Overview
Our client HSE hospital has

emergency and our maternity service.
Core services at the HSE Hospital include:
• Full emergency department
service for major accidents and trauma. The department is supported by a
separate on site urgent care centre, which is run by GP and community partners.
• Emergency assessment and
treatment services including critical care. The Trust is a designated trauma
unit and stroke unit.
• Emergency and planned
surgery and medical treatments; such as day and inpatient surgery and
endoscopy, outpatients, services for older people, acute stroke care; and
cancer services.
• Comprehensive maternity
services including consultant led care, midwifery led natural birth centre,
community midwifery support, antenatal care, postnatal care and home births.
The service is supported by a special care baby unit.
• Children’s services
including emergency assessment, inpatient and outpatient services
• Sexual health services
• Diagnostic services
including accredited pathology and imaging service

services including physiotherapy and occupational therapy

Clinical
Duties
(guideline only)
The post
holder’s immediate line manager will be the Divisional Director for Medicine,
reporting ultimately to the Medical Director.
It is
envisaged that the new Trust clinical management structures will incorporate a
Service Director whose responsibilities will include palliative medicine/end of
life care reporting to a Clinical Director and Divisional Medical Director
within the enlarged trust-wide Medical division.
Outline of
Responsibilities
The
consultant will lead on and develop end of life and specialist palliative care
within the hospital with the support of the Dublin Community SPC Consultant and
the existing 2 sessions of block contract time for the hospital per week. The consultant will be expected to negotiate
the delivery of clinical pathways, working alongside medical and nursing
colleagues. It is expected that when
required (eg annual leave), the hospital post will provide appropriate clinical
cover and support to the Hounslow community post to ensure seamless delivery of
care between community and hospital settings.
The
Consultant will be expected to engage in all aspects of Good Clinical Practice
including Teaching, Audit and Clinical Governance. The duties are summarised as follows:
Clinical
Duties
1. To
provide the medical lead for specialist palliative and end of life care at the
hospital, liaising closely with medical and surgical teams, particularly those
from acute medicine, oncology, Accident and Emergency and care of the elderly.
2. To
support colleagues working in primary care and specialist colleagues providing
palliative and end of life care services within the Leinster Region.
3. To
provide an in-hospital rapid access specialist consultation service to patients
referred from community sites (home, nursing home etc).
4. To
provide access to specialist palliative care advice for colleagues working in
outpatients, Accident and Emergency, ward based environments and community
settings, including nursing and care homes.
5. To
provide a domiciliary visiting service for those patients for whom transfer
between services is envisaged, or has occurred (eg nursing home to hospital,
hospital to home etc)
6. To ensure
active clinical communication between specialist palliative care services
within the locality and with regional centres.
7. To
participate in the regular Palliative Care MDT meetings and other site specific
MDT meetings as appropriate at the hospital
8. To
facilitate equitable access of all patients facing death within the County to
specialist palliative
9. To ensure
that ultimately the hospital has a 7 day a week face to face service for end of
life and specialist palliative care.
This will involve committing to providing at least 3 hours per day at
weekends and on Bank Holidays of specialist time, the duties to be covered
generically by the team as a whole, with each weekend session attracting a half
day in lieu during the working week.
Outside the hours covered by the face to face service, out of hours
provision will continue to be provided by the 24/7 telephone advice service
through the Hospice and its on call consultant rota.
Non-clinical
Duties
a) To
participate in development of and undertake all duties and functions pertinent
to the Consultant’s area of competence, as set out within the Clinical
Directorate Service Plan and in line
with policies as specified by the Employer.
b) To ensure
that duties and functions are undertaken in a manner that minimises delays for
patients and possible disruption of services.
c) To work
within the framework of the hospital / agency’s service plan and/or levels of
service (volume, types etc.) as determined by the Employer. Service planning
for individual clinical services will be progressed through the Clinical
Directorate structure or other arrangements as apply.
d) To
co-operate with the expeditious implementation of the Disciplinary
Procedure
e) To
formally review the execution of the Clinical Directorate Service Plan with the
Clinical Director / Employer periodically. The Clinical Directorate Service
Plan shall be reviewed periodically at the request of the Consultant or
Clinical Director / Employer. The Consultant may initially seek internal review
of the determinations of the Clinical Director regarding the Service Plan.
f) To
participate in the development and operation of the Clinical Directorate
structure and in such management or representative structures as are in place
or being developed. The Consultant shall receive training and support to enable
him/her to participate fully in such structures.
g) To
provide, as appropriate, consultation in the Consultant’s area of designated
expertise in respect of patients of other Consultants at their request.
h) To ensure
in consultation with the Clinical Director that appropriate medical cover is
available at all times having due regard to the implementation of the European
Working Time Directive as it relates to doctors in training.
i) To
supervise and be responsible for diagnosis, treatment and care provided by
non-Consultant Hospital Doctors (NCHDs) treating patients under the
Consultant’s care.
j) To
participate as a right and obligation in selection processes for non-Consultant
Hospital Doctors and other staff as appropriate. The Employer will provide
training as required. The Employer shall ensure that a Consultant
representative of the relevant specialty / sub-specialty is involved in the
selection process.
k) To
participate in clinical audit and proactive risk management and facilitate
production of all data/information required for same in accordance with
regulatory, statutory and corporate policies and procedures.
l) To
participate in and facilitate production of all data/information required to
validate delivery of duties and functions and inform planning and management of
service delivery.
Teaching Commitments
·
regular commitments to ward based teaching of
undergraduates.
·
participation in the lecture programmes for 2nd
and 4th & 5th year medical students.
·
teaching of other groups as required including other
specialty juniors and specialist nurses, MRCP & MRCGP candidates.
Job Plan
A formal job plan will be agreed between the appointee, the Operational Medical
Director and Lead Palliative Care Medicine Consultant, on behalf of the Trust’s Medical
Director, based on the provisional timetable. This will be signed by the Chief
Executive and will be effective from the commencement date of the appointment.

Balance: DCC = 8 DCC (7 at the Hospital and availability for 1 in community), 2 SPA
It will be reviewed at 3 months & thereafter
annually or at any time, but no less than 3 months after a previous review, as
requested by the appointee, Operational Medical Director or Clinical Lead &
adjusted accordingly to the agreement of both parties.
Requirements
This post is open to doctors who hold Registration
as a Specialist in the Specialist Division of the Registrar of Medical
Practitioners maintained by the Medical Council in Ireland in the specialty of
Clinical and Palliative
Care Medicine.
Office Accommodation
Office accommodation will be provided for the
appointee.
Management Responsibility
All Consultants are required to attend the monthly
Directorate Meetings. Post holders will be expected to share in administrative
duties allocated by mutual agreement within the Directorate. As part of the
Consultant Team, the consultant will
offer mentoring support and lead the existing clinical team through; joint
clinics, one-to-one support, individual case management and complex cases,
identifying development needs and suggesting CPD (Continuous Professional
Development) opportunities and responding to clinical enquiries via e-mail and
telephone.
