Overview
The Trust has a purpose built anaesthetic pre-assessment service run by
nurse practitioners with regular Consultant sessions. There is a 32 bedded Short Stay Surgical Unit
in which selected patients for minor and intermediate procedures are treated on
a 12, 24 or 48 hour basis. All patients
are pre-operatively assessed prior to admission and the use of integrated care
pathways together with nurse led discharge has made effective use of the beds.
The main theatre suite contains 11 of the 17 operating theatres with a large
patient reception area and central recovery. The adjacency of ICU, the
Emergency Department and the short stay surgical unit has helped bring together
the core of our operative provision within one purpose built area. An emergency
theatre and a trauma theatre is available for emergency work all day increasing
consultant input and reducing out of hours emergency workload.
A 24-hour pain relief
service is provided for the busy obstetric unit, with 73 beds and averages 3500
deliveries per year. There is a
well-established Acute and Chronic Pain service with 3 chronic pain Consultants
and 2 pain sisters. Anaesthesia for ECT is provided for in the psychiatric
department
Our 13 bed adult Critical Care Unit currently is staffed to 6 HDU (level 2) and
7 ICU (level 3) beds.
The intensive care unit admits approximately 240
critically ill patients yearly with high APACHE 2 scores. The majority of these patients are ventilated
and have at least two organ failures.
The high dependency unit admits approximately 370 patients per year, a
high proportion of which are planned following elective surgery.
The critical care unit has 24 hour cover from a
team of fourteen consultants. Nursing
care is delivered on a 1:1 basis for level 3 patients and 1:2 for level 2
patients and there is multi-disciplinary support from physiotherapists,
dieticians, technicians, a pharmacist and administrative staff.
There are daily ward rounds which provide a forum for teaching and exposure to
practical procedures
Clinical
Duties
(guideline only)
The newly
appointed consultant will be expected to provide anaesthetic cover in theatre
for a variety of surgical specialties, with a mixture of both fixed and
flexible lists. The department organizes
its own rota to cover theatre sessions and there is part annualisation of the number of sessions within the job plan.
In addition, the appointed consultant will provide cover for both theatre and
the critical care unit when on call on weekday nights, and theatre cover at
weekends.
The
consultant will have a continuing responsibility for the care of the patients
in his / her charge and for the proper functioning of the departments and to be
able to be contacted when necessary.
· Professional supervision and
management of junior medical staff. If appropriate the post holder will be
named as Educational supervisor to a specific member to oversee their training
and as an initial source of career advice to such doctors.
· Responsibility for undertaking the
administrative duties associated with the care of patients and the
administration for the department.
· Requirement to co-operate with local
management in the efficient running of services and an expectation to share
with consultant colleagues in the medical contribution to management.
· Requirement to work within the
financial and other constraints decided upon by the Trust. Additional expenses
of any kind will not be committed without the approval of the appropriate
manager / budget holder.
· Complying with employment and
personnel policies and procedures in the management of employees of the Trust.
· Travel as necessary between hospitals
/ clinics will be required but a planned and cost effective approach will be
expected.
Weekly
provisional sessional commitment
The post is
for 10 Programmed Activities (PA).
Of these:
8.5 PA are for direct clinical care
(*2.38 PA
are for on-call work and 6.12 for weekday, daytime work)
1.5 PA are for Supporting PA’s**
The 6/12 PA
allocated for weekday daytime clinical duties will be the equivalent of four
theatre lists, each of approximately five hours duration. In addition, there are 40 flexible sessions
to be worked over an annual period. The annual sessions consist of 33 (equating
to 1PA each week) and seven to allow for variability in the on-call
commitment*.
*** The
on-call PA allocation may vary from one year to the next. Any changes will be
accommodated by adjusting the annual number of flexible clinical sessions
above.
· A typical week will therefore be four
lists, on-call work (an average of eight hours), and six hours of supporting
activity, some of which the Trust has agreed may be worked from home. There has been a synchronized SPA session
throughout the Trust that occurs on the fourth week of each month with the day
of the week varying.
