Overview
Our HSE clients
is looking for a dynamic and enthusiastic colleague to join our team of 4
Consultants providing comprehensive Gastroenterology and Liver services. Most
importantly we are seeking someone who will fit in well with our happy and
productive team in our very modern and well equipped unit. Subspecialty
interests will be very welcome, particularly subspecialty training in
hepatology, but we would also welcome expertise in endoscopy training, EUS, GI
physiology Diagnostic and Therapeutic endoscopy, bowel cancer screening,
inflammatory bowel disease, and nutrition services are already well developed,
complementary skills will be very welcome.
The post holder
will provide outpatient, inpatient and endoscopic Gastroenterology services
along with on-call gastroenterology including weekend morning ward rounds and
out of hours GI bleed service (1:5 rota).The new appointee will also be
expected to take part in business planning of Gastroenterology services in
collaboration with the other consultants in the department and hospital
management.
This is a full-time position but candidates wishing to work part-time or
job-share are encouraged to apply. If a part-time or job-share person is
appointed, modification of the job content will be discussed with the appointee
on a personal basis. They will be required to devote substantially the whole of
his/her time for the appointed sessions to the duties of the post and to give
them priority at these times.
Specific Duties
(a) Provision
with Consultant colleagues of a service to HSE Hospitals group, with
responsibility for the prevention, diagnosis and treatment of illness, and the
proper functioning of the department;
(b)
Out-of-hours responsibilities, including participation in Consultant on-call
rota
(c) Cover for
colleagues' annual leave and other authorised absences;
(d) Any
responsibility which relates to a special interest.
(e)
Professional supervision and management of junior medical staff including the
observance of local employment and human resource policies and procedures;
(f)
Responsibilities for carrying out teaching, examination and accreditation
duties as required and contributing to undergraduate, postgraduate and
continuing medical education activity, locally and nationally;
(g) Participating
in medical audit, the Trust’s Clinical Governance processes and in CPD
is provided in
job plans and attendance at audit and other governance meetings is mandatory;
(h) Managerial,
including budgetary responsibilities where appropriate and compliance with
standing orders and standing financial instructions of the Trust.;
(i) Where it is
agreed between the parties in job planning and in line with the 2003 contract,
work on behalf of HSE Hospital trust such as domiciliary consultations, or
services provided by the Trust for other agencies, e.g. the prison service.
(This excludes work done under direct arrangements between an individual Consultant
and a third party, e.g. Category 2;)
(j) The post
holder must at all times carry out his/her duties with due regard to the
Trust’s Equal Opportunities Policy;
(k) It is the
responsibility of all employees to comply with Trust Health and Safety policies
and maintain a safe and healthy environment for patients, visitors and staff.
(l) All
employees have an individual responsibility to have knowledge of and employ the
basic principles of infection control practice and to ensure adherence to the
requirements in the Infection Control Manual.
(m) In line
with IMC Good Medical Practice it is the responsibility of the post holder to
ensure that all duties are carried out to the highest possible standard, and in
accordance with current quality initiatives within the area of work
(n) The post
holder is expected to respond in a timely fashion to legitimate requests from
Trust officers – this might include investigations of incidents or complaints;

(o) All staff
who have access to or transfer data are responsible for that data and must
respect confidentiality and comply with the requirement of the Data Protection
Act 1998, in line with the Trust’s policies.
(p) The post
holder is responsible for data quality and complying with the policies,
procedures and accountability arrangements throughout the Trust for maintaining
accuracy and probity in the recording of the Trust’s activities.
(q) The post
holder is expected to comply with Trust policies and procedures;
(r) Staff are
required to comply with the requirements of the Freedom of Information Act 2000
in line with Trust Policy.
(s) Any other
duties which may be required from time to time.
Standard Duties and responsibilities
a) To participate in development of and undertake all
duties and functions pertinent to the Consultant’s area of competence, as set
out within the Clinical Directorate Service Plan and in line with policies as specified by the
Employer.
b) To ensure that duties and functions are undertaken
in a manner that minimises delays for patients and possible disruption of
services.
c) To work within the framework of the hospital /
agency’s service plan and/or levels of service (volume, types etc.) as
determined by the Employer. Service planning for individual clinical services
will be progressed through the Clinical Directorate structure or other
arrangements as apply.
d) To co-operate with the expeditious implementation
of the Disciplinary Procedure
e) To formally review the execution of the Clinical
Directorate Service Plan with the Clinical Director / Employer periodically.
The Clinical Directorate Service Plan shall be reviewed periodically at the
request of the Consultant or Clinical Director / Employer. The Consultant may
initially seek internal review of the determinations of the Clinical Director
regarding the Service Plan.
f) To participate in the development and operation of
the Clinical Directorate structure and in such management or representative
structures as are in place or being developed. The Consultant shall receive
training and support to enable him/her to participate fully in such structures.
g) To provide, as appropriate, consultation in the
Consultant’s area of designated expertise in respect of patients of other
Consultants at their request.
h) To ensure in consultation with the Clinical
Director that appropriate medical cover is available at all times having due
regard to the implementation of the European Working Time Directive as it
relates to doctors in training.
i) To supervise and be responsible for diagnosis,
treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating
patients under the Consultant’s care.
j) To participate as a right and obligation in
selection processes for non-Consultant Hospital Doctors and other staff as
appropriate. The Employer will provide training as required. The Employer shall
ensure that a Consultant representative of the relevant specialty /
sub-specialty is involved in the selection process.
k) To participate in clinical audit and proactive risk
management and facilitate production of all data/information required for same
in accordance with regulatory, statutory and corporate policies and procedures.
l) To participate in and facilitate production of all
data/information required to validate delivery of duties and functions and
inform planning and management of service delivery
Teaching Commitments
·
regular commitments to ward based teaching of
undergraduates.

