Overview

Main Tasks & Overview of Responsibilities

 

The successful candidate will be joining a large medical division to provide consultant input and leadership for a General Medical Ward. They will be supported by a comprehensive team including an appropriate complement of junior doctors.

 

The hospital has introduced the SAFER and SHOP principles forward activity and it is expected that two consultants will be appointed for the purpose of covering a 35 bedded medical ward.

 

There is also a requirement to take a full role in the General Medical Rota currently on a 1 in 12 basis though under review.

 

As well as providing clinical expertise on the shop floor, you would be supported by our Quality Improvement Directorate in helping to develop and enhance our ward-based care. The hospital has strong links with the nearby University and there are opportunities to pursue roles in undergraduate or postgraduate education.


Your clinical duties will include:

 

· Daily morning ward rounds including clinical assessment and treatment of a diverse caseload of patients admitted to the ward.

· On call G(I)M rota

· Up to 2 outpatient clinics per week – specialty to be determined

 

The post is a fixed term position with eligibility to a substantive post and therefore essential criteria include Full IMC Specialist Registration with a CCT (or proposed date for CCT within 6 months of appointment) in Acute Medicine or GIM.

 

We would be open to discussion with dual accreditation candidates who may have a CCT in a relevant specialty with GIM.

 

Candidates must be able to demonstrate a commitment to General Medicine.

 

Job Plan


A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead General Medicine Consultant, on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.

It will be reviewed at 3 months & thereafter annually or at any time, but no less than 3 months after a previous review, as requested by the appointee, Operational Medical Director or Clinical Lead & adjusted accordingly to the agreement of both parties.

The proposed job plan provides an example of how the working week would be structured; the appointee will be able to discuss a definitive job plan with the Service Manager and Clinical Director upon accepting the post.

 

a)       Direct Clinical Care includes:

All Consultants will be allocated 1 core SPA per week for professional development (CPD), Job Planning, Mandatory Training, attendance at regular team meetings which will have a clinical governance aspect and participating in Audit.

 

Additional SPA time will be made available to Consultants on the basis of evidenced need. Such allocation will be agreed and subsequently reviewed by the Consultant and their Clinical Director during the Job Plan review session. Clinical Directors will be able to approve a second SPA but anything above that level will need the approval of the Chair of Division.

 

SPA time will generally be undertaken within the Trust or at other agreed HSE sites or academic institutions. The authorisation of the Chair of Division will be required where it is proposed that SPA time will be undertaken at any other location e.g. at home.

 

On-Call

 

The on-call commitment is currently one in 12 for unselected General Medicine which covers weekday evenings and 1 in 12 for weekends.

 

Special Interest

 

The successful candidate will be expected to contribute to all areas of the team’s activity and opportunity to develop a special interest will be supported if this can be arranged with the appropriate department.

 

Resource Management

 

The post holder will be required to ensure that services are delivered within agreed parameters. Any external duties that impinge on Programmed Activities you may wish to be included in your job plan must be agreed with the Clinical Manager at least 1 month in advance, where practicable. Standard annual leave notification is in line with Trust policies and the leave year is from the 1st April.

 

Standard Duties and responsibilities

 

a) To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan  and in line with policies as specified by the Employer.

b) To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.

c) To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.

d) To co-operate with the expeditious implementation of the Disciplinary Procedure 

e) To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.

f) To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.

g) To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.

h) To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.

i) To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.

j) To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 

k) To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.

l) To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery

Teaching Commitments

·       regular commitments to ward based teaching of undergraduates.

·       participation in the lecture programmes for 2nd and 4th & 5th year medical students.

·       teaching of other groups as required including other specialty juniors and specialist nurses & MRCP Candidates.


Requirements

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of Clinical and General Internal Medicine

Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant  will offer mentoring support and lead the existing clinical team through; joint clinics, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.