· Theatre sessions run from 08.00 –
13:00 or 13:00 -18:00.
· The department works in a flexible
and collaborative way and so changes may be made during times of colleagues’
annual / study leave and under exceptional circumstances.
· An extra five hour session may be
undertaken by local agreement for an extra payment of 1.25PA.
· Annual and study leave arrangements
are co-ordinated within the Department to ensure that there is an acceptable
level of consultant cover.
·
Current
annual leave entitlement, in line with Consultant terms and conditions of
service are 30 days plus 2 statutory days and bank holidays. Study leave is
currently allocated.
Standard Duties and responsibilities
a) To participate in development of and undertake all
duties and functions pertinent to the Consultant’s area of competence, as set
out within the Clinical Directorate Service Plan and in line with policies as specified by the
Employer.
b) To ensure that duties and functions are undertaken
in a manner that minimises delays for patients and possible disruption of
services.
c) To work within the framework of the hospital /
agency’s service plan and/or levels of service (volume, types etc.) as
determined by the Employer. Service planning for individual clinical services
will be progressed through the Clinical Directorate structure or other
arrangements as apply.
d) To co-operate with the expeditious implementation
of the Disciplinary Procedure
e) To formally review the execution of the Clinical
Directorate Service Plan with the Clinical Director / Employer periodically.
The Clinical Directorate Service Plan shall be reviewed periodically at the
request of the Consultant or Clinical Director / Employer. The Consultant may
initially seek internal review of the determinations of the Clinical Director
regarding the Service Plan.
f) To participate in the development and operation of
the Clinical Directorate structure and in such management or representative
structures as are in place or being developed. The Consultant shall receive
training and support to enable him/her to participate fully in such structures.
g) To provide, as appropriate, consultation in the
Consultant’s area of designated expertise in respect of patients of other
Consultants at their request.
h) To ensure in consultation with the Clinical
Director that appropriate medical cover is available at all times having due
regard to the implementation of the European Working Time Directive as it
relates to doctors in training.
i) To supervise and be responsible for diagnosis,
treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating
patients under the Consultant’s care.
j) To participate as a right and obligation in
selection processes for non-Consultant Hospital Doctors and other staff as
appropriate. The Employer will provide training as required. The Employer shall
ensure that a Consultant representative of the relevant specialty /
sub-specialty is involved in the selection process.
k) To participate in clinical audit and proactive risk
management and facilitate production of all data/information required for same
in accordance with regulatory, statutory and corporate policies and procedures.
l) To participate in and facilitate production of all
data/information required to validate delivery of duties and functions and
inform planning and management of service delivery
Teaching Commitments
·
regular commitments to ward based teaching of
undergraduates.

participation in the lecture programmes for 2nd
and 4th & 5th year medical students.
·
teaching of other groups as required including other
specialty juniors and specialist nurses & FRCA candidates
A formal job plan will be agreed between the
appointee, the Operational Medical Director and Lead Anaesthetics- ICU/ ITU Consultant, on behalf of the Trust’s Medical
Director, based on the provisional timetable. This will be signed by the Chief
Executive and will be effective from the commencement date of the appointment.
It will be reviewed at 3 months & thereafter annually or at any time, but
no less than 3 months after a previous review, as requested by the appointee,
Operational Medical Director or Clinical Lead & adjusted accordingly to the
agreement of both parties.
Requirements
This post is open to doctors who hold Registration
as a Specialist in the Specialist Division of the Registrar of Medical
Practitioners maintained by the Medical Council in Ireland in the specialty of
Clinical and Anaesthetics-
ICU/ ITU.

Management Responsibility
All Consultants are required to attend the monthly
Directorate Meetings. Post holders will be expected to share in administrative
duties allocated by mutual agreement within the Directorate. As part of the
Consultant Team, the consultant will
offer mentoring support and lead the existing clinical team through; joint
clinics, one-to-one support, individual case management and complex cases,
identifying development needs and suggesting CPD (Continuous Professional
Development) opportunities and responding to clinical enquiries via e-mail and
telephone.
