Overview
The hospital is a modern purpose designed and built
facility which opened in the 1990’s.

The Emergency Department
The Emergency Department consists of majors (with
resuscitation room) and an urgent care centre (including minor injuries, minor
ailments and GP priority presentations). The resuscitation 5 room currently
comprises of 7 adult bays, Including 1 dual purpose negative pressure & 1
burns bay and 2 paediatric bays.
The ED has just completed a redevelopment of the
department doubling the footprint. The majors area in the department has
doubled from 11 to 21 trollies as well as a brand new designated Urgent Care
and Paediatric departments. A dedicated CT scanner along with two X-ray rooms
will be located in the new department. The redevelopment of the space was to
bring capacity in line with demand and prepare for changes in activity due to
Shaping a Healthier Future and so this is an exciting time to shape the future
of emergency care within the Trust.
The HSE hospital has just been given a rating of
‘Good’ in the areas of ‘Caring and ‘Well Led’ for the ED and ‘Requires
Improvement’ for ‘Safe’ and ‘Responsive’, identifying challenges around
staffing levels and capacity particularly, which will be addressed within the
new ED redevelopment.
Standard
Duties and responsibilities
a) To
participate in development of and undertake all duties and functions pertinent
to the Consultant’s area of competence, as set out within the Clinical
Directorate Service Plan and in line
with policies as specified by the Employer.
b) To ensure
that duties and functions are undertaken in a manner that minimises delays for
patients and possible disruption of services.
c) To work
within the framework of the hospital / agency’s service plan and/or levels of
service (volume, types etc.) as determined by the Employer. Service planning
for individual clinical services will be progressed through the Clinical
Directorate structure or other arrangements as apply.
d) To
co-operate with the expeditious implementation of the Disciplinary
Procedure
e) To
formally review the execution of the Clinical Directorate Service Plan with the
Clinical Director / Employer periodically. The Clinical Directorate Service
Plan shall be reviewed periodically at the request of the Consultant or
Clinical Director / Employer. The Consultant may initially seek internal review
of the determinations of the Clinical Director regarding the Service Plan.
f) To
participate in the development and operation of the Clinical Directorate
structure and in such management or representative structures as are in place
or being developed. The Consultant shall receive training and support to enable
him/her to participate fully in such structures.
g) To
provide, as appropriate, consultation in the Consultant’s area of designated
expertise in respect of patients of other Consultants at their request.
h) To ensure
in consultation with the Clinical Director that appropriate medical cover is
available at all times having due regard to the implementation of the European
Working Time Directive as it relates to doctors in training.
i) To
supervise and be responsible for diagnosis, treatment and care provided by
non-Consultant Hospital Doctors (NCHDs) treating patients under the
Consultant’s care.
j) To
participate as a right and obligation in selection processes for non-Consultant
Hospital Doctors and other staff as appropriate. The Employer will provide
training as required. The Employer shall ensure that a Consultant
representative of the relevant specialty / sub-specialty is involved in the
selection process.
k) To
participate in clinical audit and proactive risk management and facilitate
production of all data/information required for same in accordance with
regulatory, statutory and corporate policies and procedures.
l) To
participate in and facilitate production of all data/information required to
validate delivery of duties and functions and inform planning and management of
service delivery
Job Plan - 10PA
A detailed
work programme and a job plan will be arranged and agreed by the appointee with
the Clinical Director in Medicine and the Chief Executive of the Trust. The
post holders will undertake fixed clinical and non-clinical sessions according
to an agreed timetable with other members of the Consultant Team. The post will
be divided into a ratio 8/2 direct clinical care (DCC) / supporting programmed
activities (SPA). Specific SPA objectives and outcomes will be agreed with the
Service Lead within 3 months of appointment.
The current
consultant work pattern provides hands on Consultant ‘shop floor’ presence from
08:00 to 22:30 Monday to Friday, 8am to 16:00 Saturday and Sunday to directly
lead and guide the team to provide a high standard of clinical care for
