Overview


This is a new post and an exciting and innovative opportunity to join an enthusiastic, well-motivated team in the High Dependency Unit / Intensive Care Unit at our clients HSE hospital.

 

Applicants must have extensive experience in high dependency care / intensive care including the care of general and subspecialty, (including cardiology and surgical), paediatric patients.  Additional experience in a relevant subspecialty particularly neurology, gastroenterology, retrieval, respiratory or long term ventilation, would be advantageous.

 

Applicants will have undertaken the Paediatric High Dependency ‘Special Interest’, (‘SPIN’) / Intensive Care, module or have equivalent training. This would include a minimum of 6 months of Paediatric Intensive Care.

 

Successful applicants will participate in a specialist HDU Consultant team within a Tertiary Paediatric Hospital.

 

It is envisaged that hybrid posts, blending HDU and a complementary subspecialty will be developed, depending on the needs of the service and interests of the candidate.

 

The post holder will take an active part in the management of the service. S/he will be expected to supervise and assist in the training of the Advanced Nurse Practitioners, (ANPs) in HDU, Junior Medical / SPIN Trainees and Nursing Staff.

 

The High Dependency Unit/ Intensive Care Unit

 

The HDU, (19 beds), is in the middle of the Hospital, part of the Critical Care Unit, adjacent to both the Paediatric Intensive Care Unit, (24 beds) and the Burns Unit, (5 beds). It is separately staffed to its current 15 bed capacity and has approximately 650 admissions per year. Patients are admitted as a step-down from PICU, a step-up from the general wards, from theatre following surgical procedures and from the Emergency Department. Patients who require HDU level care, are also admitted from hospitals throughout the Region. In tandem with the PICU, the HDU supports the Trust’s specialist clinical tertiary and quaternary care services.

 

Operational Model

 

The hospital recognises the existing model of care needs to change to improve consistency of care beyond current arrangements and fulfil the requirements of 7 Day Services.

 

Patients in the HDU are jointly cared for by the Consultants in High Dependency Care and either their admitting paediatric medical or surgical teams or, in the case of PICU step-downs, the most appropriate clinical team at the time of transfer . Currently Monday to Friday, 08:00 to 17:00 hours, there is a team of PICU Consultants rostered on a weekly basis for the HDU, who support the work of the parent teams with a supplementary ward round with the ANPs. They also help with clinical care through facilitation of investigations, procedures and multidisciplinary meetings. When requested, they also respond to deteriorating patients within the hospital who may require high dependency care.

 

Out of hours, including at weekends, clinical care is currently undertaken by the on-call teams and supported by the appropriate General or Subspecialty, (including surgery), Consultant. In addition, OOH the PICU Consultant / middle grade staff are currently called upon to provide assistance when patients acutely deteriorate within the hospital. In the future it is anticipated that expansion of the HDU Consultant team will eventually enable full HDU Consultant cover 24/7.

 

The Trust is developing an ‘Acute Care [Nursing] Team’ to support deteriorating patients within the hospital.

 

This HDU / ICU Consultant post is part of the developing programme of appointments to support the model evolving to a 7 day service. The HDU consultants will take overall responsibility for advice and care of patients on paediatric HDU. They will be involved in delivering direct clinical care including leading ward rounds and escalation/de-escalation in treatment. Close liaison with other critical care consultants and parent specialty consultants will be required. In addition, the HDU Consultants will be members of the ‘out of hours’ ‘Consultant On-Call Team’ which will enhance the senior support to deteriorating children on the wards and the HDU.

 

The successful applicant will participate in an Acute Paediatric Consultant rota with ‘on call’ commitments on a 1 in 10 frequency overall on weekdays, (Monday to Thursday).  When on call, the Consultant will be resident in the hospital until at least 7PM, or later, depending on the acuity of patients in the Trust. The Trust is currently piloting extending acute on call residence to 9pm. On call consultants will then be available and expected to return to the hospital for a sick patient within 30 minutes . This acute on call may be piloted as a HDU on call alongside the existing acute General Paediatric on call rota.

 

There are very close links between the HDU and PICU. They have separate nursing staff complements and physiotherapy services are shared across the Critical Care Unit. A ‘Matron for Critical Care’ has recently been appointed. HDU ANP’s work an extended 10 hour day currently in the ‘working week’ but when the HDU Consultants are appointed, this will include weekends. Middle grade medical staff work with their Consultant’s / Consultant Teams’ patients in hours. The second on call medical middle grade staff cover the HDU’s medical patients OOH and support surgical middle grade staff when requested. OOH, cardiac patients receive support from the resident Cardiology Registrar.

 

A long term hospital Integrated Workforce Plan will support the development of a dedicated, integrated HDU junior medical and ANP team.

 

Standard Duties and responsibilities

a) To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan  and in line with policies as specified by the Employer.

b) To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.

c) To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.

d) To co-operate with the expeditious implementation of the Disciplinary Procedure 

e) To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.

f) To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.

g) To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.

h) To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.

i) To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.

j) To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 

k) To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.

l) To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery

Teaching Commitments

·       regular commitments to ward based teaching of undergraduates.

·       participation in the lecture programmes for 2nd and 4th & 5th year medical students.

·       teaching of other groups as required including other specialty juniors and specialist nurses.

 

Job Plan

A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead
Paediatrician- ICU Consultant, on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.

It will be reviewed at 3 months & thereafter annually or at any time, but no less than 3 months after a previous review, as requested by the appointee, Operational Medical Director or Clinical Lead & adjusted accordingly to the agreement of both parties.

 

A rota providing clinical and operational cover to the High Dependency Unit, (HDU), from 8:00 AM to 5:00 PM every day, including weekends, on a 1 in 5 week basis.

 

The successful applicant will participate in a 1 in 10 out of hours on call rota during ’non HDU’ weeks with commitments to be determined (see Operational Model above).

The job plan is subject to annual review with the Clinical Lead / Divisional Director, with any changes being agreed within the department.


Requirements

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of Clinical and Paediatrics- ICU.

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant  will offer mentoring support and lead the existing clinical team through; joint clinics, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.
