The Post

This is a new Consultant post in the Clinical Haematology Department, which is part of the Directorate of Specialist Medical Services based at our client’s HSE trust hospital. The post holder will provide a Consultant service in Thrombosis and Haemostasis and Haemoglobinopathy.

 

The Trust

The hospital is a major teaching Trust with two hospitals.

 

The Trust has recently undergone a comprehensive re-building programme through the completion of one of the largest PFI initiatives in the HSE.  The programme, which was completed in 2009, has new facilities for medicine, obstetrics/gynaecology, and ophthalmology and brings children’s services on to the hospital site. The site now consists of 2 hospitals combined in one facility.

Our client’s hospital is a major teaching hospital.  It is situated immediately adjacent to the campus of a Medical School and University. The University is also in close vicinity.  It covers a very wide range of supra-regional, regional and district specialities, including nephrology, renal and pancreas transplantation, clinical haematology, cardiology and cardiothoracic surgery. The secondary site, on the same site, has beds for regional genetics and regional obstetrics and gynaecology.

The HSE trust aims within the next 5 years to be one of the top 5 HSE Trusts for research and innovation in the Republic of Ireland. The hospital have a number of important assets to help us achieve this ambitious goal. The Clinical Research Facility (CRF), purpose built for experimental medicine research and incorporating an MRI research scanner suite, is one of only 2 in the HSE with both adult and paediatric facilities. The Centre for Applied Diagnostics and Experimental Therapeutics, a multimillion pound new development uniquely designed and built within the hospital to enable application of proteomics, metabolomics and genomics to experimental medicine.

Department of Haematology

The University Department of Clinical Haematology provides a regional tertiary referral service for haematological malignancy, stem cell transplantation, Thrombosis, Haemostasis and Haemoglobinopathies. In addition, it provides general clinical haematology services for the local population of the Leinster Region. This service is provided by five consultants with an interest in malignant haematology, four consultants with an interest in Thrombosis and Haemostasis, Two consultants with an interest in Haemoglobinopathy, one with an interested in laboratory diagnostics and one consultant with an interest in blood transfusion. Two consultants with an interest in general and malignant haematology work at Secondary Hospital. Although there is a high level of sub-specialisation within the department, the unit is nevertheless very cohesive and collaborates closely on all levels.

 

The in-patient base- This recently opened facility includes 14 en-suite rooms, 10 of which are fitted with HEPA filtration. Day-Case treatment and apheresis procedures are provided in the Haematology Day Unit which is immediately adjacent to, and inter linked with the in-patient base. In addition there is also a purpose built out-patient suite including the Haemophilia Centre, which ensures that all aspects of Haematology are co-located in the new hospital development.

 

The anticoagulant service is provided by a team of specialist nurses with a Consultant Haematologist lead. There is a comprehensive critical care service on site. Children requiring specialist haematological care are currently referred to the Hospital.

Clinical Duties (guideline only)

·       The appointee will be expected to work closely within an integrated, multidisciplinary team of doctors and nurses. He/she will provide support to the haemostasis and thrombosis and Haemoglobinopathy service including participation in out patient clinics. Specifically, these are  likely to include:

·       Two General Haemostasis, Haemophilia and Thrombosis clinics weekly.

·       Sickle Cell clinic once weekly

·       Young persons Haemoglobinopathy clinic every 4 weeks

·       Teenage haemoglobinopathy clinic every 3 months

·       Joint haemophilia/ orthopaedic clinic every 6 months (alternating with colleagues)

·       Medical cover for haemophilia/ haemostasis day unit one in 4

·       Medical cover for haemostasis in-patients on daily basis

·       The appointee will participate in the clinical research conducted within the haemostasis and thrombosis service.

·       The appointee will participate in the ward attending rota in conjunction with the other Thrombosis and Haemostasis Consultants (1 in 4) and with the haemoglobinopathy consultant (1in 2)

·       The appointee will undertake laboratory duties in line with his/her clinical interests.

·       The appointee will be expected to undertake both undergraduate and postgraduate teaching, both inside and outside the Trust.

·       The appointee will participate in administrative and management roles of the department in conjunction with other consultants

·       The consultant will be expected to provide consultant cross-cover for annual/study leave and short-term sickness for other consultants in the department on a rotational basis.

Emergency work and On Call arrangements

The on call rota has been recently reviewed in light of cancer standards. There are 3 separate Consultant on call rotas: The malignant haematology rota (1:5) covering inpatients and acute complications in outpatients, a rota for haemostasis and thrombosis (1 in 3) and one for general haematology covering also secondary hospital (1:4). The workload for this on call rota has been taken into consideration when job planning.

 

Our client has a policy for emergency admission of acutely unwell patients. This will be directly under the haematology team or initially via the on call medical teams depending on diagnosis and time of presentation. The specialist trainees and Speciality Doctors / Registrar’s are currently on a 1 in 10 partial shift on-call rota, the resident element of which is 9am to 9.30pm daily.

Teaching Commitments

·       regular commitments to ward based teaching of undergraduates.

·       participation in the lecture programmes for 2nd and 4th & 5th year medical students.

·       teaching of other groups as required including other specialty juniors and specialist nurses.


Standard Duties and responsibilities

a)       To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan  and in line with policies as specified by the Employer.

b)       To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.

c)       To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.

d)       To co-operate with the expeditious implementation of the Disciplinary Procedure 

e)       To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.

f)        To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.

g)        To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.

h)       To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.

i)         To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.

j)         To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 

k)       To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.

l)         To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery


Job Plan

A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead Haematology Consultant, on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.

It will be reviewed at 3 months & thereafter annually or at any time, but no less than 3 months after a previous review, as requested by the appointee, Operational Medical Director or Clinical Lead & adjusted accordingly to the agreement of both parties.

Requirements

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of Clinical and Haematology.

Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant  will offer mentoring support and lead the existing clinical team through; one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.