patients attending the Emergency Department. With an increase in the consultant
numbers the shop floor hours covered by the consultant body as a whole may
increase. On call is covered one weekday per week and currently one weekend in
eight at 5% WTE remuneration. Annual and study leave is covered internally and
flexibility in sessions will be required on occasions to provide this level of
senior clinical supervision.
The
Department is busy, seeing over 112,000 patients per year (30% paediatric) and
we have achieved a high clinical standard of care and the 95% 4 hour target by
working as a close, cooperative team. 24 hour middle grade cover is provided
with 4 Senior Registrars posts in Emergency Medicine and 10 other Registrars
posts. At present there are 12 SHO posts (rotating every 4 months), 1 six month
SHO post on GP rotation, 3 six month EM trainees and 2 six month posts
incorporated in House Officer (CMT) and (AM). There are also 4 HO’s who work
predominantly in the Observation Unit. It is expected that the post holder will
take an active role in teaching and mentoring ED staff.
The minor
injuries/illness stream is supported by a 4 ENP team working 8am to 10 pm
daily. There is a 8 bedded observation unit within the department for patients
needing a prolonged period of observation, extended investigations or input
from the Acute Assessment Therapies Team (OT & Physio) to enable safe
discharge home. This unit is directly overseen by the Consultant on clinical
duty and ED Registrar overnight. PACS is installed and the department is very
well supported by the Radiology Department with ready access to all
investigations and prompt reporting.
The
department is fully computerised for patient attendances, investigations
(radiology & pathology) and results and discharge summaries. Within the
department we have Point of Care Testing for FBC, U&E and ABG and several
ultrasound machines. The fully integrated Urgent Care Centre opened in October
2010. Responsibilities of the Emergency Medicine Consultant will include
supervision & delivery of the UCC service.
On a day-to-day
basis during clinical shifts this will encompass supervision of the junior
doctors and provision of clinical support and advice where relevant to the UCC
service whilst also ensuring the provision of acute and timely treatment to
patients allocated to the UCC flow. Duties of the EM Consultant will include
participation in delivering the UCC performance targets and clinical governance
processes. The post holder is expected to work closely with our joint providers
in view of a recent renegotiation of the contract. The Paediatric area is
staffed by a separate team of senior and junior doctors employed within the
Women and Children’s Directorate. Teaching duties will include medical
students, sessions for the SHO’s and middle grade teaching on Tuesday morning
including preparation for MCEM and FCEM.
The post
holder will be required to participate in the Trust and Directorate’s Clinical
Governance activities, with a commitment to audit, appraisal for themselves, and
CPD, and regular appraisal and mentoring of other team members. It is expected
that the post holder will engage in specific management tasks and contribute to
monthly departmental meetings (sub-directorate and Clinical Effectiveness) and
hospital committees in agreement with the Service Lead and the Directorate
Policy Board.
Job plans in
excess of 10 PA’s will be subject to annual review to ensure that all activity
contained in the job plan is clinically and organisationally necessary.

A formal job plan will be agreed
between the appointee, the Operational Medical Director and Lead Respiratory
Consultant, on behalf of the Trust’s Medical Director, based on the provisional
timetable. This will be signed by the Chief Executive and will be effective
from the commencement date of the appointment.
It will be reviewed at 3 months
& thereafter annually or at any time, but no less than 3 months after a
previous review, as requested by the appointee, Operational Medical Director or
Clinical Lead & adjusted accordingly to the agreement of both parties.

Requirements
This post is open to doctors who
hold Registration as a Specialist in the Specialist Division of the Registrar
of Medical Practitioners maintained by the Medical Council in Ireland in the
specialty of Clinical and Emergency Medicine.

Management
Responsibility
All Consultants are required to
attend the monthly Directorate Meetings. Post holders will be expected to share
in administrative duties allocated by mutual agreement within the Directorate.

through; joint clinics, one-to-one support, individual case management and
complex cases, identifying development needs and suggesting CPD (Continuous
Professional Development) opportunities and responding to clinical enquiries
