Overview
This is a Consultant Paediatric post
with commitments to both the acute and neonatal service. The commitment to the
neonatal service is a significant part of the job plan. It is hoped the
successful candidate will help develop the Neonatal Service and Paediatric
Service in the Connaught region.
The acute commitment to General
Paediatrics involves regular service weeks covering the General Paediatric
wards (08:30 – 17:00hrs) and Neonatal Unit (12:30 – 17:00hrs) in rotation with
all the other Paediatric Consultants. Out of hours on-call involves resident
(16:30 – 21:00hrs week days and 08:30 – 21:00hrs weekends) in rotation and
non-resident cover for General Paediatrics and Neonates in rotation. When not
on a service week the commitment (09:00 – 12:30hrs) two mornings a week is for
Neonates with a Neonatal Ward Round.
There are two clinics (one Neonatal
and one a combination of Neonatal and General Paediatrics). The post has two
identified SPA sessions, three un-contracted sessions and an administration
session.
There are two sites within the remit:
First Hospital:
570 beds and provides a comprehensive range of In-patient, Day case and
Outpatient facilities together with Accident and Emergency and Diagnostic
facilities. The Acute Mental Health Unit is also based on site. There are a
range of Critical Care Services including an Adult Intensive Care Unit, Adult
High Dependency Unit and Level 2 Neonatal Unit. There are also nine Operating
Theatres. State of the art facilities are provided within the Radiology Department
and the hospital is film less with digital images and reports available to
clinicians across the site. The Pathology Service is also modern using
specialist equipment and is well staffed. Modern innovations have been
introduced to enable authorised results to be viewed on computer screens in all
wards and clinics.
Second Hospital:
430 beds, and provides acute emergency and elective medical and surgical
services for adults, Adult Intensive Care and Coronary Care; Consultant-led
Obstetrics services with Special Care Baby Unit, Inpatient Consultant-led
Paediatrics and has a busy new Accident and Emergency unit. There are seven
operating theatres. The hospital also provides Sub-regional Oral and Maxillo
Facial Services, a full range of locally provided and visiting specialist
outpatient services and has an extensive range of diagnostic services and
facilities including 64 slice CT, Nuclear Medicine and MRI. State of the art
facilities are provided within the Radiology Department and the hospital is
film-less with digital images and reports available to clinicians across the
site. Many of the hospitals Corporate and Management offices are located on
this site, along with clinical staff residences.
Specific Duties of the Post
This is a full time post offering 8
sessions DCC and 2 SPA sessions. Part time arrangements will be considered. All
the Consultant Paediatricians including the Community Paediatricians within the
hospital contribute to the non-resident on-call rota (1:7rota) and service week
(1:7 rota). While non resident out of hours Consultants are responsible for the
General Paediatric patients and Neonates in the hospital.
The Consultant has a tier two doctor
and 2 tier one staff (1 or 2 doctors with a Paediatric Nurse Practitioner)
providing resident cover. The post holder will also provide onsite cover one
weekday evening (5pm -9pm) per week and on-site cover for “long days” Saturday
and Sunday (9am -9pm) on a 1:7 rota.
While working on-site resident on-call
the Consultant will be supervising either 2 tier one doctors or 1 tier one
doctor and a Paediatric Nurse Practitioner. They will be supported by a
non-resident on-call Consultant. While resident they will be responsible for
the care of General Paediatric patients and Neonates of 28 weeks gestation and
above.
Prolonged ventilation is not performed
at the second hospital and babies requiring more than 48 hours ventilation or
babies with significant problems are transferred out by the Regional Neonatal
Transport Team. The post holder will participate in the consultant of the week
system-this is currently on a 1:7 rota. During this time the Consultant is
responsible for the General Paediatric ward 5 days a week and performs a daily
ward round.
During the afternoon the Consultant is
also responsible for the Neonatal Unit. The Lead Neonatologist for the hospital
performs regular ward rounds on the Neonatal Unit and the post holder is
expected to share this responsibility. At present the Consultant who is
responsible for the General Paediatric ward will take responsibility for any
child that presents with a safe guarding problem. There are separate Child Safe
Guarding Leads for the hospital but all Paediatric Consultants contribute to
the assessment of child protection cases.
Teaching Commitments

Job Plan
A formal job plan will be agreed between the appointee, the Operational Medical
Director and Lead Paediatrician-
Neonates Consultant, on behalf of the Trust’s Medical Director, based
on the provisional timetable. This will be signed by the Chief Executive and
will be effective from the commencement date of the appointment.
It will be reviewed at 3
months & thereafter annually or at any time, but no less than 3 months
after a previous review, as requested by the appointee, Operational Medical
Director or Clinical Lead & adjusted accordingly to the agreement of both parties.

Standard Duties and responsibilities
a)
To participate in development of and undertake all duties and functions
pertinent to the Consultant’s area of competence, as set out within the
Clinical Directorate Service Plan and in
line with policies as specified by the Employer.
b)
To ensure that duties and functions are undertaken in a manner that minimises
delays for patients and possible disruption of services.
c)
To work within the framework of the hospital / agency’s service plan and/or
levels of service (volume, types etc.) as determined by the Employer. Service
planning for individual clinical services will be progressed through the
Clinical Directorate structure or other arrangements as apply.
d)
To co-operate with the expeditious implementation of the Disciplinary
Procedure
e)
To formally review the execution of the Clinical Directorate Service Plan with
the Clinical Director / Employer periodically. The Clinical Directorate Service
Plan shall be reviewed periodically at the request of the Consultant or
Clinical Director / Employer. The Consultant may initially seek internal review
of the determinations of the Clinical Director regarding the Service Plan.
f)
To participate in the development and operation of the Clinical Directorate
structure and in such management or representative structures as are in place
or being developed. The Consultant shall receive training and support to enable
him/her to participate fully in such structures.
g)
To provide, as appropriate, consultation in the Consultant’s area of designated
expertise in respect of patients of other Consultants at their request.
h)
To ensure in consultation with the Clinical Director that appropriate medical
cover is available at all times having due regard to the implementation of the
European Working Time Directive as it relates to doctors in training.
i)
To supervise and be responsible for diagnosis, treatment and care provided by
non-Consultant Hospital Doctors (NCHDs) treating patients under the
Consultant’s care.
j)
To participate as a right and obligation in selection processes for
non-Consultant Hospital Doctors and other staff as appropriate. The Employer
will provide training as required. The Employer shall ensure that a Consultant
representative of the relevant specialty / sub-specialty is involved in the
selection process.
k)
To participate in clinical audit and proactive risk management and facilitate
production of all data/information required for same in accordance with
regulatory, statutory and corporate policies and procedures.
l) To participate in and facilitate
production of all data/information required to validate delivery of duties and
functions and inform planning and management of service delivery
Requirements
This post is open to
doctors who hold Registration as a Specialist in the Specialist Division of the
Registrar of Medical Practitioners maintained by the Medical Council in Ireland
in the specialty of Clinical and Paediatrics- Neonates.

Management Responsibility
All Consultants are
required to attend the monthly Directorate Meetings. Post holders will be
expected to share in administrative duties allocated by mutual agreement within
the Directorate. As part of the Consultant Team, the consultant will offer mentoring support and lead the
existing clinical team through; joint clinics, one-to-one support, individual
case management and complex cases, identifying development needs and suggesting
CPD (Continuous Professional Development) opportunities and responding to
clinical enquiries via e-mail and telephone.
