Overview
Our HSE client requires two Consultant Paediatricians to join
their service. These posts have arisen from our desire to improve the training
opportunities for our paediatric trainees in region, while maintaining 2 acute
paediatric sites until reconfiguration. The paediatric services in the hospital
are in the process of reconfiguration with the plan for the entire paediatric
and neonatal services of the area to be merged and provided at one site.
The
post holders will join a team of 10 WTE paediatric consultants, and will be
expected to contribute to the out-of-hours resident on call duties as detailed
in the job plan. In addition the post holder will participate in all aspects of
the Paediatric Service at the Hospital including paediatric service weeks, and
paediatric follow-up with regular out-patient clinics. Adequate time off will
be given in lieu of out-of-hours work.
The
client would also look to these new posts to support specialist services
appropriate to Level 2 activity, such as ambulatory paediatrics, diabetes,
cardiology, gastroenterology, adoption and epilepsy (diabetes, allergy/resp,
neurodisability, gastro, adolescent paediatricss, ambulatory, audiology). There
will be the opportunity to evolve or develop a special interest to complement
those of the rest of the team and to contribute to audit and research on the
unit.
The
posts will be based in the Paediatric Department at the HSE teaching Hospital
in Leinster, and will be expected to deliver clinics in peripheral settings.
The
posts comprise of 10 sessions including the minimum of two sessions for
supporting professional activities (SPAs). The number of sessions and/or the
balance of direct care sessions (DCCs) and SPAs may subsequently vary with the
agreement of the post holder and the HSE but based on the needs of the
department will require agreement at job planning.
This
consultant post includes a hybrid component and there will be 6 other
colleagues with identical job plans. There will be fixed commitments to middle
grade resident-on-call duties. There will be approximately 39 nights, 4.3
weekend long days and 8.7 week evenings per year at the hospital on the middle
grade tier. You will work 3.7 ward weeks including weekends per year as 3rd
tier (non-resident). In addition there will be commitments for consultant of
the week cover and out-patient clinics.
It
is envisaged that the job plan will be reviewed significantly when we merge
with the paediatric team from our sister unit at a nearby hospital, in their exciting
single site paediatric service and separately staffed neonatal unit in the Specialist and Critical Care Centre.
Clinical Duties (guideline only)
The
exclusive responsibility of the post is for paediatric medicine. The specific
and fixed clinical responsibilities are as follows:
1.
Ward week at tier 3 at the Hospital, occurs on a 1:14 basis including the
preceding weekend ward rounds on a 1:14 basis (3.7/yr). It attracts 12
sessions. It involves tier 3 activity, supervising a team of tier 1 and 2
doctors and being responsible for all inpatients, new admissions and the
newborn infants in the postnatal and neonatal unit. Responsibilities will
include attending the morning meeting, supervising the ward round, ensuring
allocation of work for the day, supporting teaching and clinical supervision of
junior staff.
2.
Weekday resident-on-call at middle grade level at the hospital with a
commitment for 39 nights/year. During these resident-on-call nights, a
nominated consultant will be available (off site) for advice and support.
Duties will involve a complete and independent responsibility of all
in-patients, Level 2 neonatal service and new admissions.
3.
Weekend resident on-call at middle grade level, 9am-9:30pm on a 1:12 basis (4.3
per year). This is scheduled on-call activity covering all inpatients, acute
admission and Level 2 Neonatology. There will be a non-resident consultant
covering for support, who will also attend for ward rounds.
4.
Weekend resident on-call (nights) - there are none anticipated in the current
job plan.
5.
Non-resident consultant on-call (16 hours, 5pm to 9am) covering the hospital on
a 1:14 basis. Your duties will involve a complete responsibility of all acute
paediatric referrals, in-patients, safeguarding enquiries and Level 2
neonatology. There will be middle-grade and a junior tier doctor to help and
you are expected to be available if needed within 30 minutes of being called.
6.
Paediatric outpatient clinics (including specialty clinics) shared with other
paediatric consultants. The current commitment would depend on the workload
generated but will be around 38 clinics annually. Each clinic attracts additional
administrative time.
7.
Safeguarding – During your ward week and when on–call you will be responsible
for liaising with partner agencies in delivering medical opinion for child
protection.
8.
Other responsibilities include:
a)
To participate in development of and undertake all duties and functions
pertinent to the Consultant’s area of competence, as set out within the
Clinical Directorate Service Plan and in
line with policies as specified by the Employer.
b)
To ensure that duties and functions are undertaken in a manner that minimises
delays for patients and possible disruption of services.
c)
To work within the framework of the hospital / agency’s service plan and/or
levels of service (volume, types etc.) as determined by the Employer. Service
planning for individual clinical services will be progressed through the
Clinical Directorate structure or other arrangements as apply.
d)
To co-operate with the expeditious implementation of the Disciplinary
Procedure
e)
To formally review the execution of the Clinical Directorate Service Plan with
the Clinical Director / Employer periodically. The Clinical Directorate Service
Plan shall be reviewed periodically at the request of the Consultant or
Clinical Director / Employer. The Consultant may initially seek internal review
of the determinations of the Clinical Director regarding the Service Plan.
f)
To participate in the development and operation of the Clinical Directorate
structure and in such management or representative structures as are in place
or being developed. The Consultant shall receive training and support to enable
him/her to participate fully in such structures.
g)
To provide, as appropriate, consultation in the Consultant’s area of designated
expertise in respect of patients of other Consultants at their request.
h)
To ensure in consultation with the Clinical Director that appropriate medical
cover is available at all times having due regard to the implementation of the
European Working Time Directive as it relates to doctors in training.
i)
To supervise and be responsible for diagnosis, treatment and care provided by
non-Consultant Hospital Doctors (NCHDs) treating patients under the
Consultant’s care.
j)
To participate as a right and obligation in selection processes for
non-Consultant Hospital Doctors and other staff as appropriate. The Employer
will provide training as required. The Employer shall ensure that a Consultant
representative of the relevant specialty / sub-specialty is involved in the
selection process.
k)
To participate in clinical audit and proactive risk management and facilitate
production of all data/information required for same in accordance with
regulatory, statutory and corporate policies and procedures.
l)
To participate in and facilitate production of all data/information required to
validate delivery of duties and functions and inform planning and management of
service delivery
Teaching Commitments
·
regular commitments to
ward based teaching of undergraduates.

