the six Local Authorities. Teams are provided in partnership with Local
Authorities, with differing levels of integration across Munster.
Acute Care Services
Acute Care is delivered from the three hospitals
based in-patient facilities. Acute care is led by acute care consultants within
a collaborative model of care which maintains community consultant involvement
and offers continuity.
Home Treatment operates as part of Acute Care and
are led by the acute care consultants.
It provides the gateway for inpatient beds and in addition provides
input to individuals who are suitable for early discharge from inpatient care.
Rehabilitation Services
There are currently 3 open inpatient rehabilitation
units.
There is a locked intensive rehabilitation unit
based in the community hospital.
The rehabilitation units are managed by dedicated
rehabilitation consultants, supported by Community Rehabilitation teams.
Forensic Services
Forensic Services are based at a Medium Secure
Unit; a recently built unit with multidisciplinary forensic community teams,
and Consultant Forensic Psychiatrists.
Primary Care and Psychological Therapy
Services
Primary Care Mental Health Services provide initial
assessment and treatment for people with mild to moderate mental
disorders. Referrals onto the CMHT’s for
specialist management are made when appropriate. There is a well-established
record for providing a range of specialist secondary and tertiary psychological
treatments. The department has established procedures for referral, with
specific professionals linked to each Community Mental Health Team. In the West
there is a well developed Psychology Department with close liaison with the
Psychology department in the University hospital in the region.
The Liaison Service has recently expanded, with dedicated Liaison
Consultants across the three acute sites. The consultation style of services
provides a comprehensive assessment and suggests follow-up for patients
attending A&E department with an acute psychiatric presentation and those
admitted to the medical wards with psychiatric elements to their
presentation. An Alcohol Liaison
service provides input to the hospitals.
Older Persons Mental Health Services
Older Persons Mental Health Services are provided
in the community by community mental health teams- elderly. They are organised
in line with the Local Authorities. They provide primary and secondary care
services, as well as memory services and day hospital provision.
Patients from the nearby hospitals are admitted to
wards for elderly mentally ill inpatients locally. One ward is for primarily
cognitive disorders and the other for older people with functional mental
disorder.
Substance
Misuse Services
Six Community Drug and Alcohol Teams provided in
Partnership with each local Authority work across the region. There are active partnerships within the
local Substance Misuse Action Teams (SMATs), Police, and Probation
Services. The service also has a
Specialist Midwifery Service and mobile Harm Reduction Service that covers the
six counties of Munster, both of which have been seen as National examples of
good practice.
In-patient detoxification and rehabilitation
services are provided. This is a purpose
built unit for the treatment of substance misuse/alcoholism provided by CAIS,
an independent provider.
Learning Disability Services
Community services are provided in partnership with
the individual Local Authorities, centred on multidisciplinary teams. Specialist in-patient beds are provided, the
Intensive Support Service provides community assessment and management of
challenging behaviours and there are well-developed supported and intensively
supported living schemes.
Child and Adolescent Mental Health Services
(CAMHS)
CAMHS are based within the Children and Young
Persons division. CAMHS Tier 4 Inpatient
services are provided by a purpose built unit. Sat alongside the inpatient unit
is a multi-disciplinary Intensive Community Support Team which works extended
hours and prevents admission to out of area beds and facilitates early
discharge from the inpatient unit.
Clinical Duties of Post Holder
The post holder will provide specialist psychiatric
assessment, treatment and management for adults aged 18 upwards with serious
and enduring mental illness. The post
holder will provide medical input to patients under secondary care when
required by the care coordinator as well as provide consultation and
supervision for Primary Mental Health Services in their catchment area. Where appropriate the post holder will carry
a case load.
The service will deliver a needs led service and
therefore the post holder will be expected to manage those above 65 until such
time as their needs are more appropriately met by the Older Persons Mental
Health Services. (Transition
arrangements are under review).
The post holder will be based with the CMHT and
work with the county manager to ensure clinical leadership of the
multi-disciplinary team. The post holder
will assist with clinical supervision of the team. It is anticipated that the
post holder will assist in training of team members in clinical matters.
The post holder will provide Responsible Clinician
services to community patients.
The service operates an Acute Care Model with two
dedicated inpatient consultant managing the new to service admitted
patients. The inpatient consultants also
have responsibility for the Home treatment team patients. The post holder will work collaboratively
with the Acute Care Services. There is a
dedicated session within the job plan for consultation with the Acute Care
Service. In the event of one of the
acute care consultants being on leave overall responsibility for the inpatients
is held by the other acute consultant.
The post holder is expected to support acute care with their acute care
session.
The post holder will
be expected to hold a current valid driving licence as the job involves
community assessments and travelling to distant sites.

