Overview

 

The post is for 10 PA’s based at our clients University Hospital based in Munster. The successful candidate will join a team of seven consultants. The hospital is looking to recruit a Histopathologist to join a group of enthusiastic colleagues. Opportunities exist to develop a wide area of specialist interests.

Histopathology Services

The Department is established for 4 Consultant Histopathologists, one acting as Head of Department on a rotational basis. As an interim measure the current Head of Department is the Clinical Lead for Pathology. Occasionally, a trainee pathologist from the HSE Rotational Training Scheme is seconded to the department.

The Department provides a comprehensive, timely and reliable service in histology, cytology and autopsies to all users of the services within and outside Munster. There are close links with the other pathology disciplines.

The surgical specimens cover all the usual disciplines with the exception of neuropathology, paediatric pathology, ophthalmic pathology and major lung resections. Since 1 August 2001, all perinatal & obstetric post mortem examinations are carried out by pathologists at the Women’s Hospital.

There are about 70 diagnostic renal biopsies per annum, currently reported by the Hospital.

Immunofluorescence examination is also available for skin biopsy diagnosis.

Job Summary

·       Consultant level duties in the Histopathology Department participating in all work received in the department.

·       To contribute where appropriate to the provision of Coroner’s post mortems in association with 2 other colleagues.

·       Working collaboratively with colleagues to further develop and improve the service

·       Administrative duties as required within the Directorate

·       Provision of care for colleagues’ leave

·       Participation in clinical audit and CME/CPD appropriate to experience at Consultant level, together with contribution to education and training of medical students and junior staff

·       Professional management and supervision of technical staff as required.

·       Attendance at Directorate, Multidisciplinary meetings, District, Network and Regional meetings as required by the interests of the department

Specific Duties

To share responsibility with the other pathologists for the diagnostic and clinical services in Munster. This includes:

·       Examination and reporting of all types of histopathology specimen. Some degree of site specialisation has been achieved, all of the incumbent pathologists having areas of special interest. The appointee will be expected to participate in this subspecialisation and this will be determined according to the needs of the department. The Department would hope to accommodate any special interests which the appointee may already have. Nevertheless, there is a general pool of uncomplicated specimens which are handled, at least initially, by all of the pathologists. There is no formal requirement for a Saturday morning work commitment.

·       Examination and reporting of a range of Non-gynaecological cytology specimens is shared with other consultants.

·       To undertake, where appropriate, hospital post mortem examinations and to contribute to Coroner’s autopsies. These are performed by invitation of Coroner and are paid additionally to contract. On the assumption that approval is obtained from Coroner, the appointee will have an equal share of the Coroner’s work with the other senior pathologists. The appointee will also be expected to train junior staff in this work.

·       To participate in regional and national EQA schemes relevant to specialty reporting.

·       To share responsibility for professional standards in the laboratory (safety, quality assurance and ensuring compliance with standards required to satisfy Clinical Pathology Accreditation).

·       To assist the Head of the Department in the management of the Department; deputising or taking full responsibility according to circumstances and at the request of the Departmental Head.

·       To have in hand some audit / research activity, originating either from within the Department or in association with clinicians.

·       To provide mutual cover for the purposes of annual leave and study leave, within the limits of site specialisation and the need where appropriate to provide a provisional diagnosis only.

·       To participate in medical audit and continuing medical education. A commitment to CPD is expected in accordance with Royal College of Pathologists recommendations. The Trust is also committed to support Consultant Pathologists in CPD activities. Funding for CME is held centrally by the Medical Director. An annual appraisal of all pathologists is carried out by the Medical Service Head for Pathology

·       To participate in the education and training of medical students and junior medical staff which may include one or two trainee pathologists.

·       Professional management and supervision of laboratory staff.

·       Attendance at Directorate, District and Regional meetings as required to represent the interests of the Department.

·       Participation in other administrative duties and committee work as required

Productivity

Clinical time is allocated in sessions (blocks of time) and consultants are expected to ensure optimum utilisation of the sessions allocated to them, in terms of time and productivity. In particular:

·       The role will be expected to manage a Consultant-level workload with an appropriate turnaround time.

·       To make use of additional SPAs to undertake audits and other value added activities.

·       The use of the RCPath points system is in place and an annual workload is established based on the number of DCCs in each individual contract. The department manages workload in accordance with the RCPath points system and as a result the number of points allocated is based on the number of direct clinical care sessions within each individual’s job plan. The department also sets turnaround time targets to meet the needs of service users, particularly in relation to cancer pathways and it is expected that these will be met. These are subject to change, as required by the needs of the service.

Teaching Commitments

·       regular commitments to ward based teaching of undergraduates.

·       participation in the lecture programmes for 2nd and 4th & 5th year medical students.

·       teaching of other groups as required including other specialty juniors and specialist nurses.

 

Job Plan

A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead
Histopathology Consultant, on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.

It will be reviewed at 3 months & thereafter annually or at any time, but no less than 3 months after a previous review, as requested by the appointee, Operational Medical Director or Clinical Lead & adjusted accordingly to the agreement of both parties.

Standard Duties and responsibilities

a) To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan  and in line with policies as specified by the Employer.

b) To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.

c) To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.

d) To co-operate with the expeditious implementation of the Disciplinary Procedure 

e) To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.

f) To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.

g) To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.

h) To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.

i) To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.

j) To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 

k) To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.

l) To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery

Requirements

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of Clinical and Histopathology.

Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant  will offer mentoring support and lead the existing clinical team through; joint clinics, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.
