Overview


Following the integration of our existing Community Mental Health Teams (CMHTs), Crisis Service and Assertive Outreach Teams the newly formed locality based Adult Mental Health Teams (AMHTs) undertake both an assessment and treatment function. One of the aims of this is to reduce the need for patients to be moved from one team to another especially when they are experiencing a crisis and are consequently very unwell. It also enables patients to be familiar with a wider group of staff from the HSE trust, so therapeutic relationships with individual patients and carers are maintained during a period of crisis for the patient, an inpatient admission or due to planned or unplanned leave of their nominated care coordinator.

 

The AMHTs provide an extended hours service and the assessment function of the team operates a 7 day/week service from 8 am to 9pm. Overnight cover is provided by up to 5 staff, based in each county who work closely with staff on the inpatient wards, out of hours GPs and colleagues from the emergency services to support patients who are experiencing a crisis. The treatment function of the team works from 9am – 5pm 7 days a week, with a minimum requirement, to run two late night clinics until 8pm each week, with the capacity to increase this if local demand requires it.

Clinical Duties (guideline only)

1.       The Consultant will provide senior medical input and clinical leadership to the trust. Medical responsibility for patients usually remains with the GP, except where a medical member of the trust takes a patient on for continuing treatment. The post holder will have responsibility for those patients seen by himself/herself, together with those patients seen by medical staff directly supervised by the post holder.

2.       The post holder will work alongside the team manager and the other Consultant Psychiatrist in the assessment team to ensure the effective functioning and development of the service.

3.       The consultant will provide direct clinical supervision and line management to the trainees and speciality doctors and any senior trainee placed with the team. This will include 1 hour per week clinical supervision for trainees and a pro rata equivalent level of supervision to speciality doctors- in conjunction with other consultants.

4.       The consultant will be expected to provide direct clinical care and ensure medical involvement in the CPA process.

5.       Providing timely written correspondence to relevant professionals  documenting  assessments, on-going management, progress, and eventual discharge using Trust IT and clinical records system – training will be provided where needed.

6.       Work collaboratively with other agencies. In particular liaison with staff from other teams to manage smooth and timely transfer into and out of the community team. 

7.       Active commitment to delivering the Trust’s clinical governance agenda.

8.       Contribution to developing and delivering improved clinical services as outlined in the National Service for Mental Health.

9.       The consultant will be expected to contribute to the collection of data as required by the Trust and other relevant agencies. This includes the timely recording of clinical activity data, and participation in clinical audit with appropriate administrative support.

10.    Governance – all services are expected to maintain effective governance including measuring and reporting performance. This is based on national standards and local contracts. Consultants provide a lead in the team in reviewing performance and governance. Key quality and outcome measures are embedded in the care packages aligned to each care cluster.

11.    The consultant will be expected to attend most regular directorate managerial meeting as necessary, particularly Joint Consultant Committee and Directorate/Managerial liaison meeting.

12.    The consultant will be expected to participate in and attend the local training programme for junior doctors.

13.    The consultant would be expected to participate in both a CPD peer group and a clinical supervision group with fellow consultants.

14.    Consultant would be expected to maintain their own programme of training and CPD accreditation with the support of their AMD/Lead consultant/Clinical Director.

15.    The consultants will normally cross-cover for each other for annual leave, study leave and brief periods of sickness absence; in addition there is a day time cover rota.  The consultants take part in the Dublin Adults of Working Age out-of-hours rota. This is a 1 in 14 rota, with 1%, band B intensity. There is a core trainee on shift out of hours.


Teaching Commitments

·       regular commitments to ward based teaching of undergraduates.

·       participation in the lecture programmes for 2nd and 4th & 5th year medical students.

·       teaching of other groups as required including other specialty juniors and specialist nurses.

 

Job Plan

A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead Consultant
Psychiatry- Adult, on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.

It will be reviewed at 3 months & thereafter annually or at any time, but no less than 3 months after a previous review, as requested by the appointee, Operational Medical Director or Clinical Lead & adjusted accordingly to the agreement of both parties.

Standard Duties and responsibilities

a) To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan and in line with policies as specified by the Employer.

b) To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.

c) To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.

d) To co-operate with the expeditious implementation of the Disciplinary Procedure 

e) To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.

f) To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.

g) To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.

h) To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.

i) To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.

j) To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 

k) To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.

l) To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery


Requirements

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of Clinical and Psychiatry.

Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant  will offer mentoring support and lead the existing clinical team through; joint clinics, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.
