Overview

 

This is a Consultant post within the department of Histopathology a 10 Programmed Activity (PA) post. The post holder will lead on the following specialities Urology, Head and Neck and Lower Gastro-intestinal pathology.

Any applicant who is unable for personal reasons, to work full-time will be eligible to be considered for the post. If such a person is appointed, modification of the job content will be discussed on a personal basis with the Trust in consultation with consultant colleagues. The HSE Trust provides local general hospital and community services to around 260,000 people in Connacht Region and the surrounding areas. The Trust is a provider of acute services - providing inpatient and outpatient services at hospital as well as a wide range of services in the community. The hospital houses the full range of district general hospital services under one roof. The development of their site was completed in 2010 and the continued up-grading of existing areas ensures the Trust has state of the art operating theatres, treatment areas and equipment.

The hospital provide high quality, friendly and effective community health services from some 10 sites including health centres and GP surgeries. Covering Connaught and beyond, they offer a multidisciplinary services include rapid response in the community and homebased care, so that those with long term conditions and the frail elderly, can remain in their own homes to be cared for.

The post includes direct clinical care services, strategic advice and general education. The Histopathology department supports clinical and healthcare services across the whole local healthcare economy as part of an integrated system.

The Trust has a robust programme for mentoring new Consultant appointments and supports study leave (leave and funding) for CPD requirements The Medical Advisory Committee (MAC) meets monthly and is chaired by the Medical Director. The Senior Medical Staff Committee (SMSC) is chaired by an elected representative. All Trust consultants may attend the meeting, but it is not compulsory to do so.

Accommodation and Facilities

This CPA accredited laboratory department is located at the rear of the hospital site adjacent to the mortuary facilities. The department is open from 7.30am to 5.00pm. The department has individual consultant offices, shared admin and clerical office. The laboratory has appropriate specimen reception, processing /trimming room, main laboratory and immuno-chistochemistry room. There is a separate non gynaecology and andrology preparation room. The department has a library which houses a 10 station multi-header, which can also be used for local teaching/mdt meeting. The public mortuary can accommodate 135 deceased, which includes facilities for bariatric bodies. There are appropriate changing facilities, office and two viewing rooms. The department also hosts HM Coroner and has an inquest room with supporting facility for the Coroner. General Services provided The laboratory provides a wide range of Histopathology, Cytopathology and Autopsy services to the HSE Trust, the District.

Histopathology

A broad range of surgical material is examined. As well as general surgical, medical and gynaecological samples, there are significant numbers of specialist samples including haematological, dermatological, gastroenterological, urological, breast screening, plastic head and neck cases.

All work is reported by specialist Histopathology with a named deputy. There are well developed multi-disciplinary site specific teams for both neoplastic and non-neoplastic disorders. Oncologists form part of the core team for each tumour site. The histopathologists are part of those teams which generally meet weekly to review local cases from a diagnostic and therapeutic standpoint. These multi-disciplinary team meetings are held either in the MDT room adjacent to route 020. The successful applicants should be prepared to develop site specific interests. These will be discussed with the current Consultants and we would aim to fulfil the needs of the department together with the interests of individual applicants.

Key Duties of the Post
The appointee will be expected to:

·       Work with other consultants to provide a Histopathology service with a possibility for developing responsibility for gastro-intestinal, urological or head and neck pathology.

·       Provide supervision and training of junior medical staff

·       Provide prospective cover for colleagues for purposes of annual, study and professional leave. This is currently arranged by exchange of on-call duties

·       Participate in the established teaching programme for junior doctors and medical students. Act as Educational Supervisors for junior doctors.

·       Ensure that staff in their work area receive appropriate Health & Safety and Risk Management training commensurate with the range and level of work to be undertaken

·       Take personal responsibility for maintaining a high quality of service and ensure evidence based practice by participating in appropriate EQA and relevant Medical Royal College CPD scheme

·       Participation in on-going multi-disciplinary clinical audit and research activity as deemed appropriate

Teaching Commitments

·       regular commitments to ward based teaching of undergraduates.

·       participation in the lecture programmes for 2nd and 4th & 5th year medical students.

·       teaching of other groups as required including other specialty juniors and specialist nurses.

 

Job Plan

A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead
Histopathology Consultant, on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.

It will be reviewed at 3 months & thereafter annually or at any time, but no less than 3 months after a previous review, as requested by the appointee, Operational Medical Director or Clinical Lead & adjusted accordingly to the agreement of both parties.

 

A job plan and work schedule will set out agreed arrangements for how work is organised, up on appointment

Standard Duties and responsibilities

a) To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan  and in line with policies as specified by the Employer.

b) To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.

c) To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.

d) To co-operate with the expeditious implementation of the Disciplinary Procedure 

e) To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.

f) To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.

g) To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.

h) To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.

i) To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.

j) To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 

k) To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.

l) To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery

Requirements

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of Clinical and Histopathology.

Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant  will offer mentoring support and lead the existing clinical team through; joint clinics, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.