participation in the lecture programmes for 2nd
and 4th & 5th year medical students.
·
teaching of other groups as required including other
specialty juniors and specialist nurses.
Job Plan
A formal job plan will be agreed between the appointee, the Operational Medical
Director and Lead Gastroenterology Consultant, on
behalf of the Trust’s Medical Director, based on the provisional timetable.
This will be signed by the Chief Executive and will be effective from the
commencement date of the appointment.
It will be reviewed at 3 months & thereafter
annually or at any time, but no less than 3 months after a previous review, as
requested by the appointee, Operational Medical Director or Clinical Lead &
adjusted accordingly to the agreement of both parties.
Example Timetable
The hospital have adopted a rotational working
pattern; one consultant covers the ward with a morning ward round five days per
week, for 3 weeks at a time. This will average 6.5 weeks of the year. The ward
has 24 beds. A second consultant provides a daily acute GI service a week at a
time, again 6.5 weeks per year. The remaining consultants are ‘off the wards’
and some flexible working is expected to in order to maintain clinic and
endoscopy capacity across both sites.
Agreement should be reached between the appointee and
the Service Director with regard to the scheduling of all other activities,
including the Supporting Professional Activities. The timetable outlined is
flexible and we are very willing to discuss alternative arrangements with
suitable candidates who have a specialist area of interest that complements the
primary role. The following provides scheduling details of the clinical
activity and clinically related activity components of the job plan which occur
at regular times in the week. Agreement should be reached between the appointee
and the Head of Service with regard to the scheduling of all other activities,
including the Supporting Professional Activities.

Acute GI and Ward Round activity in the week will
replace the fixed sessions; this will be on a rotational basis among all
Consultants (1:5 rota). Therefore an annual delivery of approximately 29
morning sessions and 35 afternoon sessions would be expected. Weekend ward
rounds are timetabled for 3 hours Saturday and Sunday 1:8 with the GI on call.

(Including 0.66 PA Predictable On-Call and 0.16PA Unpredictable
On-Call)
• SPA – 1.5PA
3.4 On Call Availability (currently 5% Intensity
Supplement)
The GI team currently provides a reduced GIM rota which will cease by summer
2017 and an OOH GI bleed rota (1:8 including the new appointee) with weekend
cover. The successful candidate will be required to live within 15 miles of
their base hospital, or 30 minutes travelling time when on call. The current PA
calculation is 0.16 for out of hour’s endoscopy. Weekend gastroenterology ward
rounds are also timetabled in the job plan on a 1:5 basis
Study & Training
The applicant is expected to participate in
professional continuing medical education; study leave is provided for this
purpose, and the appointee will be entitled to apply to the Trust Study Leave
Committee for a contribution to funding of this activity. The Trust supports
the requirements for Continuing Medical Education as laid down by the Royal
College of Physicians and is committed to providing time and financial support
for these activities.
Requirements
This post is open to doctors who hold Registration
as a Specialist in the Specialist Division of the Registrar of Medical
Practitioners maintained by the Medical Council in Ireland in the specialty of
Clinical and Gastroenterology
Medicine.

appointee with secretarial & IT support.
Management Responsibility
All Consultants are required to attend the monthly
Directorate Meetings. Post holders will be expected to share in administrative
duties allocated by mutual agreement within the Directorate. As part of the
Consultant Team, the consultant will
offer mentoring support and lead the existing clinical team through; joint
clinics, one-to-one support, individual case management and complex cases,
identifying development needs and suggesting CPD (Continuous Professional
Development) opportunities and responding to clinical enquiries via e-mail and
telephone.
