Overview


The Consultant Paediatric Emergency Medicine will join a team of 7 Paediatric Emergency 
Medicine Consultants at our clients hospital in the Connacht Region. There is a locum position available- which is separate from this position - Both comprise 10 programmed activities with a commitment to 1:6 weekends on call for the Emergency Department.


Applicants are required to be on the IMC Specialist Register for one of the following: General Paediatrics
 with subspecialty accreditation in Paediatric Emergency Medicine (or equivalent recognised competence based training), Emergency Medicine with dual accreditation in Paediatric Emergency Medicine (or equivalent recognised competence-based training)

 

The hospital is a large, multidisciplinary dedicated children’s hospital with a capacity of almost 300 beds. There are 11 operating theatres, including a separate burns unit theatre and endoscopy unit, 5 other anaesthesia delivery areas and a 31-bedded Intensive Care Unit. Kids’ Intensive care and Decision

Support (KIDS) is the regional transport and advice service for critically ill children in the Connacht Region. KIDS receives over 1200 referrals/year and transfers over 50% of these children to Paediatric Intensive Care Units at a nearby hospital and University Hospital and, in certain circumstances, out of the region of Connacht Region. The HSE Trust provides secondary level paediatric services for the child and adolescent population of much of the city, both in the hospital and through its community and clinical support services. It is also the regional paediatric referral centre for the Connacht Region and many of its specialist services extend their referral base beyond regional boundaries. The Trust is the regional paediatric Major Trauma Centre for the region


As well as general paediatrics and paediatric surgery, the clinical services provided by the trust include:

cardiology and cardiac surgery; endocrinology; ENT surgery and cochlear implantation; gastroenterology and hepatology; inherited metabolic diseases; neurosurgery; oncology and haematology; ophthalmology; orthopaedic and trauma services; spinal surgery; paediatric dentistry; plastic surgery (including cleft lip and palate work); respiratory medicine and cystic fibrosis; rheumatology; thoracic surgery. The hospital is a designated regional and supra-regional centre for craniofacial surgery, cardiothoracic surgery (hypoplastic left heart syndrome), dermatology (epidermolysis bullosa), endocrinology (Alstrom’s disease), paediatric burns, complex liver disease and liver transplantation, and is the national centre for paediatric small bowel transplantation.


There is comprehensive provision of diagnostic and support services including all aspects of paediatric

laboratory medicine, radiology and pharmacy. Therapy services include dietetics, physiotherapy, orthoptics,

occupational therapy, and speech and language therapy. Pastoral and social support is offered by a multi-faith

chaplaincy, the bereavement support team and by the social work department.

 

The department currently consists of:

 

• 3-bedded resuscitation area

• 10 examination cubicles (including 2 high dependency cubicles)

• Minor injuries area

• Treatment room

• 6-bedded Observation Unit (5 beds and 1 cubicle)

 

Work was recently completed on a major refurbishment and modernisation project to enhance the physical

environment of our ED.

 

There is full secretarial support and office accommodation, with access to full internet and IT facilities.


Clinical Duties (guideline only)

Provision with consultant colleagues of an Emergency Service for children and young people in the Connacht Region, with responsibility for the prevention, diagnosis and treatment of illness and injury, and the proper functioning of the department.

• Responsibility for the continuing care and treatment of patients in your charge, and personal performance of clinical duties as agreed in your job plan, allowing for all proper delegation to, and teaching of, trainee staff.
• Provision of an out-of-hours service with consultant colleagues.
• Responsibility for the professional supervision, training and management of trainee medical staff, carrying out teaching, examination and accreditation duties as required, and contributing to postgraduate and continuing medical education activity locally and nationally, as appropriate.
• Participation in all aspects of the Trust’s clinical governance processes.
• Participation in the Major Incident Procedure.
• Attendance at weekly departmental meetings and monthly business meetings.
• Responsibility for maintaining awareness of professional guidelines such as IMC ‘Duties of a Doctor’, and acting in accordance with them.

Standard Duties and responsibilities

a) To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan  and in line with policies as specified by the Employer.

b) To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.

c) To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.

d) To co-operate with the expeditious implementation of the Disciplinary Procedure 

e) To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.

f) To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.

g) To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.

h) To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.

i) To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.

j) To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 

k) To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.

l) To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery

Additional Duties

Each consultant within the ED team has their own individual roles and responsibilities, delivered as part of their allocated SPA time. For these new posts, the team are happy to encourage and support the specific interests of the appointed candidates, where such interests would enhance and improve the services within the department.

Teaching Commitments

·       regular commitments to ward based teaching of undergraduates.

·       participation in the lecture programmes for 2nd and 4th & 5th year medical students.

·       teaching of other groups as required including other specialty juniors and specialist nurses.

A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead Emergency Medicine Consultant, on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.


It will be reviewed at 3 months & thereafter annually or at any time, but no less than 3 months after a previous review, as requested by the appointee, Operational Medical Director or Clinical Lead & adjusted accordingly to the agreement of both parties.


Requirements

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of Clinical and Emergency Medicine.

Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant  will offer mentoring support and lead the existing clinical team through; joint clinics, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.
