Overview

 

Our client are seeking to appoint a dynamic acute physician or speciality physician with an interest in acute medicine to join our team of six whole time equivalent acute physicians (additional posts may be available by the time of interview). Candidate’s dual trained in General Internal Medicine and speciality medicine would be welcome to apply and speciality sessions would be considered and accommodated where possible.

The hospital is a medium sized hospital, serving Connaught and surrounding areas. The hospital has approximately 450 inpatient beds, and provides a broad range of general medical, surgical and Women’s and Children’s services. With a busy A&E department that manages all medical, surgical and child health emergencies seeing 80,000 patients a year. The Trauma Unit is a registered Level 2 Unit and the department deals with all emergencies. As the local population grows (current catchment is 300,000), the hospital continue to develop their facilities.  In addition to providing general acute services, the University Hospital increasingly provides more specialist services such as cancer care and complex cardiology.

The Post on Offer

These appointments will strengthen our acute medical department in its aim to provide timely consultant review to the unselected medical take. They will also allow an excellent teaching programme to be delivered to University undergraduate medical students. Teaching will be central to all of the acute medical consultant’s job plans.

The hospital believes acute medicine to be hard but rewarding work that requires a balanced job plan allowing time for reflection, away from direct clinical care. This is essential for sustainability and to ensure we have the best diagnostic physicians working at the front door to ensure high quality patient care, junior doctor supervision and efficient service delivery.

 

Job Outline

The successful candidates would rotate through the following core shift patterns bringing welcome variety to the job:

 

  • ED Cover and floor physician (08:00 – 17:00 Monday to Friday) In the morning, takes responsibility for ensuring any medical patients within the ED receive a medical review after the night teams shift. Then is responsible to ensure every newly admitted medical patient receives a post take ward round and senior decision making in a timely fashion until 17:00. At 17:00 hands over to the on-call physician.

 

  • MAU cover (08:00 – 13:00 Monday to Friday) Works with an elderly care physician to ensure that a 27 bedded MAU / Short stay ward patients are seen on a daily basis and have a robust management plan.

 

  • 1:7 MAU Weekend service (Saturday and Sunday 09:00-13:00) weekend round. As above for MAU cover.

 

  • Ambulatory Emergency Care (AECU) (09:00-19:00 Monday to Friday). Providing an alternative to admission this consultant delivered service supported by junior doctors and nurses aims to provide access to inpatient facilities (IV treatments and diagnostics etc) but manages patients on a daily basis as an outpatient.

 

  • General medical on call (17:00-22:00 Monday to Friday and weekends 12 hours resident). On call by telephone the hospital is redesigning this rota based on sustainability and it will likely be different when the successful candidate is in place. This is currently 1:18 rota but weekend and out of hours work (including MAU rounds) will be capped at 1.5PAs.

 

  • Outlier cover (09:00-13:00 Monday to Friday) Our experience is that although we would like to cohort short stay acute medical patients on the short stay unit, operational pressures mean that short stay patients sometimes do move to outlier wards. Robust acute medical consultant cover to these areas ensures timely decision making.

 

Teaching: General physicians will be best placed to deliver large components of the undergraduate medical education at the University. The commitment from the acute medical department will be as follows: Year 1 & 2 – 40 sessions of introduction to systems based history and clinical examination skills requiring small group facilitation and bedside teaching. Senior Rotation (Year 4) – the acute care module will involve groups of 6 students rotating to the AMU every 4 weeks for an apprenticeship style attachment. This will involve a mixture of observation, junior and senior shadowing, small group case discussion and informal lectures. There will also be the opportunity to assume specific educational or organisational roles for those with an interest in medical education

Standard Duties and responsibilities

a) To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan  and in line with policies as specified by the Employer.

b) To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.

c) To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.

d) To co-operate with the expeditious implementation of the Disciplinary Procedure 

e) To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.

f) To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.

g) To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.

h) To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.

i) To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.

j) To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 

k) To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.

l) To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery

Teaching Commitments

·       regular commitments to ward based teaching of undergraduates.

·       participation in the lecture programmes for 2nd and 4th & 5th year medical students.

·       teaching of other groups as required including other specialty juniors and specialist nurses.

 

Job Plan

A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead
Acute Medicine Consultant, on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.

 

The job plan on appointment will consist of a split of 7.5:2.5 DCC/SPA.  The SPA component is made up of core activities (appraisal, CME, routine departmental meetings etc.) and non-core activities. Non-core activities are discussed and agreed within the Trust (via an agreed tariff and, on occasion, through ad personal agreement). Non-core SPA may reflect activities such as educational supervision, research, leadership and quality improvement. Following appointment, non-core SPA may be used to participate in our consultant development programme, or in acquiring or consolidating specific skills and expertise. Non-core SPA is only available if specific activities and objectives are mutually agreed in job planning.

 

Team Job Plan

 

This schedule outlines the Direct Clinical Care component (DCC) of the job plan. In addition the candidate will be on the General medial on call rota. Sessions marked* are an opportunity to deliver SPA activity and clinical admin. The amount required is outlined in the accompanying table and is delivered over the 7 week cycle rather than per week. There are 23 spare sessions per cycle to deliver the 11 sessions of SPA and clinical administration. The remaining 12 sessions can be taken as time off in lieu. It would be expected the successful candidate would organise their own timetable with regard to fitting this activity into the job plan.

 It is assumed the successful candidate will be available for work 42 out of 52 weeks per year taking into account study and annual leave. Prospective cover is calculated to ensure a fair job plan of 52 weeks of shifts can be done in 42 weeks inside a 10 PA job plan although there will be the opportunity to work extra sessions by mutual agreement.

Review of Job Plan


The job plan is subject to review once a year and is signed off between the post holder, the CSU Lead and an Executive Director of the Trust. Participation in annual appraisals is a contractual requirement. The Trust has procedures for resolving disagreements concerning job plan, either following appointment or at annual review.

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of Clinical and Acute Medicine

Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant  will offer mentoring support and lead the existing clinical team through; joint clinics, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.
