Overview

Our HSE Trust client is looking for an Emergency General Surgeon Consultant to join the Department of General Surgery.  This is a new full time post encompassing 10 Pas and has been created to enhance the emergency surgery pathway.  All duties will be carried out at the hospital but prospective candidates should also be aware of the potential impact of the “Healthier Together” programme in the area which in future will mean that hospital sites will work in cluster to deliver acute general surgery.  This will mean cross site working, potentially with a nearby University Hospital Trust.

 

Applications are invited from general surgeons, with a specialist interest in emergency surgery. The post holder will join the on call rota for general surgery and will also lead on a number of clinical innovations including the Surgical Assessment Unit; ambulatory pathways and the establishment of a daily ‘hot clinic’. Other responsibilities will include out-patients, day case surgery, general surgery and endoscopy dependent on the successful candidates skill mix.

Clinical Duties (guideline only)

The General Surgery Department is a busy unit, dealing with high levels of demand.  In 2014/1, 6,621 elective General Surgical patients were treated at the hospital of which 1,245 were inpatients and 5,376 as day cases.  In addition a further 2,241 patients were treated non-electively.  A total of 23,254 outpatient attendances were recorded for the department comprising 11,789 new, 10,891 follow-ups and 574 out-patient procedure appointments.

The key areas for the post holder will include:

• The clinical duties will be shared with the other consultants and will include all duties normally expected in a General Surgery department. Although the job is primarily to work in Emergency General Surgery, there will be an elective component to the job plan.  Participation in the on call rota on a 1:7 basis, including weekends.  The usual commitment for prospective cover is anticipated at this level

• The applicant must be proficient in the management of all stages of the emergency surgery pathway, from triage and assessment to surgery and post-operative care.

• The provision of clinical leadership in the delivery of emergency general surgical services and responsibility for the supervision, training and appraisal of junior staff. Collaborative working with nursing and other colleagues to organise effectively and manage the case-mix in general surgery on a day-to-day basis. The post holder will be asked to lead the development of the SAU on quality, safety and improved patient flow. This will be done in conjunction with his general surgical colleagues

• Active involvement in the planning, scheduling and delivery of elective and emergency surgery.  Working co-operatively with colleagues and others within the Trust to achieve the Government's targets for service improvement and delivery in relation to outpatient, elective and emergency care.

• In conjunction with other clinical teams and the wider health economy develop and implement integrated elective and emergency care pathways throughout the Trust. Participation in relevant audit and quality assurance processes, including reviews of manpower requirements.

• Teaching of the junior doctors is an integral part of the clinical work. It is envisaged that all consultants will have an involvement in teaching and training junior colleagues, but overall responsibility will be with one consultant (The Educational Supervisor) on a rotating basis (if applicable).

• The post-holder will be expected to take part in the administration of the department and the involvement in the management of resources will be regarded as essential.

• Share, with colleagues, on-call responsibility for the Department, including responding to unexpected absences and higher than usual patient numbers.

• Undertake specific management functions, as agreed with the Lead Consultant, in the running of the Department.

• Attend the Departmental Meetings arranged at monthly intervals. 

• Ensure, wherever possible, clinical practice is standardised and evidence based.

• Be responsible for developing protocols for the management, referral and admission of patients in agreement with other clinical departments and where appropriate, in line with Trust policy.

• Provide management of in-patients and out-patients

• Together with fellow consultant colleagues, the clinical supervision of all staff working in the department in addition to the organisation and delegation of clinical work within the department.

• Ensure the Trust’s policy is implemented in the event of a major incident.

• Develop an area of special interest as outlined above, in agreement with the Lead Consultant and Director of Clinical Services, that is compatible with the existing special interests of colleagues and the strategic direction of the Trust as a whole.

• Manage clinical and non-clinical complaints in accordance with Trust policies with particular emphasis on timeliness and local resolution.

• Adhere to Trust Policy with regards 18 weeks Waiting List Management, Booked Admissions, Booking and Choice and the like.

• Understand and be responsive to activity targets under Payment by Results (PbR)

A mentor will be available to the successful applicant.

Teaching Commitments

·       regular commitments to ward based teaching of undergraduates.

·       participation in the lecture programmes for 2nd and 4th & 5th year medical students.

·       teaching of other groups as required including other specialty juniors and specialist nurses & FRCS candidates.

 

Job Plan

A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead
General Surgery Consultant, on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.

Currently, this post carries a 10 PA session contract comprising:

 

·       8  PAs for Direct Clinical Care (DCC includes on-call) comprising 5.25 clinical sessions, 1.25 PA ward rounds/MDT, 1.5 PA for participation in a 1 in 7 Consultant on-call rota, 0.5 patient Admin.

 

·       1.5  PAs for supporting professional activity (SPA)  includes dedicated audit session

 

All Consultants will have 1.5 SPA allocated to cover core requirements (continuing professional development, participation in Job Planning, appraisal, mandatory training, essential audit and clinical governance activity) and, subject to agreement in advance, may be worked away from the hospital site.  The evidence of their activity in relation to this SPA will be required as part of the consultant’s revalidation portfolio.

Any SPAs allocations above the core 1.5 SPA will be determined through the Team Job Planning exercise coordinated by the Lead Consultant within each Division/Specialty as appropriate.  SPAs will be allocated for specific tasks/responsibilities and will include a PA value for the task as well as expected outputs.  Any SPAs above the core SPA must be worked on site.

 

·       DCC Programmed Activities are 4 hours in duration, 08:30 to 12:30 AM and 1:30 to 5:30 PM are the default timings

It will be reviewed at 3 months & thereafter annually or at any time, but no less than 3 months after a previous review, as requested by the appointee, Operational Medical Director or Clinical Lead & adjusted accordingly to the agreement of both parties.


Requirements

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of Clinical and General Surgery.

 

Standard Duties and responsibilities

a) To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan  and in line with policies as specified by the Employer.

b) To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.

c) To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.

d) To co-operate with the expeditious implementation of the Disciplinary Procedure 

e) To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.

f) To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.

g) To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.

h) To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.

i) To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.

j) To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 

k) To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.

l) To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery

Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant  will offer mentoring support and lead the existing clinical team through; joint clinics, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.
