Overview

 

About the Hospital

The hospital provides community, acute (hospital-based treatment) and specialist health services to around half a million people living in the Munster Region areas. The hospital also treat patients who choose to use our services from surrounding areas, including Cork, Clare, Kerry, Limerick, Tipperary, Waterford.

Since it was established in April 2002, the hospital has provided mainly hospital services. In 2010, our Trust also started providing community therapy services and intermediate care services. These services provide short-term specialist care to people who have been discharged from hospital but need extra support, care and rehabilitation before they go home or to the place where they normally live.


In April 2011, the hospital expanded to provide community health services for the district to include Adult Community Nursing and Children’s and Families’ Health Services to become a new integrated care organisation for local people.


The hospital completed a major hospital development programme in March 2011, which involved opening new, state-of-the-art hospitals in the region. Alongside the excellent modern hospital in Galway, these fantastic new buildings are helping to transform local patient care.

The Anaesthetic Department

The Anaesthetic Department is part of the Directorate for Theatres, Anaesthetics and Critical Care. Surgical services are located principally at two Acute Hospitals.

 

Hospital surgical services are provided with a 6 bedded Intensive Care Unit. The main theatre suite comprises six theatres, in addition to which, there are separate maternity theatres.

 

Anaesthetic services are also provided in Hospital. The three theatres available provide over-night and day-case surgery on this site. Anaesthetic services are not currently provided at our two other community hospitals in the region.

 

At the hospital there is an 12 bedded Intensive Care Unit, and 18 operating theatres situated in the main theatre suite and on the Day Unit. In addition there are two Post-Anaesthetic Care Units (PACUs) and we also provide services to Treatment Centre based off site. Surgical services are supported by the PACU (Post-Anaesthetic Care Unit), Anaesthetic and Emergency Theatre Teams.

 

Within the Structure the hospital have a Directorate Manager and Clinical Director responsible for the integrated Directorate and we also have a Deputy Clinical Director based on each site and team of three Deputy Directorate Managers. Also have a Lead Clinician for Intensive Care, along with Clinical Leads for Day Surgery, Acute Pain, Chronic Pain, Obstetrics, Paediatrics and CPX testing as well as a number of Anaesthetists who have links with the various specialties.

 

Surgical services currently supported include; General surgery (including Colo-rectal and Upper GI), Orthopaedics and trauma, Obstetrics and Gynaecology, ENT, Maxillo Facial, Oral surgery, Ophthalmology and Urology. Enhanced Recovery Programmes are well advanced in Orthopaedics and are being rolled out in other specialties. Preoperative assessment sessions are provided as well as a full nurse-led pre-assessment service.

 

Special Interest.

 

It is envisaged that the post holder will carry out their duties as a full time consultant Anaesthetist with dedicated sessions in their area of specialist interest.  The posts are contracted for 10 programmed activities per week, of which it is anticipated up to 3 sessions will be delivering the area of specialist interest.  The demands of the anaesthetic department mean that close and ongoing collaboration with consultant colleagues is required. There is likely to be flexibility of sessions given, although there may be a need to organise this around service development and reconfiguration. Some existing consultants work flexibly whilst others have fixed sessions, we can accommodate either approach; it is an exciting time new colleagues to make a real impact on the future direction of the anaesthetic service within the Trust.

 

Standard Duties and responsibilities


a) To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan and in line with policies as specified by the Employer.


b) To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.


c) To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.


d) To co-operate with the expeditious implementation of the Disciplinary Procedure 


e) To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.


f) To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.


g) To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.


h) To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.


i) To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.


j) To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 


k) To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.


l) To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery

Teaching Commitments

·       regular commitments to ward based teaching of undergraduates.

·       participation in the lecture programmes for 2nd and 4th year medical students.

·       teaching of other groups as required including other specialty juniors and specialist nurses & FRCA, MRCA candidates.

 

Job Plan

A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead
Anaesthetics Consultant, on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.


It will be reviewed at 3 months & thereafter annually or at any time, but no less than 3 months after a previous review, as requested by the appointee, Operational Medical Director or Clinical Lead & adjusted accordingly to the agreement of both parties.

 

The post is advertised as a whole time 10PA post, although less-than-full time applications or job sharing arrangements may be considered.

The indicative timetable, below, is based on 10 PA’s - actual job plans may vary according the number of PAs agreed and changing service configurations over time.

Additional PAs may be offered as the final timetable and job plan are developed in partnership with the successful candidate. This may include additional SPA* and/or DCC according to the skills that the candidate can bring to the service and changing service requirements and configurations.

* SPA: The Trust, in line with the Academy of Medical Royal Colleges, accepts that 1.5 SPA reflects the minimum time required to meet the needs of core consultant roles and revalidation; the Trust thus describes this essential time as “Core SPA”, and will be offered to all consultants.  Additionally agreed activities are described as Additional SPAwhich will be reviewed through job planning and time allocated against activities with a measurable outcome directly linked to agreed objectives.

Requirements

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of Clinical and Anaesthesia.

Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant  will offer mentoring support and lead the existing clinical team through; joint clinics, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.