Job Plan
A formal job plan will be agreed between the appointee, the Operational Medical
Director and Lead Consultant Psychiatry- Adult, on behalf of the
Trust’s Medical Director, based on the provisional timetable. This will be
signed by the Chief Executive and will be effective from the commencement date
of the appointment.
It will be reviewed at 3 months & thereafter annually
or at any time, but no less than 3 months after a previous review, as requested
by the appointee, Operational Medical Director or Clinical Lead & adjusted
accordingly to the agreement of both parties.

On Call and Cover Arrangements
There is a 1:9 on call rota for consultants (Tier
3), which attracts Intensity Band 1
On call, leave and cover arrangements will be made
with the community consultant colleagues.
The post holder will be expected to live within an
area commensurate with on call duties.
Standard Duties and responsibilities
a) To participate in development
of and undertake all duties and functions pertinent to the Consultant’s area of
competence, as set out within the Clinical Directorate Service Plan and in line with policies as specified by the
Employer.
b) To ensure that duties and
functions are undertaken in a manner that minimises delays for patients and
possible disruption of services.
c) To work within the framework
of the hospital / agency’s service plan and/or levels of service (volume, types
etc.) as determined by the Employer. Service planning for individual clinical
services will be progressed through the Clinical Directorate structure or other
arrangements as apply.
d) To co-operate with the
expeditious implementation of the Disciplinary Procedure
e) To formally review the
execution of the Clinical Directorate Service Plan with the Clinical Director /
Employer periodically. The Clinical Directorate Service Plan shall be reviewed
periodically at the request of the Consultant or Clinical Director / Employer.
The Consultant may initially seek internal review of the determinations of the
Clinical Director regarding the Service Plan.
f) To participate in the
development and operation of the Clinical Directorate structure and in such
management or representative structures as are in place or being developed. The
Consultant shall receive training and support to enable him/her to participate
fully in such structures.
g) To provide, as appropriate,
consultation in the Consultant’s area of designated expertise in respect of
patients of other Consultants at their request.
h) To ensure in consultation with
the Clinical Director that appropriate medical cover is available at all times
having due regard to the implementation of the European Working Time Directive
as it relates to doctors in training.
i) To supervise and be
responsible for diagnosis, treatment and care provided by non-Consultant
Hospital Doctors (NCHDs) treating patients under the Consultant’s care.
j) To participate as a right and
obligation in selection processes for non-Consultant Hospital Doctors and other
staff as appropriate. The Employer will provide training as required. The
Employer shall ensure that a Consultant representative of the relevant
specialty / sub-specialty is involved in the selection process.
k) To participate in clinical
audit and proactive risk management and facilitate production of all
data/information required for same in accordance with regulatory, statutory and
corporate policies and procedures.
l) To participate in and facilitate production of all data/information
required to validate delivery of duties and functions and inform planning and
management of service delivery
Requirements
This post is open to doctors who hold Registration
as a Specialist in the Specialist Division of the Registrar of Medical
Practitioners maintained by the Medical Council in Ireland in the specialty of
Clinical and Psychiatry.

Management Responsibility
All Consultants are required to attend the monthly
Directorate Meetings. Post holders will be expected to share in administrative
duties allocated by mutual agreement within the Directorate. As part of the
Consultant Team, the consultant will
offer mentoring support and lead the existing clinical team through; joint
clinics, one-to-one support, individual case management and complex cases,
identifying development needs and suggesting CPD (Continuous Professional
Development) opportunities and responding to clinical enquiries via e-mail and
telephone.