lecture programmes for 2nd and 4th & 5th
year medical students.
·
teaching of other
groups as required including other specialty juniors and specialist nurses.
Job Plan
The duties described here are provisional and will be the subject of annual
review and will form a composite part of the job plan which will be agreed
between the post holder and the Chief Executive or nominated deputy.
The work programme will
be based on a contract for 10 sessions under the Contract for Consultants in
the HSE, incorporating 8 direct clinical care sessions (DCC) and 2 sessions for
Supporting Professional Activities or ‘SPA’ (teaching, meetings, clinical
governance activity, research, etc). Educational supervision, if undertaken,
will attract an additional 0.25 SPAs per trainee.
Uncontracted time will
be taken and organised flexibly so as not to impact on service delivery or
outpatient service delivery.
A formal job plan will be agreed between the appointee, the Operational Medical
Director and Lead General Paediatrics Consultant, on behalf of the Trust’s
Medical Director, based on the provisional timetable. This will be signed by
the Chief Executive and will be effective from the commencement date of the
appointment.
It will be reviewed at 3 months & thereafter annually or
at any time, but no less than 3 months after a previous review, as requested by
the appointee, Operational Medical Director or Clinical Lead & adjusted
accordingly to the agreement of both parties.
Resident
On-call
1.
Week
night on-calls:
On-call approximately 39 nights a year (with prospective cover). During these
resident-on-call nights, a nominated consultant will be available (off site)
for back-up. Duties will involve a complete responsibility of all in-patients,
new admissions and neonatal activity.
2.
Weekend
on-calls: Weekend on-call
approximately 1:12 long days amounting to 8.6 weekend long days (approximately
4.3 weekends). During these resident on-calls, a nominated consultant will be
available (off site) for back-up. Duties will involve a complete responsibility
of all inpatients, new admissions and neonatal activity.
3.
Week
evenings: Covering 4:30pm-9pm, approximately 1:6,
amounting to 8.67 per year. During these resident sessions a nominated
consultant will be available (off site) for back-up. Duties will involve a
complete responsibility of all inpatients, new admissions and neonatal
activity.
Consultant of the week
With these appointments there will be
10 consultants contributing to the ward week arrangements. Your job plan will
include 3.7 such weeks per year. During this week you will be independently
responsible for all activity within the department. You will chair the morning
meetings, allocate the work for the day, supervise the ward rounds on the
paediatric ward, manage all safeguarding enquires, liaise with the visiting
neonatal consultant on 3 days of the week and supervise the neonatal ward round
for the remaining 2 days. You will support all the learning activities of that
week.
These weeks are obligatory commitments
and cannot coincide with leave. Active teaching of junior staff and medical
students is expected during ward weeks
Consultant on-call
You will participate in a consultant
on-call rota and contribute on a 1:14 basis covering the unit as the senior
most medical doctor in the unit. This is an out-of hours cover from 5pm to 9am,
where there will be a middle grade and a tier 1 doctor available in the
department. During this time you will not be resident in the unit, but will be
responsible for all in-patients, new admissions and neonatal activity.
Outpatient Clinics
Your job plan allows for up to 78
clinics per year (2/week when not on ward week). They may be delivered anywhere
within the Health Board as meets the demands of the service. Within this
allocation you will be able to pursue your special interest, however it is
expected that you will continue to offer a regular general paediatric clinic
service. Clinical administration time will be given in addition.
Supporting Professional Activity (SPA)
The posts include plans comprising 10
sessions including the minimum of two sessions for supporting professional
activities (SPAs). During this time you will be expected to participate in
teaching and training, audit and research, quality improvement, protocol
development, database management, general management related duties, attending
non-clinical meetings, service development clinical supervision and support,
and personal development. These activities will be monitored in annual
appraisals.

Requirements
This post is open to
doctors who hold Registration as a Specialist in the Specialist Division of the
Registrar of Medical Practitioners maintained by the Medical Council in Ireland
in the specialty of Clinical and General Paediatrics.

All Consultants are
required to attend the monthly Directorate Meetings. Post holders will be
expected to share in administrative duties allocated by mutual agreement within
the Directorate. As part of the Consultant Team, the consultant will offer mentoring support and lead the
existing clinical team through; joint clinics, one-to-one support, individual
case management and complex cases, identifying development needs and suggesting
CPD (Continuous Professional Development) opportunities and responding to
clinical enquiries via e-mail and telephone.
