Overview

This is an exciting opportunity for a new full time Consultant Medical Oncologist based at our client’s HSE Hospital - Leinster -  Oncology with visiting sessions at the nearby Treatment Centre.


This post will provide an opportunity to develop site specific-services for the systemic therapy of cancer in Leinster as part of the development of the Oncology Service.  This will be the third substantive Medical Oncology post to be appointed in Leinster, the ninth of their initial intended complement of nine whole time equivalent consultant oncologists (3 medical, 6 clinical). Our client envisages that the priority for this post will be to contribute to the development and delivery of our services in Upper GI and Lung cancers. However, as this is a new department, there will unrivalled opportunities to contribute to the development of the medical oncology service across the Trust. Other site specialist interests may be accommodated, for example in gynaecological oncology. Input into the care of gynaecological oncology patients attending the secondary site is envisaged in the initial job plan, and this could be developed according to the interests of the postholder. The post-holder will also participate in the Acute Oncology cover at the hospital. Support for the Acute Oncology service will be shared with the other Medical & Clinical Oncology consultants currently in post.


In addition to the consultant oncologists, 3 trust grade doctors in Oncology provide support to outpatient, chemotherapy and acute oncology services. A 4th such post has been agreed and will be appointed to in the near future. Arrangements for junior medical staff (training posts) are currently under discussion with nearby university hospitals. The Centre incorporates a new radiotherapy facility plus supporting physics and radiology staff to deliver the majority of radiotherapy treatment for the Dublin population within the county. This complements the existing chemotherapy and outpatient services. There has been a recent transition of patients from the previous services delivered by other providers. In the transitional phase, Oncology patients have continued to have their radiotherapy treatments at a diminishing rate ‘out of county’ as the Dublin Centre becomes fully operational.


The twin departments at both sites will have a common governance structure, an integrated electronic management system and e-prescribing facility.


Clinical Duties (guideline only)

The principal duties of the post will be to develop and support existing services within Dublin via site specific MDT’s to help support and develop an Oncology services across the Trust. This will include supporting and the Acute Oncology nursing team across the Trust

·       The post-holder will be expected to provide specialised Medical Oncology services to patients of the Hospitals HSE Trust, General Practitioners and to attend multi-disciplinary meetings. The successful candidate will be expected to work within multi-disciplinary teams in providing high quality clinical care. It is anticipated work will be undertaken on 2 sites and closer to the patients home. The Trust is seeking to extend services to GP surgeries/health centres etc. and job plans may be revised in due course

·       The post-holder will provide consultation and advisory service to clinical colleagues in the Trust and Primary Care.

·       The post-holder will attend site-specific and directorate level meetings.

·       To collaborate with colleagues to deliver a high quality, timely service and to work towards meeting the Trust’s and departmental objectives

·       The post-holder will develop close collaborative links with Clinicians working in his/her area of specialist expertise and work to develop services to patients in these areas to the highest standard, according to departmental protocols and Trust guidance. To include development of diagnostic and therapeutic protocols and guidelines with all appropriate stakeholders and across organisations where necessary

·       The post-holder will be supported to participate in the Trust’s annual appraisal as described by DOH and leading to job planning process

·       The post-holder will seek approval for any absence from the Trust such as annual leave, professional leave, study leave, time in lieu for Bank Holiday working & other leave and make available this record at your job plan review

·       The post-holder will participate in the on call rota to ensure that both emergency and urgent services are provided out of normal hours, to be shared equally with Consultant colleagues in post

·       Consultant Oncologists (Medical and Clinical) will provide cross cover for periods of annual and study leave

·       To facilitate effective quality assurance and audit and to ensure that services comply with national standards and guidance

·       The post-holder will take part in departmental and multi-disciplinary audit programmes in addition to undertaking individual audit. Participation is required in the Departmental Clinical Governance Programme

·        The post-holder will participate in regular clinical meetings, post-graduate activities and maintain continuing medical education in accordance with the requirements of the relevant Royal College and Irish Medical Council.

·       Share responsibility for data protection arising out of the use of computers

·       Comply with all relevant Trust policies and procedures

·       Adhere to the pledges as laid out in the NHS Constitution

Standard Duties and responsibilities

a) To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan  and in line with policies as specified by the Employer.

b) To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.

c) To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.

d) To co-operate with the expeditious implementation of the Disciplinary Procedure 

e) To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.

f) To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.

g) To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.

h) To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.

i) To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.

j) To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 

k) To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.

l) To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery

Teaching Commitments

·       regular commitments to ward based teaching of undergraduates.

·       participation in the lecture programmes for 2nd and 4th & 5th year medical students.

·       teaching of other groups as required including other specialty juniors and specialist nurses.


Job Plan

A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead Oncology Consultant, on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.

It will be reviewed at 3 months & thereafter annually or at any time, but no less than 3 months after a previous review, as requested by the appointee, Operational Medical Director or Clinical Lead & adjusted accordingly to the agreement of both parties.

There is also an on call commitment with the other consultant Oncologists with a rota of 1 in 8. On call duties are classed as Category A (5%) and attract an additional 0.25 PA for all consultants participating in the rota. The on call consultant will, under usual circumstances, be available for telephone advice out of hours and conduct a ward round review of oncology patients on Saturday & Sunday. There may be an occasional requirement to assess a patient out of hours at other times.

The directorate includes Haematology, Oncology and Palliative Care and has strong links with other specialties and clinical support services in the Trust.

Requirements

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of Clinical and Oncology Medicine.

Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant  will offer mentoring support and lead the existing clinical team through; joint clinics, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.
