Overview

 

The department has 9 general paediatricians (including these posts), of which 3 contribute to diabetes on call but, together with one of the academic consultants also contribute to the general paediatric rota and outpatient clinics.

The department admits around 5000 children under its care annually and provides 15 general paediatric and 10 rapid access clinics each week. There are around 56,000 new attendances to the hospital emergency department annually, of which about 10% are admitted.

 

Approximately 5% of emergency department attendances are direct referrals to the medical on call team by the primary care services. Up to 30 paediatric patients are admitted within each 24 hour period (may be more in winter) and currently one of 9 consultants is responsible for these patients.

 

The post-take ward round is undertaken by the on call consultant the following day, including at weekends. At a suitable time, children who are already under the care of another consultant (or other speciality) are transferred to that consultant's team. Each consultant is then responsible for arranging on going care and follow-up where appropriate. The general paediatric consultants also take part in the 1 in 9 consultant of the week duties when all other elective work and the outpatient clinics are cancelled. Outpatient duties are scheduled for rest of the weeks.

 

A dedicated GP hotline service is provided by the consultant of the week between 11am and 1pm. Seriously ill children are resuscitated and stabilised in the Emergency Department which has Emergency Paediatric Consultant presence until midnight, although they may stay longer if there is a clinical need. Children in the Paediatric Intensive Care Unit are managed by the Paediatric Intensivists until discharged to a ward under the care of the general paediatric consultant of the week or a specialty team. There is a neonatal surgical unit, but no neonatal intensive care at the hospital.

 

Children’s Community Nursing Team (CCNT) assists in early and safe discharge of children. They help in administration of I.V antibiotics at home or providing general assessment and support following discharge. They are supported by the OPAT (Outpatient Parenteral Antibiotic Therapy) and IV access teams, who advise on and monitor patients receiving IV antibiotics at home, and can assist in obtaining IV access. Child protection is the responsibility of all professionals working with children. In the hospital there is a team of clinicians based in the centre who have special expertise in this area and who provide a 24 hour support service for child protection. Social services are now largely community based.

 

The general paediatric department consists of 5 teams covering the 9 week rota cycle. Each team has 2-3 general paediatricians, associate specialists or staff grades, ST trainees, Foundation year doctors and GP trainees. . They have a dedicated ward (4C) for the majority of general paediatrics and diabetes in patients, supported by Advanced Nurse Practitioners. The general paediatricians are encouraged to offer special interests of their choice to complement the existing interests within the paediatric department; and that is consistent with the strategic direction of the Trust and its needs.

 

The current special interests within the paediatric department are eating disorders, diabetes, epilepsy and allergy. They also have a general paediatrician who works part time as a Primary Care Physician, linking with the GP and community services.

 

There is a senior staff meeting for general paediatrics twice monthly.

 

Main duties and Responsibilities and Key Relationships

 

·       Responsible for the examination, investigation, diagnosis and treatment of acutely unwell general medical patients.

·       To work as part of a multidisciplinary Team.

·       Contribute to both general paediatric outpatient clinics and Rapid Access clinics.

·       Responsible for all inpatients admitted under medical team referred by the GP or emergency department until discharged.

·       Advice and clinical consultation to other inpatient services as appropriate e.g. general surgery.

·       As part of consultant of the week to support the GP Hotline to advise local primary care providers in paediatric case management.

·       Working with local Clinical Commissioning Groups (via the Clinical Director) to promote general paediatric care.

·       Liaison with senior colleagues to ensure the provision of effective paediatric clinical services.

·       Responsibility in respect of the reasonable and effective use of hospital and district resources in liaison with General Paediatrics budget holder.

·       Liaison through the Clinical Business Unit with other staff of the hospital and the health authority.

·       Teaching and training of medical students, junior staff (medical and nursing) within the department and wards

·       Contribution to Clinical Governance activities in the Department

·       Participation in improvement initiatives in the Department e.g. implementation of National Guidance.

Other responsibilities:

a) To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan  and in line with policies as specified by the Employer.

b) To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.

c) To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.

d) To co-operate with the expeditious implementation of the Disciplinary Procedure 

e) To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.

f) To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.

g) To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.

h) To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.

i) To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.

j) To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 

k) To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.

l) To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery

Teaching Commitments

·       regular commitments to ward based teaching of undergraduates.

·       participation in the lecture programmes for 2nd and 4th & 5th year medical students.

·       teaching of other groups as required including other specialty juniors and specialist nurses.

 

Job Plan

The duties described here are provisional and will be the subject of annual review and will form a composite part of the job plan which will be agreed between the post holder and the Chief Executive or nominated deputy.


A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead General Paediatrics Consultant, on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.

It will be reviewed at 3 months & thereafter annually or at any time, but no less than 3 months after a previous review, as requested by the appointee, Operational Medical Director or Clinical Lead & adjusted accordingly to the agreement of both parties.

Example:

This is a 10PA job which includes 8.5 DCC and 1.5 SPA.

The on call Band is Category B and attracts a 1.0% supplement. Clinical sessions will be worked predominantly 9-5, with one session until 7pm followed by 8 am post take ward round provided when on call.

Each consultant contributes to a 1 week in 9 ‘hot week’ rota, during which clinics and elective work will be cancelled and the consultant duties will be focused on acute care and inpatient referrals only.

DCC includes:

·       Outpatient referral triage SPA includes:

·       1 PA Appraisal and including Mandatory Training

Requirements

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of Clinical and General Paediatrics.

Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant  will offer mentoring support and lead the existing clinical team through; joint clinics, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.
